A space containing propulsion machinery must be equipped with a fire detection and alarm system of an approved type. The heat detection in the engine room had not been tested by third party and crew was not comfortable testing during inspection. Have certified or licensed state or local fire extinguishing agency test and certify heat detector prior to the carriage of passengers. 46 CFR 181.405(a)(1)

The number of penetrations in a watertight bulkhead must be minimized. A penetration in a watertight bulkhead must be as high and as far inboard in the bulkhead as practicable and made watertight. Crew has drilled several 1 inch holes to run wiring through several watertight bulkheads from crash compartment to engine compartment. Fill all holes to ensure watertight integrity of bulkheads prior to carriage of passengers. 46 CFR 179.320(c)

Every vessel subject to Part III of Title III of the Communications Act must have a detailed inspection of the radio installation by an FCC-licensed technician in accordance with § 80.59 once every five years. Vessels safety telephony certificate is expired. Vessel is restricted to 1000 ft. from shore until renewed. 47 CFR 80.903

Each vessel that operates on the high seas, or that operates beyond three miles from the coastline of the Great Lakes, must have on board a FCC Type Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. EPIRB hydrostatic release unit expires at the end of the month (13 days) Replace HRU prior to the carriage of passengers. 46 CFR 180.64

On a vessel on which the number of passengers permitted on upper decks is limited by stability criteria, as indicated by the vessel's stability letter, the maximum number of passengers allowed on an upper deck must be indicated by a durable marking of at least 25 millimeters (1 inch) numbers and letters at the entranceway to that deck. Upper deck max pax accommodation is marked for 36 pax instead of 33 pax per stability letter and COI. Make change and mark accordingly. 46 CFR 185.602(g)

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. A first-aid kit must be easily visible and readily available to the crew. Vessel had incomplete kit with expired items. Provide new first aid kit to the satisfaction of MI. 46 CFR 184.710

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces. High water level alarm in (crash compartment) did not work upon initial testing. Vessel captain corrected on the spot to the satisfaction of MI. 46 CFR 182.530(a)

Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines, one at the tank connection and one at the engine end of the fuel line to stop fuel flow when servicing accessories. The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located, preferably from an accessible position on the weather deck. If the handle to the shutoff valve at the tank is located inside the machinery space, it must be located so that the operator does not have to reach more than 300 millimeters (12 inches) into the machinery space and the valve handle must be shielded from flames by the same material the hull is constructed of, or some noncombustible material. Vessel does not have shielding around fuel shut off valve handles located in E.R. Provide shielding of adequate noncombustible material around valves as discussed with attending Marine Inspector. 46 CFR 182.455(b)(4)

The life line shall be properly secured around the sides & ends of the life float. The lifeline, cage & pendants must be as furnished by the mfgr w/ the approved life float or buoyant apparatus. Replacement lifelines & pendants must meet the reqmnts in §160.010 of this chapter. The netting for the cage is breaking down & needs to be repaired. The lifeline attachment to the life float is torn & must be repaired. OLD T: 46 CFR §180.15

The bilge pipe piercing the collision bulkhead shall be fitted w/ a screw-down valve located on the ... after side of the collision bulkhead if it is readily accessible under service conditions. The valve was frozen & must be repaired or replaced. OLD T: 46 CFR §182.25-5

he discharge ends of fuel vents shall be fitted w/ a removable flame screen of 30x30 corrosion resistant mesh. The flame screen for the stbd vent was improperly installed. Replace w/ properly installed flame screen.

Repair to full operation to the satisfaction of the attending marine inspector prior to the carriage of passengers. 46CFR181.405(a)(1)

All vessels must have navigation lights that are in compliance with the applicable sections of the international and inland navigation rules. The port running light did not illuminate. Vessel is limited to daytime operations until repairs are made to the satisfaction of the attending marine inspector.

Openings through which connectors enter shall be adequately closed. Outlet forward of the port engine and outlet located in the centerline between both engines were not adequately closed. 46CFR183.10-20(f) "Old T"

The insulated cape of the connector must be secured. Dead end wires were found above generators in the engine room. Remove or properly secure dead end wires in an enclosure.

Each disposable hydrostatic release unit(HRU) must be marked with an expiration date of two years after the date on which the unit was installed. EPIRB HRU was expired. Replace HRU prior to carrying passengers.

Each item of survival craft equipment must be of good quality, and efficient for the purpose it is intended to serve. Life float has a 2" puncture. Repair to the satisfaction of the attending inspector.

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. One of the combined ceiling light/vent protective cover and bulb is missing. Replace missing protective cover to reduce risk of electrical hazard. CITE: 46 CFR §183.200

Each life jacket kept in a stowage container must be readily available; other items are currently stowed in same locker as PFDs, cluttering ease of access. Remove clutter w/PFD storage & arrange PFDs for easier access during emergencies. 46 CFR §180.78(a)(2) - Corrected before completion of inspection.

Each paddle must be ... lashed to the life float to which it belongs .... One pair of paddles is unlashed and loose. Connect paddles to LF. 46 CFR §180.175(f)(2). Corrected prior to conclusion of inspection.

Each ship transporting more than 6 psgrs for hire equipped w/a radiotelephone station … must carry a radio operator who … hold[s] a marine radio operator permit or higher class license. The Master's radio operators' permit was not onboard at the time of the inspection. Provide copy of Master's FCC Marine Operator's Permit.

A separate circuit, w/overcurrent protection at the main distribution panel, must be provided for each radiotelephone installation. There are two back-up VHF radios (w/o DSC) powered by a common circuit. Provide separate circuits for the two auxiliary/back-up VHF radios. 46 CFR §183.392

Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. There is significant corrosion built up on the exposed electrical terminals behind the helm, which can lead to increased resistance, failure of the circuit, or fire. Clean corrosion from terminal blocks behind helm; replace wasted terminals as necessary. 46 CFR §176.800(a)

Controls and valves for operation of fixed gas fire extinguishing system must ... [n]ot located in a space that might be inaccessible in the event of fire in the space protected by the system. The break-glass control in the wheelhouse for releasing the CO2 is inaccessible due to a mounted storage box. Remove/relocate storage box in way of CO2 break-glass & pull on bridge. 46 CFR §181.410(b)(ii)

All cable and wire must: ... have stranded copper conductors with sufficient current capacity for the circuit in which they are used. Exterior upper deck lighting is wired with solid core 'Romex' wire. Within 30 days provide plan and propose date to replace all solid core wiring with UL Boat Cable or UL Marine Cable.

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull.... Marine Inspector discovered small penetrations in vessel's FRP layer on the keel where vessel rests on dry dock blocks. Some rot in the wood was discovered. Identify extent of problem area and repair to the satisfaction of the Marine Inspector prior to credit drydock.
