All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing machinery failure, and personnel injury. Vessel engine's exhaust pipe fractured at the weld near the turbo. Prior to clearing this deficiency, the vessel must; (1) complete repairs with new pipe fabrication and welding procedure.

A pre-engineered fixed gas fire extinguishing system must be capable of manual actuation from outside the space in addition to automatic actuation by a heat detector. Vessel's fixed fire extinguishing system discharged and depleted. Provide documentation of servicing for fixed firefighting system.

Vessel must comply with the applicable requirements for any radio installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. Vessel did not have valid Safety Radiotelephony Certificate on-board. Every vessel subject to Part III of Title III of the Communications Act must have a detailed inspection of the radio installation by an FCC-licensed technician in accordance with 47 CFR 80.59 once every five years. If the ship passes the inspection, the technician will issue a Communications Act Safety Radiotelephony Certificate. Provide proof of satisfactory Safety Radiotelephony Certificate to attending Marine Inspector.

Vessel's port shaft failed while underway. Shaft fractured 1 inch from main engine. Prior to clearing this deficiency the vessel must; (1) schedule out of water inspection with attending Marine Inspector for inspection of damage, (2) diagnose and report damage to shaft and any other components affected, (3) Submit written proposal to me for repair/replacement, and (4) conduct an operational underway test with attending Marine Inspector.

The Certificate of Inspection and any SOLAS Certificates must be posted under glass or other suitable transparent material, such that all pages are visible, in a conspicuous place on the vessel where observation by passengers is likely. If posting is impracticable, such as in an open boat, the certificates must be kept on board in a weathertight container readily available for use by the crew and display to passengers and others on request.

Employers must ensure that they and their crew-members meet the requirements of 46 CFR Part 16. Vessel could not provide proof of crew members enrollment in a drug and alcohol program required by 46 CFR Part 16. Provide proof of crew enrollment in required drug and alcohol program as required by 46 CFR Part 16 to attending Marine Inspector.

The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel to determine the presence of alcohol in the system of each individual who was directly involved in the SMI. Vessel's testing devices were expired. Until such time that valid testing devices are available onboard,vessel representative has stated that obtaining obtaining the devices and conducting the required alcohol tests can be accomplished within 2 hours from the time of occurrence of the SMI. Vessel will need to replace testing devices as required and or show written policy attesting to how the vessel will obtain the devices and conduct the required alcohol tests within 2 hours from the time of occurrence of the SMI.

The owner, charterer, master or managing operator shall instruct each crew member, at least once every three months, as to the duties that the crew member is expected to perform in an emergency including, but not limited to, the emergency instructions listed on the emergency instruction placard required by 46 CFR 185.510. Each EPIRB must be tested monthly using the integrated test circuit and output indicator, to determine that it is operative. Vessel was not able to show more than 03 sets of logged drills and EPIRB tests over last 1.5 years. Provide next logged drills and EPIRB tests to attending Marine Inspector & continue as internal policy as required by the regulations.

A vessel must carry either a first-aid kit approved under approval series 46CFR 160.041 or kit w/ equivalent contents and instructions. Items in kit were expired/ missing. Make kit in compliance with regulations.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: U.S. Coast Pilot or similar publication; Coast Guard Light List; Tide tables; and Current tables. Vessel did not have electronic versions or hard copies of these publications. If vessel intends to use electronic versions of these publications they must be downloaded for immediate use.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date charts of large enough scale to make safe navigation possible. Vessel's charts were not updated. Provide current/corrected charts for operational areas.

In a vessel of 12 meters or more in length, but less than 50 meters in length, a masthead light must be visible at the following minimum range: 5 miles. Vessel's masthead light was inoperable. Repair/replace to satisfaction of attending Marine Inspector.

Each disposable hydrostatic release unit (HRU) must be marked with an expiration date of two years after the date on which the unit is installed (or as per manufacturer recommendations). Markings were nonexistent or unreadable on EPIRB HRU. Replace with properly marked HRU's to manufacturer specifications, or provide proof of the date HRU's were installed. Make satisfactory to attending Marine Inspector.
