An inflatable liferaft or inflatable buoyant apparatus must be serviced at a facility specifically approved by the Commandant for the particular brand, and in accordance with servicing procedures meeting the requirements of part 160, subpart 160.151, of this chapter—(1) No later than the month and year on its servicing sticker affixed under 46 CFR 160.151-57(n). Vessel's IBA was being serviced at the time of inspection. Install serviced life raft to clear deficiency.

Each life jacket carried on a vessel on oceans, coastwise, or Great Lakes route, must have a life jacket light approved in accordance with § 161.012 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.112. Each life jacket light must be securely attached to the front shoulder area of the life jacket. Vessel is restricted to a limited coastwise route until life jackets lights are installed on all life jackets.

Each hydrostatic release unit, other than a disposable unit, must be serviced:(b) Each disposable hydrostatic release unit must be marked with an expiration date of two years after the date on which the unit is installed. EPIRB hydrostatic release not marked with expiration date. Install date unknown. Replace EPIRB hydrostatic release and mark expiration date for 2 years after install.

For fixed-gas fire extinguishing systems, the inspections and tests required by Table 176.810(b) of this section, in addition to the tests required by 46 CFR 147.60 and 147.65. The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. Provide recent JOY FIRE servicing record for the engineroom fixed fire extinguishing system.

This part contains requirements for the design, construction, installation, and operation of propulsion and auxiliary machinery, piping and pressure systems, steering apparatus, and associated safety systems. Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Machinery space fuel oil return manifold valves were leaking diesel fuel oil. Inspect hoses and valves, determine source of fault and repair or replace.

The flexible section must be accessible and as near the upper end of the fill pipe as practicable. When the flexible section is a nonconductor of electricity, the metallic sections of the fill pipe separated thereby must be joined by a conductor for protection against generation of a static charge when filling with fuel. Flexible hose from both fuel tanks to deck fill fittings were missing conductor. Install conductor to electrically join metallic sections of tank to metallic section of fill pipe for both fuel oil tank fills.

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules, except that a vessel of more than 19.8 meters (65 feet) in length must also have navigation lights that meet UL 1104, “Standards for Marine Navigation Lights,” or other standard specified by the Commandant. Stern light mounting was not secured causing light to angle. Repair or replace stern light mounting to appropriate angle.

EAP education program: Each EAP education program must include at least the following elements: display and distribution of informational material; display and distribution of a community service hot-line telephone number for crewmember assistance, and display and distribution of the employer's policy regarding drug and alcohol use in the workplace. Community service hot-line telephone number was not available or displayed. Post hot-line telephone number for crew member assistance.

Each floating waterlight must be approved in accordance with § 161.010 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.110. Waterlights on ring life buoy and life floats were found not in compliance with manufacturer instructions for battery replacement.

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. First-Aid Kit contained expired items and was missing directions for the use of the First-Aid kit. Maintain a First-Aid Kit compliant with the requirements in 46 CFR 160.041.

Life floats and buoyant apparatus must not be stowed in tiers more than 4 feet high. When stowed in tiers, the separate units must be kept apart by spacers. No spacers were installed between life floats. Place spacers between life floats.

Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include the following information: (1) Date of the drill and training; and (2) General description of the drill scenario and training topics. Vessel had no record of abandon ship drills. Conduct and document abandon ship drill.

Marine employers shall establish programs for the chemical testing for dangerous drugs on a random basis of crewmembers on inspected vessels who: (1) Occupy a position, or perform the duties and functions of a position, required by the vessel's Certificate of Inspection; (2) Perform the duties and functions of patrolmen or watchmen required by this chapter; or, (3) Are specifically assigned the duties of warning, mustering, assembling, assisting, or controlling the movement of passengers during emergencies. One crew member was not listed on the company’s covered crew member list. Provide documentation showing crew member was subject to pre-employment testing and is enrolled in a random testing program prior to the crew member serving in a position described above.

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. Every vessel subject to Part III of Title III of the Communications Act must have a detailed inspection of the radio installation by an FCC-licensed technician in accordance with § 80.59 once every five years. The FCC-licensed technician must use the latest FCC Information Bulletin, How to Conduct an Inspection of a Small Passenger Vessel. If the ship passes the inspection, the technician will issue a Communications Act Safety Radiotelephony Certificate. Vessel did not have a current FCC Safety Radiotelpephony certificate on board.

Each survival craft must be stowed so that when the vessel sinks the survival craft floats free. Life float water lights are mounted under the pilot house overhang in a manner which could bind upon float free release. Move lights to facilitate float free arrangement.

The emergency instructions must be designed to address the particular equipment, arrangement, and operation of each individual vessel. Emergency instructions were not vessel specific. Provided and post vessel specific emergency instructions.

Monthly inspections required by NFPA 10 may be conducted by the owner, operator, person-in-charge, or a designated member of the crew. Vessel was not conducting or logging monthly inspections of portable fire extinguishers. Conduct inspections and provide record.

Each vessel that operates on the high seas, or that operates beyond three miles from the coastline of the Great Lakes, must have on board a FCC Type Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. EPIRB registration had the incorrect MMSI number for the vessel. Correct registration.

The Rules concerning shapes shall be complied with by day. The lights and shapes specified in these Rules shall comply with the provisions of Annex I of these Rules (33 CFR part 84). Vessel was not carrying day shapes. Provide day shapes.

Vessel shall complete an annual inspection prior to operating.
