Vessel submitted a temporary manning deviation request to sail-short one 3rd Assistant Engineer, as required on the vessel's COI, for their voyage from Cleveland, OH to Duluth, MN. Vessel is authorized to operate short one 3rd Assistant Engineer for a one-time transit to Duluth, MN in accordance with the submitted checklist/plan. If work/rest limitations, and the 3 watch system, cannot be maintained due to vessel traffic, adverse weather, or other reasons, the vessel shall cease operations and anchor or moor, in accordance with the submitted plan, until the vessel is in compliance with applicable regulations. Station bill shall be updated and emergency and security duties shall be fulfilled throughout period of deviation. 46 CFR 15.515(a)

A Survey, either general or partial, according to teh circumstances, shall be made everytime an accident occurs or a defect is discovered which affects the safety of the vessel. A pin hole leak was discovered in the no. 08 port ballast tank while doing a routine inspection of the tanks. Upon arrival to Duluth effect repairs to the No. 08 port ballast tank to the Satisfaction of ABS or Attending Coast Guard Marine Inspector.

Due to the mooring arrangement at the time of the annual inspection the rescue boat could not be lowered, released, or get underway. The Master shall conduct a MOB drill and send a statement attesting to the proper operation of the rescue boat operation and satisfactory drill.

During the person overboard drill the rescue boat was unable to be retrieved after getting half way up and the brake started allowing the cable to pay out. Contact the servicing company and determine cause, repair, and demonstrate proper operation of the rescue boat's davit, winch, and braking system. Conduct another 1.1 overload weight test to ensure adequacy of the braking system under overload condition to the satisfaction of an attending CG Inspector.

The inflatable gate seals for cargo hold 7 have been replaced. Testing revealed poor workmanship and multiple failures due to different causes. The installation is to properly redone. Once re-work has been completed the seals are to be inflated, water applied to the cargo side of the gates and the seals are to be demonstrated to be free of defective workmanship and leaks. To the satisfaction of an attending CG Marine Inspector.

A survey either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or the efficacy of completeness of its life-saving appliances, fire-fighting or other equipment, or whenever any important repairs or renewals are made. The survey shall be such as to insure that the necessary repairs or renewals have been effectively made, that the materials and the workmanship of such repairs or renewals are in all respects satisfactory, and that the vessel complies in all respects with the regulations of this subchapter. The stern thruster electric motor experienced an electrical short causing a small electrical fire. The stern thruster shall remain secured until repairs are completed to the satisfaction of an attending marine inspector and class surveyor.

The Engine Order Telegraph (EOT) system does not fully operate as designed. The EOT was manufactured in the 1940s and parts and replacements in kind are no longer options. The EOT does send signals in both directions and it can be utilized in an emergency if needed. However, the synchro drifts and though the order for position sent from the bridge can be seen and acknowledged in the engine room the dial drifts slightly and the wrong direction alarm buzzer sounds constantly unless the operator in the ER keeps adjusting the dial. Submit a plan to MSD Sturgeon Bay to replace the EOT or fix it within 30 days. Other means to send engine orders on board were verified and the EOT can still be used but it may not remain in continued service and must ultimately be replaced.

The breaker box labeled EL 110 in the actuator room has openings in the breaker face plate that present a hazard to personnel. Blank off the openings in a suitable fashion to eliminate contact hazards to personnel.

Immersion suits must be labeled with the vessel name. Properly label the third mate's and the Captain's suits.

Due to the mooring arrangement at the time of annual inspection the rescue boat could not be lowered, released or get underway. The crew is to conduct a full MOB drill and take video of the lowering, releasing and running of the rescue boat within 30 days.

Governor failure on starboard main engine.

Hull damage in the port quarter resulting in approx 100 gal/ hr entering the after tank. Make temporary or permanent repairs to the satisfaction of an attending Classification Surveyor. Repair report is to be sent to the local OCMI upon completion. Permanent repairs to be completed by 31 MAR 2021.

Turbo charger failure on the port main

The #1 Port Ballast tank has significant bottom shell pitting in the aft inboard corner adjacent to the bell mouth. The pitting is in excess of 33% and covers an area approximately 18" in diameter. A cement box patch has been installed as a precaution. This area must be inspected annually by a CG Marine Inspector. Permanent repairs are to be made at the next scheduled dry dock which has a current due date of 30 April 2024.

A temporary hull repair was affected in the vessel's aft-peak. A 24" x 14" x 8.5" cement-box was placed IWO an 18" crack located in the aft-peak bottom-plate, just aft of the transverse watertight bulkhead at FR82, and 8' port of CL. This temporary repair is considered satisfactory for continued vessel operations, but must be permanently repaired by no later than the next sceduled drydocking to the satisfaction of USCG/ABS.

A few inch crack is present in the hull, in the aft peak tank, below the waterline, approximately at the 12 foot draft mark. Submit repair proposals for a permanent repair to [name withheld] approval of the proposal, conduct permanent repairs in Two Harbors, MN, prior to departure.

Number 2 (stbd) ship service generator reverse power relay (RPR) when tested, generators circuit breaker would not open. RPR removed and shipped for repairs. When repairs complete, Chief Engineer shall submit statement via e-mail to attending CG Marine Inspector, stating the RPR was reinstalled, tested, and working as designed. Until restored, when generators paralleled, an additional engineering watch stander shall be in engine control room to monitor the generators.

Vessel moored stbd side, unable to lower Rescue Boat. When able, crew shall lower, released, operated, and retrieved the Rescue Boat. To clear this requirement Master shall provide statement to the attending CG Marine Inspector reflecting Rescue Boat testing results.

Missing Rescue Boat operating instruction. Obtain and post operating instructions in the vicinity of the Rescue Boat.

Vessel unable to conduct MOB drill due to icing conditions alongside during spring break out. Required to conduct a MOB drill to the satisfaction of the CAPT upon entering open water and again in the presence of a CG Marine Inspector within 30 days. Extended 30 days at the request of the CAPT due to continued icing conditions.

Emergency fire pump was previously verified to run and supply adequate water pressure and flow as required. However, the pump casing was leaking excessively due to pitting on the casing gasket face. The pitted area was coated with lock tight but still leaked. Despite leaking the pump functioned as designed.

Cement box patch installed as temporary repair due to crack in bottom plate in #4 stbd ballast tank between frames 36-37 and 5th inboard bottom long. Must be removed and proper insert made at next drydock.

Noted many of the bottom longitudinals exhibiting narrowing/grooving in way of the weld to the bottom plate in ballast tanks #4 port and starboard. It is likely this same condition exists in remaining tanks. These should be included in the winter work survey and addressed prior to fit out.

Noted fractures in both fore and aft bulkheads in #4 starboard ballast tank. These fractures were located adjacent to where the catwalk support structure was welded to the bulkheads. Temporary repairs were made to the cracks but the cause remains. Modifications should be made to remove the attachment point of the catwalk to the bulkhead in all ballast tanks where this condition exists.

When working in both port and starboard #4 ballast tanks, discovered the catwalk in an extremely rotten and hazardous condition. This is a big safety issue for inspectors and surveyors. They should be replaced prior to winter surveys commencing.

Two cement boxes installed as temporary repairs to cracks in the bottom plating in #4 port ballast tank at the #3 and #4 bottom longitudinals outboard of the tunnel about midway fore and aft in the tank. These must be removed and a proper insert made at the next dry dock.

Make permenant repairs to the bulkhead between cargo holds 5 & 6 and to the combing on hatch cover 18 in cargo hold #5 to the satisfaction of an attending CG Marine Inspector.

Prior to departure from Indiana Harbor make permanent repairs to STBD lube oil pump to the satisfaction of attending Coast Guard Marine Inspector.

Off Load cargo in Indian Harbor, upon return to Escanaba, MI make repairs to the faulty Port Ship Service Diesel Engine Control Module per 46 CFR 111.12-1. Repairs and testing shall be to the satisfaction of the [name withheld] Michigan, prior to the vessels departure from Escanaba, MI.

Observed hydrostatic releases were expired on 3 life rafts. Renew expired hydro static releases as required per 46 CFR 199.190(h) due by 18April2016

In the aft upper three crew accommodation decks, Observed communication (Internet and television) cables penetrating bulkheads without proper cable stuffing tubes. Remove, fit with bulkhead penetrations and fit with stuffing tubes as required per 46 CFR 111.60 due by 1 March 2017

Observed alcohol breath analyzer calibrated three years ago. Re-calibrate the alcohol breath analyzer every year per 46 CFR 4.06-3(a) 18April2016

Observed pressure gauges missing at both the main and emergency fire pumps. Install pressure gauges on the discharge side the main and emergency fire pumps per 46 CFR 95.10.5(e) 18April2016
