Remote fuel shutoff stations must be marked in clearly legible letters at least 1" high indicating the purpose of the valve and direction of operation. Shut off not labeled.

Each life ring on a vessel must be approved in accordance with 160.050 in Subchapter Q of this chapter. Ring buoy’s grab line supports were deteriorated.

The employer shall provide EAP for all crew members. There is no evidence of a EAP program in place, no evidence of supervisory personnel receiving 60 min of training.

As appropriate for the intended voyage, a vessel must carry adequate and up to date: U.S. Coast Pilot or similar publication, Coast Guard light list. Vessel missing Coast Pilot.

The owner shall, prior to receiving a COI, submit for approval to the cognizant OCMI, at least two copies of the following plans, manuals, analyses, and calculations that are applicable to the vessel as determined by the OCMI: (12) On sailing vessels: (i) Masts, including integration into the ship’s structure; and (ii) Rigging plan showing all areas and centers of effort as well as the arrangement, dimension, and connections of the standing rigging. Owner could not produce these approved plans. Owner may submit them to MSC directly or submit to MSC through OCMI and upon approval, provide them to the attending marine inspector. Due to vessel being under less than 65’ owner may submit specifications, sketches, photographs, line drawings or written descriptions instead of any required drawings, provided the required information is adequately detailed and acceptable to the cognizant OCMI, 46 CFR 177.202(c)

The OCMI may require an inspection and testing whenever a repair or alteration is undertaken. Jib sail is not replaced in kind. Submit new sail plan to MSC with smaller Jib sail dimensions. Vessel is prohibited from using Aux sails.

At each initial and subsequent inspection for certification of a vessel the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances, including the following: Inspections of the conditions of the superstructure, masts, and similar arrangements constructed on the hull, and on a sailing vessel all spars, standing rigging, running rigging, blocks, fittings, and sails. Unable to complete inspection as vessel is missing its required Jib sail. Current Jib installed is not of the size of submitted sail plan and is not replaced inkind. Vessel is prohibited from aux sail power.

Except as provided in paragraph C of this section, a vessel must be provided with an auxiliary means of steering that is: (b) A suitable hand tiller may be acceptable as the auxiliary means of steering where satisfactory to the cognizant OCMI. Vessel has a single rudder and is not provided with an auxiliary means of steering.

Provide preventative maintenance for rigging maintenance and inspection.

Properly terminate wire connecting old horn in the junction box to assure safety.

Find reason for banging and limited propulsion/steering. Provide written report detailing any damage.

Engine did not start after switching to 'override' on fixed firefighting system.

Thimble on lower bobstay was cracked on top and bottom and wire rope was beginning to spread out. Renew both.

All 3 red flares were expired. Only daylight operations permitted until flares are replaced.

02 frames on port side inside cabin are rotted. Repair/replace to the satisfaction of the attending Marine Inspector.

Port side FWD in cabin overhead deck is showing signs of water intrusion. Plywood has rot, repair/replace to the satisfaction of attending Marine Inspector.
