The arrangements for oil fuel shall ensure the safety of persons on board. Excessive fuel oil is leaking from the main engine, oil purifiers, and sludge pumps. Dried streams of oily residue, dozens of oily rags, buckets, and guards designed to drain oil into plastic containers are evidence of an existing, on-going, and persistent problem. 74 SOLAS (97), Ch.II-2/15.2.

Resolution: Class issued a statement attesting to tightening valve packing and replacing seals. The oily buckets were removed while conducting the deficiency check for which the captain provided a receipt.
The following basic principles underlie the regulations in this chapter: containment and extinction of any fire in the space of origin. The incinerator ash reservoir door was found propped closed with a wooden plank less than 10 feet from a pile of garbage, oily buckets, and a fresh air vent. The door could not be closed as designed. 74 SOLAS (97), Ch. II-2 / Reg. 2.2.5.

Resolution: The crew modified the hinge mechanism to allow the door to close, and the [name withheld] attested to the satisfactory rectification.
The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship will remain fit to proceed to sea without danger to persons on board. Observed: (1) wet oil on turbo air filter, (2) exposed exhaust manifold, and (3) oil soaked lagging throughout the engine room. 74 SOLAS (14), Ch.I/11(a).
