The provisions of this section shall apply only to United States flag vessels. Manned tank barges and tankships authorized to carry Grade A, B, C, or D liquids at any temperature, or Grade E liquids at elevated temperatures, shall be provided with a combustible gas indicator suitable for determining the presence of explosive concentrations of the cargo carried. An indicator which bears the label of Underwriters' Laboratories Inc., Factory Mutual Engineering Division, or other organizations acceptable to the Commandant will be accepted as meeting this requirement. 1) The portable combustible gas meter O2 sensor was faulty; and 2) The silver span gas cylinder P/N 600-0050-007 was past its use by date of Nov 2024, preventing determining the suitability of the meter to detect the presence of explosive concentrations of the recovered cargo. 1) Replace in-kind the faulty O2 sensor; and 2) replace in-kind the expired span gas cylinder to the satisfaction of the attending marine inspector. 46 CFR 35.30-15

Fuses must be of the cartridge type only and be listed by Underwriters Laboratories or another independent laboratory recognized by the Commandant. The Sea-Fire Marine Fire and Detection and Monitoring system overcurrent fuse protection is field installed blade type fuses. Replace with approved UL cartridge fuses of the same amperage to the satisfaction of the attending marine inspector. 46 CFR 183.380(l)

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. The Pro Nautic 12.30 P and the Chargemaster 12/35-3 battery chargers are missing, the manufacturers required overcurrent fuse protection. Install fuses as described by 46 CFR 183.380(l) of the correct amperage in accordance with the battery charger manufacturer’s installation instructions to the satisfaction of the attending marine inspector. 46 CFR 183.200(b)

A lighting fixture must be installed as follows: (1) Each fixture must comply with 183.200; 2) Each lighting fixture and lamp holder must be fixed. A fixture must not be supported by the screw shell of a lamp holder. Lighting fixture laying on top of generator switchboard with exposed electrical components and not fixed. 1) Mount lighting fixture in accordance with prescribed regulation; or 2) Decommission the lighting fixture all to the satisfaction of the marine inspector. 46 CFR 183.410(c)(1)

S and L. Each major generator and motor shall be fitted with a nameplate of corrosion-resistant material marked with the following information as applicable: (1) Name of manufacturer. (2) Manufacturer's type and frame designation. (3) Output in KW of watts or horsepower rating. (4) Kind of rating (continuous, intermittent, etc.). (5) Revolutions per minute at rated load. (6) Amperes at rated load. (7) Voltage. (8) Frequency if applicable. (9) Number of phases, if applicable. (10) Type of winding (for direct-current motors). Port generator unit nameplate information is not legible. Make the nameplate legible to the satisfaction of the attending marine inspector. 46 CFR 183.10-5(a) Old “T”

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Crew pulls the Sea-Fire Marine Fire and Detection and Monitoring system over current fuse protection with needle nose pliers when vessel is unmanned to prevent a low power alarm and the batteries from losing their charge. Provide a means from isolating the battery power to the Sea-Fire Marine Fire and Detection and Monitoring system without requiring the crew to pull the fuses to the satisfaction of the attending marine inspector. 46 CFR 183.200(b)

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. A first-aid kit must be easily visible and readily available to the crew. The first aid kit had expired aspirin, iodine swabs, and burn gel. Replace the expired contents in accordance with the above regulation. 46 CFR 184.710

Distribution panel boards shall be the safety type. Lounge feed distribution panel LP 104 circuit breaker opening covered with electrical tape. Seal opening with an approved circuit breaker blank cover to the satisfaction of the attending marine inspector. 46 CFR 183.10-10(d) Old “T”

The life raft manufacturer shall make operating instructions and information for the ship's training manual available in English to purchasers of inflatable life rafts approved by the Coast Guard, to enable vessel operators to meet SOLAS Chapter III, Regulation 35 (III/35). Posted placard displayed near installations of life rafts on vessels included a step for deploying an embarkation ladder. Provide objective evidence from the life raft manufacturer that the current placard with the embarkation ladder is the correct one for this model life raft and installation arrangement to the satisfaction of the attending marine inspector. 46 CFR 160.151-59(a)

The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing the actions to be taken in the event of fire, heavy weather, or man overboard conditions. Missing the emergency instructions posted at the operating station. Post the instructions at the operating station with the format prescribed by 46 CFR 185.510 to the satisfaction of the attending marine inspector. 46 CFR 185.510(a)

All tank vessels are subject to the regulations contained in subchapter J (Electrical Engineering) of this chapter except as such regulations are modified by the regulations in this subchapter for tank vessels. Vessel hazardous area is Zone 2. Rudder feedback unit and electrical cable/gland are not hazardous locations and explosive atmosphere certified. Provide rudder feedback unit and electrical cable/gland that are compliant with 46 CFR 111.105-3 and the parts of 46 CFR 111.105 applicable to the vessel, to the satisfaction of the attending Marine Inspector. To be resolved during the vessel's next dry dock in April of 2027. 46 CFR 31.35-1

This subpart applies to installations in hazardous locations as defined in Articles 500 through 505 of NFPA 70, Clause 6 of IEC 60092-502:1999 or Clause 8 of IEC 61892-7:2019. Vessel hazardous area is Zone 2. Provide documentation the rudder feedback electrical equipment and deck LED lighting equipment is not required to meet the above. If the equipment must meet the above, make compliant in accordance with 46 CFR 111.105-3 and the parts of 46 CFR 111.105 applicable to the vessel, to the satisfaction of the attending Marine Inspector. 46 CFR 111.105-1

Each generator must be protected by an over-current device set value not exceeding 115 percent of the generator full load rating. Both generators have an over-current device are 125A, which exceeds the 115 percent threshold. Replace the 125A over-current device for both generators, not to exceed the 115 percent threshold.

Each steering gear feeder circuit must be protected by a circuit breaker that meets the requirements of §58.25-55 in sub-chapter F of this chapter. Each feeder circuit for steering must be protected by a circuit breaker on the switchboard that supplies it and must have an instantaneous trip set at a current of at least— (1) 300% and not more than 375% of the rated full-load current of one steering-gear motor for a direct-current motor; OR (2) 175% and not more than 200% of the locked-rotor current of one steering-gear motor for an alternating-current motor. Marine inspector was unable to verify that the circuit breaker on the switchboard that supplies it had an instantaneous trip. Prove that the circuit breaker on the switchboard has an instantaneous trip, or replace to the meet the requirements of this regulation.

The minimum capacity of the power fire pump shall be 50 gallons per minute at a pressure of not less than 60 pounds per square inch (PSI) at the pump outlet. The pump outlet shall be fitted with a pressure gage. During operational test of fire pump, pressure gage at outlet reading observed was 47 psi. Achieve required 60 psi at pump outlet to the satisfaction of the attending Marine Inspector (MI). 46 CFR 181.10-1 (e) (Old T-L)

Where liquid level indicating gages are installed, they shall be of heat resistant materials, adequately protected from mechanical damage and provided at the tank connections with devices which will automatically close in the event of rupture of the gage or gage lines. Diesel fuel tanks liquid level indicating gages are plastic and the tank connection valves are manual gate valves. Install to the satisfaction of the attending MI liquid level indicating gages that are heat resistant and tank connections with devices which will automatically close in the event of rupture of the gauge or gauge lines. 46 CFR 182.20-22(c)(5) Old T

Except as otherwise provided in this paragraph all unfired pressure vessels shall be Coast Guard approved and installed to the satisfaction of the Officer in Charge, Marine Inspection. The requirements for design, construction and original testing of such approved unfired pressure vessels are in applicable regulations in Subchapter F (Marine Engineering) of this chapter. Information concerning the applicable regulations may be obtained from the Officer in Charge, Marine Inspection. Unfired pressure vessel relief valve settings check is due. Installed relief valve has an unknown test/service date. Service/test relief valve. Also, the unfired pressure vessel is missing the pressure gauge. Install pressure gauge. Once complete schedule and test the unfired pressure vessel relief valve in the presence of the attending MI. 46 CFR 182.10-5(d)

An adult life jacket must be provided for each person carried on board a vessel. One adult life jacket not serviceable. Vessel is restricted to 19 persons total until life life jacket is replaced to the satisfaction of the attending MI. 46 CFR 180.71(a)

Battery terminals shall be of the soldered type and all connections shall be made to the battery terminals with permanent type connectors. Spring clips or other temporary type clamps are not permitted. One battery in the stbd aft battery bank had wing type terminal connector. Replace with hex nut to the satisfaction of the attending MI. 46 CFR 183.05-20(c) (Old T)

In addition to meeting the requirements of paragraphs (a) through (e) of this section, a vessel with a power driven main steering gear must be provided with the following: An independent rudder angle indicator at the operating station. Rudder angle indicator not reading the correct angle of rudder. Repair and prove proper operation to the satisfaction of the attending MI. 46 CFR 182.610(f)(2)

All escape hatches and other emergency exits used as means of escape must be marked on both sides in clearly legible letters at least 50 millimeters (2 inches) high: “EMERGENCY EXIT, KEEP CLEAR”, unless such markings are deemed unnecessary by the cognizant [name withheld] room emergency escape scuttle marking is not clearly visible. Mark this escape scuttle in such a manner so the marking is clearly visible to the satisfaction of the attending MI. 46 CFR 185.606

A vessel is inspected for compliance with the standards required by this subchapter. Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. The hot water heater and the head tank for the hot water system is not secured sufficiently creating a hazardous condition. Mount the lower section of the hot water heater and the head tank to the satisfaction of the attending MI. 46 CFR 176.800

A vessel is inspected for compliance with the standards required by this subchapter. Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. Port reduction gear pilothouse pressure gauge inoperable. Repair and prove proper operation tot he satisfaction of the attending MI. 46 CFR 176.800

Each survival craft must be secured to the vessel by a painter with a float-free link permanently attached to the vessel. Vessel's life rafts were not arranged in a float free arrangement. Reconfigure stowage arrangement so that it is float free. 180.130(a)(1) 17C

Each ring life buoy on a vessel must be approved in accordance with 160.050 in subchapter Q. Grab line is brittle and is disintegrating on both life buoys. Provide operable grab lines for ring buoys. 46CFR180.70(b)(1) 16C

Electrical equipment on a vessel must be installed and maintained to: (a)Provide services necessary for safety under normal and emergency conditions. 46 CFR183.01-15 Old T. Vessel has modified various electrical circuits. Submit current electrical configuration.

Vessel must be provided with bilge pumps in accordance with Table 46 CFR 182.520(a). Vessel overboard valve for bilge pump stuck in open position.

The construction and arrangement of berths and other furniture must allow free and unobstructed access to each berth. Remove all gear in front of rack/berthing. Remove all gear on the racks/berthing and store properly. Racks were used for storage.

An independent modular smoke detecting unit must:(1) Meet UL 217 and be listed as a single station smoke detector. Prove proper operation of each smoke detector in conjunction with detector panel.

Throttle controls for the bridge are not fasted to the control console.

Conduct DAPI Audit

A fire hose with a nozzle must be attached to each fire hydrant at all times. Both fire hoses had numerous pin hole leaks near the hydrant attachment point.

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces: (2) a machinery space bilge, bilge well, shaft alley bilge, or other spaces subject to flooding from sea water piping within the space. The bilge high water panel at the operating station is inoperable.

No manned ship subject to this part may operate unless it carries on board a shipboard oil pollution emergency plan approved by the Coast Guard...if the Coast Guard determines that the plan meets the requirements of this section, the Coast Guard will issue an approval letter. The approval period for a plan expires 5 years after the approval date. The SOPEP onboard expired on 20JUN2020.

By March 15 of the year following the collection of the data in paragraph (a) of this section...a consortium or other employer representative may submit data for a marine employer. MIS report was not onboard the vessel for review.

Insufficient number of distress flares on board. Had 8 hand red distress flares and 3 hand orange smoke flares. Required 6 of each or 12 hand red distress flares as substitute.

Fire main pressure gauge inoperable.

Make permanent repairs to steering system and conduct operational test in the presence of marine inspector

Missing mechanical plugs for containment.

Load Line certificate expired

Prove proper operation of compass light.

Anchor chain broken and anchor not present.

Provide fire dectection for all accomadation spaces.
