A portable hand bilge pump must be capable of pumping water, but not necessarily simultaneously, from all watertight compartments; and provided with suitable suction hose capable of reaching the bilge of each watertight compartment and discharging overboard. The portable bilge hand pump was inoperable. 46 CFR 182.520 (b) Code 16C.

The master shall conduct sufficient drills and give sufficient instructions to make sure that all crew members are familiar with their duties during emergencies that necessitate abandoning ship or the recovery of persons who have fallen overboard. Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include the following information:(1) Date of the drill and training; and(2) General description of the drill scenario and training topics. No drill log was found on board. 46 CFR 185.520(a)&(f) Code 16C.

The main steering gear must be designed so that transfer from the main steering gear or control to the auxiliary means of steering required by § 182.620 can be achieved rapidly. Any tools or equipment necessary to make the transfer must be readily available. Master was unaware on how to operate the emergency steering hand tiller, including that the steering hydraulics shall be disconnected prior operating the hand tiller. 46 CFR 182.610/ 46 CFR 182.620/ Corrected on the spot

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. The belt installed in the main diesel engine that is also connected to the fire pump was lose, and found skipping a pulley.

All passenger accommodations must be arranged and equipped to provide for the safety of the passengers in consideration of the route, modes of operation, and speed of the vessel. Wood rot was identified in the bottom part of one starboard side window. Crop and renew. 46 CFR 177.800(a).

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI (U.S> Coast Guard). A repairs that was not approved by the U.S. Coast Guard was found at a section of the deck, below the starboard side windows, and adjacent of the rub-rail was found with exposed wood and not properly repaired. Crop, renew and repair to the satisfaction of an attending USCG Marine Inspector. 46 CFR 176.700(a).

Vessel received special consideration letter P433-14 on 05AUG15 allowing for the installation of 04 submersible bilge pumps instead of maintaining an installed bilge system. Inspectors identified 02 of the 04 bilge pumps (the 02 forward most) did not actuate. Vessels is to rectify and provide proof of proper. 46CFR182.25-10.

The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector or TPO, as applicable. If any of the equipment or records have not been properly maintained, a qualified servicing facility must perform the required inspections, maintenance procedures, and hydrostatic pressure tests. A tag issued by a qualified servicing organization, and attached to each extinguisher, may be accepted as evidence that the necessary maintenance procedures have been conducted. Vessel has in engine space a pre-engineered firefighting systems as rewuired by 46 CFR 181.115 since the vessel is "old T". Servicing tag on system did not denote the date serviced only the month of April and no proof of servicing could be provided for that time frame. Vessel is to provide proof of servicing. 46CFR142.240.

Electrical equipment on a vessel must be installed and maintained to: (a) Provide services necessary for safety under normal and emergency conditions; (b) Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock; (c) Minimize accidental personnel contact with energized parts; and (d) Prevent electrical ignition of flammable vapors. Vessel reported smoking from engine compartment and observed charred cabling from battery installations. Vessel to diagnose condition of electrical installation and submit report and repair proposal prior to beginning work. 46 CFR 183.200 Owner/operator must provide adequate overcurrent protection and adequately sized wire gauging as found inadequate during electrical inspection conducted 20 June 2023 by MeSPA, Inc.

In addition, the owner shall, prior to receiving a Certificate of Inspection, submit for approval to the cognizant OCMI....Electrical installation including, but not limited to: (i) Elementary one-line diagram of the power system; (ii) Cable lists; (iii) Bills of materials; (vi) Power, lighting, and interior communication panel boards with number of circuits and rating of energy consuming devices; (vii) Type of capacity of storage batteries; (ix) Electrical plant load analysis. Vessel does not have a current one line diagram and over the years has had much work done. Operator shall work to identify the items listed. Owner/operator must provide accurate one-line diagram. 46 CFR 177.202 (b)(4)

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Diesel pump and gasketing was damaged causing leakage. Repair to attending marine inspector satisfaction and conduct a sea trial before returning to service.

Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. The wet exhaust piping was found leaking into steering compartment bilge.

An exposed hazard, such as rotating machinery, must be properly protected by a cover or guard. A guard is required for the alternator belt on the engine.

The cognizant OCMI may require a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. While testing the fire pump, several leaks in the fire hose were identified. Replace fire hose.

A hatch exposed to the weather must be watertight. Gaskets on steering compartment hatch and tiller access port requires replacement.

A shutoff valve must be fitted in the fuel supply line at the tank connection to stop the flow of fuel. The fuel shutoff valve does not fully close when operated.

Replace shaft.

Replace/repair propeller.

Replace forward-most keel bolt.

Remove additional keel bolt and make available for inspection.

Replace both keel bolts at strut support assembly.

Repair dry dock plug at forward engine compartment space.

Replace main engine water intake through-hull valve.

Remove all through-hull fittings, including strainers and filters, and make available for inspection.

Repair hull damage (hole), 1" diameter x 1" deep 2FT forward from keel.

Repair hull damage: 02 holes mid port quarter.

Repair hull damage (hole): 1.5' forward from keel.

Repair forward keel damage (wood rot) surrounding keel bolt.

Remove aft keel bolt supporting strut assembly and make available for inspection.

Remove propeller, shaft, and rudder and make available for inspection (clean).

Replace cutlass bearing.

Repair damage at stbd quarter below through-hull exhaust outlet. (Transom)

Repair steering compartment access hatch (not watertight).

Remove all paint from steering compartment access hatch and make available for inspection. There are signs of wood rot present at deck support beam #3 (2" x 4").

Repair/replace 1" x 4" wooden support plank used to support hydraulic steering piping (at port side aft).

Replace rusted-out engine mount bolts. Make the double nut.

Repair/replace wooden battery box cover (wood rot).

Missing lagging on MDE exhaust piping

Fire pump Ball valve is suffering from dezinctification.

Vessel is 34.5ft with a 50ft hose. the 50 ft hose is too long to be effective in a fire and is being replaced with a 25ft hose.

Properly correct 2 water leaks located at the suction and discharge sides of the fire pump.

Provide drills & Training logs

Replace Fire Pump Pressure Gauge.

Provide an approved type first-aid kit.

VHF Radio must be upgraded to VHF-DSC Class A Radio which should be connected to a GPS receiver or have a VHF-DSC Radio with integral GPS. IAW 47 CFR 80.905(a)(1)
