PSCO observe 2 unsatisfactory fire drills. During both drills when asked to perform a check of their equipment members were lacking a radio, and also were not on the correct channel. During Both drills members were observed removing their gloves to use their radios, flashlights, and to check the pressure of their SCBA. The lifeline attached to the fire team was made of combustible material in both drills the proper boundaries were not set.

The master, owner, operator, agent, or person in charge or a vessel equipped with ballast tanks that operates in the waters of the U.S. must employ one of the following BWM methods.
The Master, Owner, Operator, Agent, or person in charge of the vessel subject to this subpart must properly maintain the BWMS in accordance with all manufacture specifications.
The vessel's BWTS manual indicates that ballast is to be treated both on uptake and discharge the systems log is missing discharge events indicated in the vessels hand written log. Additionally, the system log recorded multiple errors and alarms regarding the UV light units, high difference pressure, and missing feeback on valves, indicating the system may not be working properly.
