Electrical equipment on a vessel must be installed and maintained to provide services necessary for safety under normal and emergency conditions. The sensor alarm panel on the bridge was not displaying the second engine monitoring screen. 46 CFR 120.200 (a)

Materials used in piping systems must meet the requirements of this subpart and be otherwise acceptable to the cognizant OCMI. Marine Inspector identified the piping for the fire pump discharge on the port side had corrosion build up and requires cleaning for further assessment. (46 CFR 119.700)

A device must be provided to automatically shut down power ventilation serving the protected space and engines that draw intake air from the protected space prior to release of the extinguishing agent into the space. Marine Inspector identified the engine room starboard aft fire damper was unable to fully close. (46 CFR 118.410(b)(10))

Water for cooling an exhaust pipe, other than a vertical exhaust, must be injected into the exhaust system as near to the engine manifold as practicable. The water must pass through the entire length of the exhaust pipe. Marine Inspector identified flexilbe hose connected to port main engine muffler was leaking small amount of water at high RPMs where the hose to muffler fitting was connected. (46 CFR 119.425(b)(3))

Each ring life buoy on a vessel must be readily accessible. Marine Inspector observed that each life ring was missing retroreflective tapes. (46 CFR 117.70 (b)(2))

The fire detection and alarm system must be so arranged and installed that the presence of a fire in any of the protected spaces will be automatically registered visibly and audibly in the pilothouse. MI identified an error on the fire detection panel during op testing; both port and starboard display as starboard. (46 CFR 115.810(a)(7))

The construction and arrangement of a vessel must allow the safe operation of the vessel ... giving consideration to provisions for a seaworthy hull ... and necessary facilities for passengers and crew. Marine Inspector observed and marked 07 cracked welds in void spaces and discussed their locations with the operator. (46 CFR 116.100(a))

Vessels… carry more than 150 passengers shall meet the requirements of subpart 72.05 of Subchapter H. Doors in “A” class bulkheads shall… have a latch with a minimum throw of ¾ inch. Installed latch mechanism on the forward lounge doors identified malfunctioning on the bottom and without proper upper latch catch. 46 CFR 177.10-5(a) 46 CFR 72.05-25(b)(3) (OLD T)

The fire detection and alarm system must be so arranged and installed that the presence of a fire in any of the protected spaces will be automatically registered visibly and audibly in the pilothouse. MI identified an error on the fire detection panel during op testing. 46 CFR 115.810(a)(7)

Identified holes in multiple onboard doors in “A” class bulkheads. 46 CFR 177.10-5(a) 46 CFR 72.05-25(b)(1) (OLD T)

Vessels… carry more than 150 passengers shall meet the requirements of subpart 72.05 of Subchapter H. Doors in “A” class bulkheads shall… meet the following requirements: Rugs and carpets shall not pass through doorways, but linoleum and similar coverings may do so. Forward main deck stairway doors found with carpet passing beneath. 46 CFR 177.10-5(a) 46 CFR 72.05-25(b)(6) (OLD T)

At each ... inspection for certification, the marine inspector shall view the vessel afloat and shall ...Examine all accessible parts of the exterior and interior of the hull, the watertight bulkheads, and weather deck. Marine Inspector identified that the sewage overboard piping was deteriorated. Conduct repairs to the satisfaction of the attending MI at the next dry-dock. 46 CFR 176.25-5(a)(1) OLD T

Spaces protected by carbon dioxide—“CARBON DIOXIDE GAS CAN CAUSE INJURY OR DEATH. WHEN ALARM OPERATES OR WINTERGREEN SCENT IS DETECTED, DO NOT ENTER UNTIL VENTILATED. LOCK OUT SYSTEM WHEN SERVICING.” The reference to wintergreen scent may be omitted for carbon dioxide systems not required to have odorizing units and not equipped with such units. Marine Inspector identified that the CO2 Placards required for protected spaces was missing on the exteriors of the port and starboard CO2 storage spaces, and Engine Room. Add safety placards to the exterior entry points of each CO2 storage and protected space to the satisfaction of the attending Marine Inspector. 46 CFR 122.612(i)(2)

All electrical equipment and circuits must be clearly marked and identified. Marine Inspector identified that various distribution panels on board did not properly identify "spare" breakers which appeared to be in use. 46 CFR 120.220(d)

Each... ring life buoy must be marked in clearly legible block capital letters with the vessel's name. Marine Inspector identified that the ring life buoys located on the sun deck did not clearly mark vessel's name. 46 CFR 122.604(b)

Your vessel must undergo an annual inspection within the 3 months before or after each anniversary date. Vessel did not complete annual inspection within the alloted time frame. Conduct satisfactory annual inspection to the satisfaction of the MI prior to the carriage of passengers. 46 CFR 115.500(b)(1)

..Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. Vessel's reduction gear box cooler failed. Prior to the carriage of passengers, carry out gear box cooler repairs and conduct internal sea trials to the satisfaction of the MI. 46 CFR 119.100

A door (A-Class fire control boundaries) must be of self-closing type capable of closing against a 3.5 degree list of the vessel. During inspection, fire service door at midhsips, port side, would not latch closed when activated from the control space.

Except for engine exhausts, each inlet or discharge pipe that penetrates the hull below a line drawn parallel to and at least 6 inches (15.2 centimeters) above the deepest subdivision load line must have means to prevent water from entering the vessel if the pipe fractures or otherwise fails. Vessel is equipped with drip-less shaft seals. During inspection, Marine Inspector found that port shaft seal had slipped underneath the tensioning device. Engineers were able to replace tensioning device and repair the shaft seal installation to the satisfaction of the attending Marine Inspector.

Each rescue boat and ring life buoy must be marked with Type II retro reflective material approved in accordance with 46 CFR 164.018 or other standard specified by the Commandant. Life buoys on the upped deck, port and starboard have worn retro reflective tape that needs replacement.

Electric installations on vessels must ensure protection of passengers, crew, other persons, and the vessel from electrical hazards. There is a unterminated electrical wire passing through frame 45 that must be removed and the cable pass through sealed.

Exhaust gas must not leak from the piping or any connections. There is an exhaust leak from the turbo on the port main engine.

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull. There is a crack under a Tank Support, forward of frame #47, Longitudinal Frame, port side, keel that needs repair.

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull. Second deck port Balcony Support has been damaged and must be repaired.

A vessel must be provided with a satisfactory arrangement for draining any watertight compartment, other than small buoyancy compartments, under all practicable conditions. Leaking bilge pipe in port fuel tank must be repaired.

Nonconducting mats or grating must be provided on the deck in front of each switchboard and, if accessible from the rear, on the deck in the rear of the switchboard. Nonconducting mats must be provided in the starboard false stack compartment and E/R switchboard.

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. Condense kits and expired items must be replaced.

Each lighting fixture globe, lens, or diffuser must have a guard or be made of high strength material. Port main deck outdoor light is missing its bulb and protective cover.

Fuel oil found in engine room bilge pocket. Remove and clean area.

SWBD panel not properly sealed; motor controller (MC) for Generator breaker missing. Provide temporary seal over MC till spare part becomes available and make permanent repairs.

Engine room entry fire extinguisher missing from storage bracket. Locate and install fire extinguisher.

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of machinery, fuel, and piping systems, including the following: Inspection of all fuel systems; Substantial fuel leak found on outboard side of port engine.

Fire hazards to be minimized. The general construction of the vessel must be such as to minimize fire hazards insofar as it is reasonable and practicable. Missing insulation found on fire boundary bulkhead on forward side of engine room. Correct by the next credit drydock.

A vessel is inspected for compliance with the standards required by this subchapter. Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. Vessel window on main deck port bow missing with temporary repair in place. Correct by next credit drydock

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspection of lifesaving equipment and systems, including the following: Inspection of each life jacket, work vest, and marine buoyant device. Two life jackets require renewed retro reflective tape and one life jacket found with large tear at the base.

Each survival craft must be secured to the vessel by a painter with a float free link permanently attchaed to the vessel. Manual release mechanism impedes operation of hydrostatic release. Arrangement shall be made to insure all hydrostatic releases are fully operational independently prior to carrying passengers.

Each life jacket must be marked with Type 1 retroflective material. 107 adult and 28 children life jackets with worn and non operational reflective tape. Replace with new reflective tape.

Each vessel must have adaquete emergency lighting fitted along the line of escape to the main deck from passenger accomodation spaces. 02 emergency lights inoperable. Replace in-kind.

Fire control boundary must be constructed and capable of preventing the passage of smoke and flame for one hour. MI observed three holes that penetrated through A60 door fwd port.

Penetrations in A-Class fire control boundaries for electrical cables must be constructed to prevent the passage of flame and smoke for one hour. HVAC room on upper deck is listed as an A-0 boundary and has a multi-cable transit (MCT) without fire protection installed. Install proper fire protection in MCT. 46 CFR 116.415(a)(2)(ii)

A-Class bulkheads must be composed of steel or equivalent material. In addition, they must be so insulated with approved structural insulation. Forward aluminum bulkhead in HVAC room on upper deck is listed as A-0 but is not fitted with insulation. Utility closet (A-0) on upper deck is also missing insulation on two bulkheads. Install proper insulation for both spaces to meet A-0 standard prior to next credit drydock. 46 CFR 116.415(a)(2)(i)

The cognizant Officer in Charge, Marine Inspection (OCMI) determines the total number of persons permitted to be carried on a vessel. The vessel is restricted to operating with a reduction of 150 passengers, with an additional limitation of 50 passengers maximum allowed on the uppermost (Sun) deck. The total number of passengers allowed on the vessel is 600. This restriction will be lifted upon successful completion of a Stability Test and receipt of an approved Stability Letter by the USCG Marine Safety Center. 46 CFR 115.112

At each initial and subsequent inspection for certification, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspection of its fire protection equipment. All portable fire extinguishers onboard were overdue for servicing. Service portable fire extinguishers and provide documentation prior to carrying passengers. 46 CFR 115.810(a)

When in accordance with 170.120 in subchapter S of this chapter, a vessel must be provided with a stability letter. Vessel does not have an updated stability letter from recent incline test that was completed. Provide approved stability letter prior to carrying passengers. 46 CFR 115.306

Double doors capable of independent operation and latching may have a clearance between the doors of not more than 3.2 millimeters. However, if one door must always be closed first, means shall be provided to ensure that the doors close in the proper order. Self closing doors on starboard side main separating accommodation spaces would not fully close. Prove proper closure of doors. 46 CFR 116.435(b)(5)(i)

Each ring life buoy on a vessel must be approved in accordance with 160.050 in subchapter Q of this chapter. Starboard Sun Deck life buoy has faded vessel name lettering and wasted retroreflective tape. Main Deck life buoy outer material is peeling from the main body. Repair or replace life buoys. 46 CFR 117.70(b)(1)

A fire control plan (FCP) must be posted on the vessel in a location that is accessible and visible to all passengers. The plan must show escape routes, the location of fire protection/emergency equipment, compartment titles and hazard classification of accommodation and service spaces, and structural fire protection boundaries. FCP not posted on Sun Deck and Upper Deck IAW shipboard FCP. Current FCP does not list compartment titles and structural fire protection boundaries. Update FCP and post in designated locations. 46 CFR 116.530

Stations in the maritime service must be licensed by the FCC either individually or by fleet. Vessel did not have FCC Station License onboard. Provide valid station license. 47 CFR 80.13(a)

Propeller has a crack in the blade root of one of the 5 propeller blades. The crack was reported as being approximately 75% of the blade length opened up to about 1/8” gap. Operator voluntarily taking boat out of service and vessel needs to complete drydock prior to carrying passengers.

Determine extent of corrosion in the water tank void and provide repair plan for the next drydock. Deepest pitting foung 0.2 inch.

Repair temporary pact on sewage piping.

Permanently repair bilge suction line.

Prove proper operation of 2nd deck port side fwd outboard fire door. Vessel required to carry additional crew member above minimum requirement on vessel's COI to manually operate door. Crew member shall be posted on the 01 deck.

Vessel shall conduct an operational test with a CG Marine Inspector. Test underway with varying speeds and normal maneuvering.

Make adjustments to the stowage arrangements of all IBAs on board to comply with manufacturer recommendations and to ensure float free arrangement is made.

Make permanent repair to cracked bilge manifold pipe located in engine roomm and prove proper operation to attending marine inspector.

Found both engine room supply fan fire dampers and port side engine room fire damper to not be completely operable. When fixed fire fighting system tested, dampers fail to close completely. Dampers need to be repaired/adjusted and tested to the satisfaction of the attending USCG inspector prior to carrying passengers. 46 CFR 116.610(g)(2)

Port side aft tonnage opening, which had been previously welded in place, was removed and damage was noted around the mating surfaces. Demonstrate proper fit up of tonnage opening to provide adequate resistance to water intrusion from rain and seaspray.

As per stability letter dated 6/July/2007 serial #H2-0702137, vessel is restricted to a maximum of 632 persons of which 624 may be passengers. An amended COI will be issued to reflect this change.
