Every oil fuel pipe, which... would allow oil to escape from a storage, settling or daily service tank... shall be fitted with a cock or valve directly on the tank capable of being closed from a safe position outside the space concerned in the event of of a fire occuring in the space in which such tanks are situated. PSCO observed a bolt lodged in between a quick closing valve on the diesel oil service tank located in the vessel's fuel oil purifier (FOP) room, which inhibited the valve from actuating as designed in the event of a fire.
Condition: Placed in Improper Service

Resolution: Chief Engineer immediately removed the bolt and provided records of recent maintenance and operational tests of all remote tank valves and could verify the valve had been tested during fuel change over procedures to ensure the valve is operational.
