During the lowering of the lifeboat, the PSCO observed that the wire rope used to lower the lifeboat from inside the craft had jumped off the sheave. As a result, the lifeboat could not be lowered reliably using the controls from inside the craft.
Condition: Improper/Lack of Maintenance
Action required: 17 - Rectify deficiencies prior to departure

Resolution: Received DNV Class report stating the sleeve bearing was lubricated and shaft bolt tightened to minimize clearance between the roller and the guard. Satisfactory high speed lowering test was completed and witnessed by surveyor.

PSCO observed cut wires in multiple locations that crew was unable to determine if the wires were live or not. These wires were wrapped in electrical tape, but otherwise unprotected, creating an unsafe practice for the crew.

Resolution: Received DNV class report stating. The cut wires, wire-to-wire connections wrapped only with electrical tape, and other unprotected wiring associated with the bridge glass heating equipment were removed and properly reinstalled.
