Vessel is authorized to operate short one 3rd Assistant Engineer for a one-time transit to Duluth, MN in accordance with the submitted checklist/plan. If work/rest limitations, and the 3 watch system, cannot be maintained due to vessel traffic, adverse weather, or other reasons, the vessel shall cease operations and anchor or moor, in accordance with the submitted plan, until the vessel is in compliance with applicable regulations. Station bill shall be updated and emergency and security duties shall be fulfilled throughout period of deviation. 46 CFR 15.515(a)

Upon arrival to Duluth effect repairs to the No. 08 port ballast tank to the Satisfaction of ABS or Attending Coast Guard Marine Inspector.

During the person overboard drill the rescue boat was unable to be retrieved after getting half way up and the brake started allowing the cable to pay out. Contact the servicing company and determine cause, repair, and demonstrate proper operation of the rescue boat's davit, winch, and braking system. Conduct another 1.1 overload weight test to ensure adequacy of the braking system under overload condition to the satisfaction of an attending CG Inspector.

Resolution: The brake assembly required adjustment. Another 1.1 overload and brake test was also conducted to ensure safety of the system. Installation now functions as designed.

The inflatable gate seals for cargo hold 7 have been replaced. Testing revealed poor workmanship and multiple failures due to different causes. The installation is to properly redone. Once re-work has been completed the seals are to be inflated, water applied to the cargo side of the gates and the seals are to be demonstrated to be free of defective workmanship and leaks. To the satisfaction of an attending CG Marine Inspector.

Resolution: Adjustement to the gates was needed as well as some welding to existing gate structures. Verified proper inflation and gates passed a water test.

A survey either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or the efficacy of completeness of its life-saving appliances, fire-fighting or other equipment, or whenever any important repairs or renewals are made. The survey shall be such as to insure that the necessary repairs or renewals have been effectively made, that the materials and the workmanship of such repairs or renewals are in all respects satisfactory, and that the vessel complies in all respects with the regulations of this subchapter. The stern thruster electric motor experienced an electrical short causing a small electrical fire. The stern thruster shall remain secured until repairs are completed to the satisfaction of an attending marine inspector and class surveyor.

The Engine Order Telegraph (EOT) system does not fully operate as designed. The EOT was manufactured in the 1940s and parts and replacements in kind are no longer options. The EOT does send signals in both directions and it can be utilized in an emergency if needed. However, the synchro drifts and though the order for position sent from the bridge can be seen and acknowledged in the engine room the dial drifts slightly and the wrong direction alarm buzzer sounds constantly unless the operator in the ER keeps adjusting the dial. Submit a plan to MSD Sturgeon Bay to replace the EOT or fix it within 30 days. Other means to send engine orders on board were verified and the EOT can still be used but it may not remain in continued service and must ultimately be replaced.

The #1 Port Ballast tank has significant bottom shell pitting in the aft inboard corner adjacent to the bell mouth. The pitting is in excess of 33% and covers an area approximately 18" in diameter. A cement box patch has been installed as a precaution. This area must be inspected annually by a CG Marine Inspector. Permanent repairs are to be made at the next scheduled dry dock which has a current due date of 30 April 2024.

A temporary hull repair was affected in the vessel's aft-peak. A 24" x 14" x 8.5" cement-box was placed IWO an 18" crack located in the aft-peak bottom-plate, just aft of the transverse watertight bulkhead at FR82, and 8' port of CL. This temporary repair is considered satisfactory for continued vessel operations, but must be permanently repaired by no later than the next sceduled drydocking to the satisfaction of USCG/ABS.

Number 2 (stbd) ship service generator reverse power relay (RPR) when tested, generators circuit breaker would not open. RPR removed and shipped for repairs. When repairs complete, Chief Engineer shall submit statement via e-mail to attending CG Marine Inspector, stating the RPR was reinstalled, tested, and working as designed. Until restored, when generators paralleled, an additional engineering watch stander shall be in engine control room to monitor the generators.

Noted fractures in both fore and aft bulkheads in #4 starboard ballast tank. These fractures were located adjacent to where the catwalk support structure was welded to the bulkheads. Temporary repairs were made to the cracks but the cause remains. Modifications should be made to remove the attachment point of the catwalk to the bulkhead in all ballast tanks where this condition exists.
