The following operational hazards to safety systems were identified and must be corrected: 1. Unable to take bilge suction from the center void. 2. The egress route in the machinery space needs to be properly stenciled EMERGENCY EXIT, KEEP CLEAR from both directions. 3. Reattach the life float paddles to the body of the life float. 4. Stencil both remote fuel shut-off valves.

The following operational hazards to machinery and electrical systems were identified and must be corrected: 1. The STBD generator fuel filter is not equipped with a metal bowl or flame shield. 2. The electrical receptacle on the STBD/Forward side of the machinery space is compromised and needs to be replaced. 3. The battery box covers need to be properly secured and mounted. 4. The wires for the high water bilge alarm in the lazarette need to be properly secured. 5. Remove all excess materials from the machinery space. 6. The STBD generator overload protection device (breaker) is oversized (125A) and should not exceed 89.2 amps. Replaced with an appropriate sized breaker. 7. There are numerous unsafe practices, fire hazards, and other hazardous situations that must be corrected in the machinery space.

The vessel must have a VHF-DSC radio that is properly installed. The radio must have the MMSI programmed.

The lifejacket stowage location must be marked with size and quantity. The stowage doesn't have the quantity marked.

Coast Guard inspector received the manufacturer's specifications for the newly acquired generator and identified a 660 pound deletion of weight. Simplified Stability tests will be required anytime the addition or deletion of weight exceeds 500 pounds. In addition, anytime the center of gravity of the engine/generator that is being installed significantly differs from the engine being removed, a stability test will be required. Owners and Operators of Small Passenger Vessels must abide by these regulations if they plan to re-power their vessel. The vessel is given the deadline of 01JAN2024 to conduct a satisfactory Simplified Stability test.

Inspect bulkheads in accordance with 46 CFR 176.802. Fwd engine room blkhd is not watertight.

Inspect for hazardous conditions in accordance with 46 CFR 176.830. Port generator is leaking oil excessively.

Inspect vessel for sanitation in accordance with 46 CFR 176.818. Concession area sanitation was unsatisfactory.

Inspect vessel for hazardous conditions in accordance with 46 CFR 176.830. Overhead over stbd generator is coated in oil residue.

Inspect fire equipment in accordance with 46 CFR 176.810. Renew firehose.

Inspect weather deck structure in accordance with 46 CFR 176.802. Renew port window.

Inspect fire main in accordance with 46 CFR 176.810. Renew corroded fire main piping in engine room at next drydock.

Replace port fire hose supply valve, valve is wasted.

Replace or service stbd fire extinguisher main deck. Extinguisher was missing servicing tag.

Forward bench seat loose.

Center MDE F/O supply valve handle missing. Provide handle to allow valve to be closed.

Center MDE J/W temp/alarm inop. Repair or replace and prove proper operation of temp/alarm for J/W.

Bilge piping shall be ferrous material for vital systems. Bilge piping elbows in the engine room and steering compartment were found wasted.

Vessel required to provide visual and audible alarm to indicate a high water level. H/W alarm for steering compartment inop.

Engine room bilge found with excessive amount of oil. Clean and dispose of excessive accumulation of oil from Engine room bilge.

Port SSDG Salt water strainer loose.

Port SSDG electrical control module loose and unsupported. Attach control module to bracket for support.

Starboard generator salt water valve inoperable.

Electrical equipment must be maintained to prevent fire. Generator windings are found with excess amounts of oil.

Steering linkage found with loose clamp and interference with steering quadrant.

Missing Radio Safety Certificate. Provide proof of updated document.

Reduce quantity of power-strips on the Bridge and Galley.

Collision bulkhead valve in bilge inoperable. Reach Rod unsatisfactory provide proof of repair.

Ensure proper hardwiring of port-side Engine Room Battery Charger.

ESP Instillation unsatisfactory. Ensure proper wiring and installation.

Repair vents of the port and starboard fuel tanks.

Provide a latch for the galley refrigerator.

Replace light fixtures on top deck of the vessel.

Fresh water service pump in Engine Room, not secured. Provide proof of correction.

Replace life floats due to unserviceable conditions. Provide proof of newly acquired equipment.

At Dry dock, observed three worklist items including repairing of the wood/hull area of the aft transducers sealer, anchor windlass generator belt guard/shiled being unsecured, and the fwd void bilge piping strainger being rusted away needign repair/replacement. Complete worklist according to regs prior to leaving dry dock.

All machinery and equipment must be installed and maintained in such a manner to avoid causing fire, explosion, machinery failure, and personnell injury. Witnessed excessive oil, oil residue, and deisel in the bilge of E/R and on the two house generators. Repair/renew to clean and satisfactory safe condition.

Provide proof of EPIRB and NOAA registration.

Repair or remove dead end wires on broken deck lights throughout.

Double hose clamp the raw water suction hoses for both main diesel engines.

Clean and condition all Life Floats, Paint faded Life Float, also make repairs where needed.

New floating light for the life float.

Provide spacers for Life Float.

Upper deck Life Ring Buoy recondition and paint, or prvide a new Buoy.

Prvide mounting bracket for Engine Room FFE.

Operational test of Center engine.

29March2017 Issued - The vessel re-powered its engines for a weight change of 423 lbs. The vessel is required to conduct a SST. Temporary ballast to compensate for the weight change will be allowed as only a temporary measure and will only be accepted till Nov 2017. **27Nov17 Extension request has been approved until 03Jan2018 at which time either an amendment to the vessel's current stability letter is approved by MSC or a new stability test conducted.**

Steering gear compartment needs gauging report.

Provide lights for life jacket

Louver in electrical box in engine room.

Vessel conducted repower that resulted in weight change and will require new SST.

Repair seal on fire pump

Repair/Replace 2 identified ring lifebuoys (upper deck)

Repair / Replace 2 identified lifefloats 1 painter and 1 light line.

Install VHF radio with DSC function.
