Vessel was missing FCC Radio Station Authorization. Vessel's route is <100 nm from PR on Oceans Route. Vessel's radios need to be certified by FCC Technician prior to operating beyond 1000ft from shore. 46 CFR 184.502/ 47 CFR 80.13

A pre-engineered fixed gas fire extinguishing system must: Automatically shut down all power ventilation systems and all engines that draw intake air from within the protected space. Fire boy system did not work as designed due to a corroded terminal. Vessel is restricted from carrying passengers until the system is operational and tested to the satisfaction of attending USCG marine inspector.

A vessel on an oceans route must carry — Six hand red flare distress signals and Six hand orange smoke distress signals or other standard specified by the Commandant. Vessel had enough flares on board, but did not have the parachute flare pistol as required. Vessel owner bought flare gun and sent photos prior to USCG returning to office.

Each ring life buoy on a vessel must: Be readily accessible and Be stowed in a way that it can be rapidly cast loose. Arrangement of ring life buoy is not satisfactory. Access path is blocked by numerous gear and ladder. Access path must be cleared or ring life buoy must be relocated. 46CFR180.70

Your vessel must undergo an annual inspection within the 3 months before or after each anniversary date. Vessel missed annual inspection window, vessel cannot operate until annual inspection is completed.

A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a drydock examination and an internal structural at least once every two years. Vessel cannot operate until a DD/ISE inspection is completed.

Each item of survival craft equipment must be of good quality, and efficient for the purpose it is intended to serve. Last 5' of painter line, part of which is exposed to sun, is wasted and must be replaced.

Each vessel that operates on the high seas must have on board a FCC Type Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. Hydrostatic realease unit expired in October. New unit must be installed.

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Exposed dead-end wires were located in the main propulsion machinery space; bilge float that was disconnected, forward engine bay on port side, and aft engine bay port side. Wires must be properly removed to avoid electical hazards.

No person may operate a vessel that does not have a valid certificate of number for that vessel issued by the issuing authority in the State in which the vessel is principally operated. State registration expired in May. Vessel must be registered.

A vessel must be operated in accordance with applicable laws and regulations and in such a manner as to afford adequate precaution against hazards that might endanger the vessel and the persons being transported. Vessel's anchor light is inoperable. Anchor lisht must be replaced.

An inspection for renewal of a Certificate of Inspection is conducted to determine if the vessel is in satisfactory condition, fit for the service intended, and complies with all applicable regulations. It normally includes inspection and testing of the structure, machinery, and equipment. The owner or operator must conduct all tests as required by the OCMI, and make the vessel available for all specific inspections and drills required by subpart H of this part. In addition, the OCMI may require the vessel to get underway. Vessel did not have full compliment of crew in attendence. Vessel did not get underway and conduct drills. Inspection must be completed when all crew, as required by COI, are present.

A vessel on which a pre-engineered fixed gas fire extinguishing system is installed must have a means to reset devices used to automatically shut down ventilation systems and engines. When conducting an operational test of the pre-engineered fixed gas fire extinguishing system, the shutdown mechanism could not be overridden and left the vessel without means of propulsion. Must operate prior to carrying passengers. 46 CFR 181.420(b)(3)

An adult life jacket must be provided for each person carried on board a vessel. Vessel is authorized to carry 17 people and only has 15 life jackets on board. Vessel may only operate with 13 passengers and 2 crew members until 2 more life jackets are provided. 46 CFR 180.71(a)

A vessel on an oceans route must carry six hand red flare distress signals and 6 other approved distress signal devices for a total of 12 distress signals. Vessel carried 3 packs of 4 hand red flare distress signals, 2 packs of which expire October 31. Vessel must replace the 2 packs of distress signals that expire in October by October 31, 2022. 46 CFR 180.68(a)(1)

The lifeline and pendants must be as furnished by the manufacturer with the approved life float. Vessel’s life float pendants are wasted and need to be replaced. Replacement lifelines and pendants must meet the requirements in Subpart 160.010 of this chapter. 46 CFR 180.175(f)(1)

Each paddle must be lashed to the life float to which it belongs and buoyant. Vessel must lash each paddle to the life float. 46 CFR 180.175(f)(2)

A visual and audible alarm must be provided at the operating station to indicate a high water level in a machinery space bilge, bilge well, shaft alley bilge, or other spaces subject to flooding from sea water piping within the space. During operational test of bilge system, high water level alarms were in-operable. During operational test, bilge pump discharged water in automatic mode. Bilge system's high water level alarm must be operable.

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards. MI witnessed exposed electrical wires in v-berth head. Wires must be removed or terminate the ends properly.

Deck rails must be designed and constructed to withstand a pointload of 200 pounds applied at any point in any direction, and a uniform load of 50 pounds per foot applied to the top rail in any direction. Port and starboard deck rails and aft canopy supports were loose at the mounting locations and therefore not secured to the bulwark as designed. All mounts for port and starboard deck rails and aft canopy supports must be secured using sufficient screws and washers/backing plates.

A vessel must be fitted with mooring lines necessary for the vessel to be safely moored. The mooring lines provided must be satisfactory for the size of the vessel and the waters on which the vessel operates. Vessel's port mooring line is worn beyond service and must be replaced.

The master shall conduct sufficient drills and give sufficient instructions to make sure all crew members are familiar with their duties during emergencies that necessitate abandoning ship or the recovery of persons who have fallen overboard. No training provided for deckhand [name withheld] for this part.

The master shall conduct sufficient fire drills to make sure that each crew member is familiar with his or her duties in case of a fire. No training provided for deckhand [name withheld] for this part.

The owner, charterer, master or managing operator shall instruct each crewmember, upon first being employed and prior to getting underway for the first time on a particular vessel and at least once every three months. Deckhand [name withheld] has not conducted crew training per this part.

Operation of propulsion engine overide feature of installed fixed extinguishing system (FireBoy)could not be demonstrated.

Double clamp all vital flexible hoses (bilge hoses).

MOB and Fire Drills not conducted (nor logged).

No record of monthly EPIRB tests.

First Aid Kit items expired (Aspirin- 2018 & Alcohol towlettes).

Insulation in way of engine turbocharger wasted.

No sound signal (installed air horn non-operational).

Installed exterior electrical lighting, forward of main cabin and in way of top deck operating station inoperative.

Demonstrate emergency drills to the satisfaction of the attending Coast Guard Marine Inspector.

COI not posted on vessel.

Stability Letter not posted on vessel.

Signs for fuel shut off valves wasted (and lettering too small).

Remove remaining dead end wiring / abandoned electrical wiring under top operating station. Repair missing cover for shore tie outlet.

Vessel/Company not in compliance with required Coast Guard Drug and Alcohol Program requirements.

EPIRB battery and registration was expired.

Life Float net was not origional to device, as per Subchapter Q.

Anchor light as wasted. Light lens crumbled when attempting to open to gain access to change bulb.

"Weak Link" did not have documentation for type of life float device, nor did vessel owner able to provide specs on installed weak link.

Repair or replace water ligths for lifejackets. Upon inspection, noted (3) not working properly.

Repair waterlight for lifebuoy. Upon inspection, noted light working correctly. The light had to be tapped on the sides to make to work.

Provide FCC Safety Certificate. Current certificate onboard expired on 10 Apr 08.

Provide EPIRB in float free arrangement: Current hydrostatic release unit expired 30Mar07.

Provide fuel shut-off valve at the engine for main fuel service line. 46 CFR 182.455(b)(4).

Provide onboard record of required gear inspections including monthly EPIRB test/inspections, monthly inspections of portable fire extinguishers, weekly inspections of survival craft.

Provide onboard vessel-specific emergency instructions (may use template provided by marine inspector). 46 CFR 185.510.

Replace flame screens in port and stbd fuel oil tank vents.

Seal collision bulkhead holes to make watertight. Seal holes at both ends.

Provide updated bridge log with required monthly drills to include fire, man overboard and steering test.

Paint in black matte side shields for the naviagation lights.

Replace port, starboard and aft safet rails with stronger lines or chain to be able to withstand 200lbs.

Provide Coast Guard approval number for lifefloat or replace lifefloat with an approved lifefloat.

Provide an appoved weaklink to lifefloat.

Replace reflective tape to lifefloat.

Patch holes in lifefloat.

Label remote emergency fuel shutoff valves with purpose and direction of operation.

Trim engine room hatch frame so that frame does not contact main engine exhaust pipe.

Re-laminate exhaust pipe where main deck hatch frame made a cut.

Replace expire light to three (03) adult lifejackets. Date on light showed expired, but when tested, tested sat.

Seal and provide fasteners to secure hatch to collision bulkhead.

Provide seal to main deck hatches to make them weathertight.

Provide evidence of enrollment in a drug testing consortium.

Adjust rudder to eliminate excesive play at seal or replace seal. To be corrected prior in placing the vsl back in the water.
