The electrical power available shall be sufficient to supply all those services that are essential for the safety in an emergency, due regard being paid to such services as may have been operated simultaneously. The emergency source of electrical power shall be capable, having regard to starting currents and the transitory nature of certain loads, of supplying simultaneously at least the following services for the periods specified hereinafter (section 2.1-2.6), if they depend upon electrical source for their operation. PSCO observed independent fuel supply level at .2 meters below required level. Fuel supply level must be maintained at 1.2 meters to be able to supply emergency systems throughout required time period.

Doors and door frames in "B" class divisions ventilation openings may be permitted in the lower portion of such doors. Where such openings is in or under a door the total net area of any such opening or openings shall not exceed 0.05 m2. When such opening is cut in a door it shall be fitted with a grill made of non-combustible material. PSCO observed that method to secure the lower portion of the ventilation grill was found to be missing and rendering the closure inoperable.

Doors and door frames in "B" class divisions and means of securing them shall provide a method of closure which shall have resistance to fire equivalent to that of the divisions. PSCO observed multiple B15 doors that were not able to secure due to the latching issue, the following doors were noted (OFFICE 3E082, CREW MESS 3G094).

The company should establish procedures, plans and instructions, including checklist as appropriate, for key shipboard operations concerning the safety of the personnel, ship and protection of the environment. PSCO observed checklist for fuel transfer operations was logged prematurely, checklist section for completion of transfer was logged completed while fuel was actively being transferred.

When the fire-extinguishing medium is stored outside a protected space, it shall be stored in a room which shall be situated in a safe and readily accessible position and shall be effectively ventilated to the satisfaction of the administration. PSCO noted damage to the blet on the belt driven ventilation fan in the CO2 space.

Escape routes shall be maintained in a safe condition, clear of obstacles. In spa area, which was recently modified, corridors were furnished with non-fixed seating in way of reception desk.74SOLAS II-2/13.1.2

At least two widely separated and ready means of escape shall be provided from all spaces. The secondary escape from the gym is not labeled and contains door with locking mechanism. 74SOLAS, II-2/13.2.1

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board.Ship reported loss of approximately 35 liters of stern tube oil for unknown reasons.

To assist in casualty investigations ships shall be fitted with a voyage data recorder. The VDR on board had an issue with the hard drive and can't record data for two newly installed fire screen doors or water depth. 74 SOLAS (2014) V/20.1

1410-Propulsion Main Engine- The condition of the ship and its equipment shall be maintained to conform with the provisions of the regulating to ensure that the ship will remain fit to proceed to sea without danger to the ship or persons on board. Vessel experienced a complete loss of ships power and propulsion. The ship was operating with three of four diesel generators online. All three generators tripped offline without advance warning- 74SOLAS(04) I/11

1430-Auxilliary Engines- The electrical power available shall be sufficient to supply all the services essentialfor safety in an emergency with due regard for services that must be operated simultaneously following the vessel's loss of ship service power and propulsion, the emergency generator successfully started but subsequently experienced a high temperature condtion that rendered the generator inoperable- 74SOLAS(04) II-1/42.2

74 SOLAS (2001 cons) Ch. II-2/15.2.5 - Every oil fuel pipe, which if damaged would allow oil to escape from a storage settling or daily service tank situated above the double bottom shall be fitted with a vavle capable of being closed from a safe position outside the space. Quick closing valve CTT2164 did not properly close.

74 SOLAS (2001 cons) Ch. II-2/18.1.1 - Where "A" class divisions are penetrated for the passage of electrical cables, pipes, trunks, etc. shall be made to ensure that the fire resistance is not impaired. Copper gauge line running through the bulkhead at frame 116 from the laundry room to the chiller room on stbd side is not properly insulated.

0299 - Every crew member with assigned emergency duties shall be familiar with these duties before the voyage begins. While conducting abandon ship drills, several commanders and vice commanders were not familiar with emergency steering and the location of fuel shut off valves. 74 SOLAS 14 (Cons.), CH. III, Reg. 19.2.1.

0616 - All life boats shall be provided with a tiller capable of controlling the rudder in case of failure of the steering mechanism. Port side #6 life boat tiller was found inoperable due to loose fulcrum. 74 SOLAS (97 Cons.), CH. III, Reg. 3.7.2.

0715 - Any required fixed fire detection system and fire alarm system with manually operated call points shall be operable at all times. Manual call point near fire screen door 0006.1 was not accessible: blocked by fixed structure. 74 SOLAS (97 Cons.), CH. II-2, Reg. 13.1.1.

0960 - At least one means of escape required below the bulkhead deck shall consist of a readily accesible enclosed stairway which shall provide continuous fire shelter. Laundry space escape door 0006.1 would not fully close due to missing hardware. 74 SOLAS (97 Cons.), CH. II-2, Reg. 28.1.5.

0611 - Equipment of every life boat shall consist of portable fire extinguishing equipment suitable for extinguishing oil fires. Port side #6 life boat found with inoperable extinguisher. 74 SOLAS (97 Cons.), CH. III, Reg. 41.8.28.

0799 - "A" class divisions shall be so constructed as to be capable of preventing the passage of smoke and flame to the end of the one-hour standard fire test. Fire screen door 074-18 was found with penetration located at the key hole. 74 SOLAS (97 Cons.), CH. II-2, Reg. 3.3.3.

FIRE FIGHTING SYSTEMS AND APPLIANCES SHALL BE KEPT IN GOOD WORKING ORDER AND READILY AVAILABLE FOR IMMEDIATE USE. NUMBER ONE MAIN FIRE PUMP IN INOPERABLE. Number one fire pump is inoperable, electrical motor was found with shorted windings. Unable to lower lifeboats or conduct liferaft inflation due to moored bunker barge. Complete lifeboat and liferaft launch at next US port call in the presence of the USCG.

Launching Davit (port side) not in good working order. During life raft deployment cable jumped from upper pulley, causing davit to become inoperative. Immediate corrective actions were taken to correct davit deficiency, 01 life raft successfully deployed and load tested. Provide class acceptance of davit arraignments at upper pulley.
