To qualify for a medical certificate, a mariner must provide evidence of meeting the medical and physical standards in this section on a CG–719–K or CG–719–K/E, as appropriate. The Coast Guard retains final authority for determining whether a mariner is medically and physically qualified. Columns 2 through 5 of Table 1 to paragraph (a) of this section provide the specific exam, test, or demonstrations required to obtain the corresponding credential listed in column 1. Further clarifications of the requirements contained in the table are found throughout this subpart. Vessel Master could not locate medical certificate. Provide proof of valid medical certificate.

You are responsible for all actions of your officials, representatives, and agents (including service agents) in carrying out the requirements of the DOT agency regulations. Vessel did not have valid CY24 enrollment with service agent for chemical testing. Provide proof of CY24 Chemical testing program enrollment with crew list.

Every vessel subject to Part III of Title III of the Communications Act must have a detailed inspection of the radio installation by an FCC-licensed technician in accordance with § 80.59 once every five years. The FCC-licensed technician must use the latest FCC Information Bulletin, How to Conduct an Inspection of a Small Passenger Vessel. If the ship passes the inspection, the technician will issue a Communications Act Safety Radiotelephony Certificate. Vessel's Radiotelephony Certificate expired on 16Feb23. Provide valid Radiotelephony Certificate to attending marine inspector.

Fire, abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include the following information: (1) Date of the drill and training; and (2) General description of the drill scenario and training topics. Logs recording drills, training and inspection and testing of lifesaving and firefighting equipment were not available during inspection. Vessel could only provide maintenance and drill logs dating back from Jun23. Epirb logs were also missing. Provide logs showing completion of drills, training and inspections to attending marine inspector.

The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing the actions to be taken in the event of fire, heavy weather, or man overboard conditions. The emergency instructions must be designed to address the particular equipment,arrangement, and operation of each individual vessel. Vessel specific emergency instructions were not posted at the operating station. Provide evidence of emergency instructions posted at operating station to attending marine inspector.

An adult life jacket must be provided for each person carried on board a vessel. (1) life jacket was removed from service due to cracking and foam ripping off from the shoulder. Replace life jacket with 46 CFR 180.71(c) approval standard.

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. (2) burn gels and (1) antiseptic moist towelettes were expired. Replace all expired medical equipment.

Vessels described by 46 CFR 175.110(c) must have an interconnected fire detection system in compliance with § 181.450 installed in all enclosed areas where passengers and crew have routine access, including accommodation spaces and machinery spaces. Vessel did not install an interconnected fire detection system by 27DEC2022. Vessel is restricted to a LIMITED COASTWISE route until it's compliance with the interconnected fire detection requirements of 46 CFR 181.405 have been verified by a USCG marine inspector.

Provide evidence that all crew members are enrolled in drug testing program.

Provide complete and current first aid kit.

Provide (03) 2 1/2 gallon fire buckets.

Get vessel U/W & conduct MOb, Fire & Steering drills.

Prove proper operation of SSDG.

Install blank cover in electrical panel.

Provide two new brackets for fire extinguishers in accomodation space.

Renew EPIRB hydrostatic release by the end of March 2018.

Update first aid kit.

Provide copy of pre-employment test for new deckhand and update covered employee list.

Re-register EPIRB with correct EPIRB beacon ID#.

Label fire dampers for E/R.

Post COI for passenger visibility/access

Install grab rail/hand hold on top of house to allow safe access to life floats and EPIRB.

Re-rig line connecting water light to ring buoy to allow quick removal of water light.
