At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of machinery, fuel, and piping systems, including the following: Operational test of the means provided for pumping bilges. Complete but simple instructions for the operation of a fixed gas fire extinguishing system must be located in a conspicuous place at or near each pull box and stop valve control and in the space where the extinguishing agent cylinders are stored. If the storage cylinders are separate from the protected space, the instructions must also include a schematic diagram of the system and instructions detailing alternate methods of releasing the extinguishing agent should the local manual release or stop valve controls fail to operate. Each control valve to a distribution line must be marked to indicate the space served. The current sign does not include all necessary instructions and methods to release the agent. 46 CFR 185.612(b)

The alcohol testing devices need not be carried on board each vessel if obtaining the devices and conducting the required alcohol tests can be accomplished within 2 hours from the time of occurrence of the SMI. No alcohol swabs on-board vessel. 46 CFR 4.06-15

The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing the actions to be taken in the event of fire, heavy weather, or man overboard conditions. Vessel did not have emergency instructions onboard the vessel. 46 CFR 185.510

182.500, A vessel must be provided with a satisfactory arrangement for draining any watertight compartment, other than small buoyancy compartments, under all practicable conditions. Sluice valves are not permitted in watertight bulkheads. In the fuel tank space on the stbd side the bilge overboard valve is leaking.

Vessel shall not carry passengers for hire until an underway test is completed with a qualified USCG Marine Inspector. The purpose of this test is to ensure the vessel has properly swelled up and is not making an excessive amount of water. Only after this test is satisfactorily performed will credit Dry Dock and Internal Structural Exam be given.

A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a drydock examination and an internal structural at least once every two years. Vessel's Dry Dock Date was due 31MAR2025 and has not completed a Dry Dock / Internal Structural Inspection. - 46 CFR 176.600(c)(1)

§ 177.960 Guards for exposed hazards. An exposed hazard, such as gears or rotating machinery, must be properly protected by a cover, guard, or rail. 03 Propulsion Engines are missing Guards on Rotating Machinery.

181.320(b), On a vessel of not more than 19.8 meters (65 feet) in length carrying more than 49 passengers, and on a vessel of more than 19.8 meters (65 feet) in length, each hose must: (1) Be lined commercial fire hose that conforms to UL 19 (incorporated by reference, see 46 CFR 175.600) or hose that is listed and labeled by an independent laboratory recognized by the Commandant as being equivalent in performance; (2) Be 15.25 meters (50 feet) in length and 40 millimeters (1.5 inches) in diameter; and (3) Have fittings of brass or other suitable corrosion-resistant material that comply with NFPA 1963 (incorporated by reference, see 46 CFR 175.600) or other standard specified by the Commandant. Each Bilge pump must be self priming to the fixed system. Could not demonstrate bilge suction from the main engine bilge suction pump. Demonstrate proper operation of the fixed bilge system. 46 CFR 182.520(c)

Bilge pumps automatic submersible pump may have flex pipe. Flex pipes in all voids were not in serviceable condition replace all flex hoses. 46 CFR 182.520(f)(i)

Engine cooling: all engines must be water cooled and meet the requirements here in all three engine's raw water piping was blistering. Replace all raw water hoses. Reference 46 CFR 182.720 for type spec. 46 CFR 172.420

Engine exhaust shall be provided with noncombustible hangers and blocks for support. Install exhaust run supports on all engines and all void spaces. 46 CFR 182.430

Each survival craft must be secured to the vessel by a painter line... Identified all float free arrangements painter lines not affixed to life float and in deteriorated condition. Replace and afix to Life floats. 46 CFR 180.130(a)(1)

each vessel that operates beyond three miles from the coastline must have on board a FCC Type Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. Identified EPIRB has not been registered under current owner [name withheld] and submit proof of registration. 46 CFR 180.64

CO2 storage cylinder must be securly fasten and supported and protected against damage. CO2 Bottle next to kitchenette needs to be secured to bulkhead. 46 CFR 181.410(c)(4)

An exposed hazard such as rotating machinery must be properly protected by a cover or Guard. PTO belts on ALL engines need guards or covers. 46 CFR 177.960

A positive action valve that is located close as possible to hull must be able to close. Sink discharge in Void 2 was seized could not operate. In Lazarette the starboard forward frame bay through hull needs to be plugged closed. 46 CFR 179.350(d)

all electrical circuits must be clear and marked. The Electrical distribution panel in engine room was not marked with location each circuit services. 46 CFR 183.220(d)

Electrical equipment that provides services for safety must be in good order. Light over port engine was hanging re-secure to overhead. The terminal blocks over all of the engines were found corroded, Fix or replace. 46 CFR 183.200(a)

Electrical wires shall be installed with metal supports no further than 24inches apart. Vessel's electrical cable runs were found with out fixed metal supports. All systems must be in good order to include piping systems. The hydraulic piping was found without supports. 46 CFR 183.340

Inoperable stern light

For a life float or buoyant apparatus approved, the condition must be in a good and serviceable condition. Identified the center buoyant apparatus strap for the life lines wasted and pulled the strap off. replace the life line strap with a similar design and construction equivalent with the standards as constructed.

Identified expired first-aid kit contents. Remove and replace all items expired in the vessel’s first aid kits. First aid kits on board exceed what is required in 46 C FR 160.041

Fire hoses on board must be of an approved type and in good and serviceable condition. Identified the starboard fire hose as wasted where water leaks from the hose mid way through. replace the fire hose with a similar design and construction as required by the COI.

The design of all exhaust must ensure that minimal risk of injury to personnel exist. Identified exhaust lagging missing for the center main diesel engine and is a potential burn hazard. Re-install exhaust, hot pipe, protection.

Identified a leak on the port side main Diesel engine shaft at the shaft packing/ cooling. address eat and ensure shaft packing is operating with minimal leakage and as designed.

Identified leak of the PORT main diesel engine raw water pump. address the leak and ensure the raw water pump operates as designed.

Identified an exhaust leak on the PORT main Diesel engine. Address leak where the exhaust discharges at the breather as designed.

Renew battery in water light on aft life ring.
