Vessels described by 46 CFR 175.110(c) must have an interconnected fire detection system in compliance with § 181.450 installed in all enclosed areas where passengers and crew have routine access, including accommodation spaces and machinery spaces. Installed fire detection system reported inoperable. The following requirements shall be met while fire detection system is inoperable: - CCTV in engine room shall be monitored by the crew while vessel is underway with passengers - A crewmember shall make rounds of the engine room every 30 minutes while the vessel is underway and log completion of the rounds in the vessel logbook Permanent repairs to the fire detection shall be made within 14 days. 46 CFR 181.405

Each survival craft and rescue boat on a vessel must be in good working order and ready for immediate use before the vessel leaves port and at all times when the vessel is underway. Center, foward life floats basket found broken. Port side life floats is degraded. Repair/replace lifefloats to satisfaction of MI. 46 CFR 185.700

All vessels shall be provided with a satisfactory arrangement for draining any watertight compartment. Small leak was identified on bilge piping for engine room. Repair leak to satisfaction of MI. 46 CFR 182.25-1 (OLD T)

A weathertight door with permanent watertight coamings must be provided for each opening located in a deck house. Starboard side weathertight door is missing lower closure device . Install closure device to satisfaction of MI. 46 CFR 171.124 (OLD S)

One length of fire hose shall be provided for each fire hydrant required. Port side fire hose was found holed. Hose was replaced with new hose. 46 CFR 181.15-10 (OLD T)

Have nozzle that has a 1/2 inch solid stream orifice or a combination nozzle by the Commandant iaw 162.027-6 of this Chapter. The nozzle must be bronze or another material that has a strength and corrosion resistance properties equal to those of bronze. The current nozzles on both the port and starboard side fire hoses are made of plastic. Replace with ones made of bronze or a material of equal strength and durability. 46 CFR 181.15-10(d)(2) "OLD T"

All wiring and cables shall be suitable for marine service and for the particular installation used. Dead-end wires were found above the port-side generator. Remove dead-end wires to the satisfaction of the attending Marine Inspector (MI). 46 CFR 183.10-20(a) "OLD T"

If 1 1/2 inch diameter fire hose is used, each length of hose must: (1) Be lined commercial fire hose that conforms to Underwriters' Laboratories, Inc. The fire hose on the starboard side was found to have holes and was leaking. Replace the hose to satisfaction of attending MI.

Except as otherwise provided in this section, it shall be the duty of the master to prepare and post emergency checkoff list in conspicuous place accessible to crew & passengers. No emergency instructions were available. 46 CFR 185.20-1(a) "OLD T"

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances. Vessel has cracked gusset in the bilge two frames aft of the collision bulkhead. Repair to satisfaction of Marine Inspector (MI). Provide repair proposal prior to commencing work.

Except as provided otherwise in this subpart, all electrical equipment including motors, generators, controllers, distribution panels, etc., shall be dripproof protected. 220 V panel in second void aft of collision bulkhead is severely worn/holed. Repair/replace to satisfaction of MI.

The minimum capacity of the power fire pump shall be 50 gallons per minute at a pressure of not less than 60 pounds per square inch at the pump outlet. The pump outlet shall be fitted with a pressure gage. Fire pump pressure only reached 50 psi on gauge. Repair to satisfaction of MI.

A battery charger intended for connection to a commercial supply voltage shall employ a transformer of the isolating type. An ammeter that is readily visible shall be included in the battery charger circuit. Port and starboard battery banks wired in series do not have ammeter in the charging circuit. Install ammeter to satisfaction of MI.

When a deck scuttle serves as a means of escape, it must not be less than 455 millimeters (18 inches) in diameter and must be fitted with a quick acting release and a holdback device to hold the scuttle in an open position. Deck scuttle for emergency exit from engine room cannot be opened from inside the space and does not have a hold back device. Install hatch opening and hold back devices to satisfaction of MI.

Except as provided in paragraphs (c) and (d) of this section and in § 170.200, the owner of a vessel must conduct a stability test of the vessel and calculate its vertical and longitudinal centers of gravity and its lightweight displacement. Vessel is currently undergoing three modifications. Vessel passenger restrictions are as follows until new stability test is completed after modifications: Exposed routes: Upper deck limited to 10 passengers. Main deck limited to 100 passengers. 110 total passengers. Partially Protected routes: Upper deck limited to 23 passengers. Main deck limited to 100 passengers. 123 total passengers.

Each disposable hydrostatic release unit must be marked with an expiration date of two years after the date on which the unit is installed. EPIRB hydrostatic release expired October 2022. Trips with passengers limited to 3nm offshore until hydrostatic release is renewed to the satisfaction of the attending inspector. 46 CFR 185.740(b)

All vessels shall be provided with a satisfactory arrangement for draining any watertight compartments. Pin hole leak was found on starboard bilge overboard between hull and positive valve. Repair hole to the satisfaction of attending inspector. 46 CFR 182.25-1(a) "OLD-T"

Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers. No instructions for donning were posted. Post instructions to the satisfaction of attending inspector. 46 CFR 185.516

Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. Child life jacket stowage on port side top deck did not contain child sized life jackets. Relabel stowage to the satisfaction of the marine inspector. 46 CFR 180.78

The owner or managing operator shall notify the cognizant OCMI as far in advance as possible whenever a vessel is...to undergo...alterations affecting the safety of the vessel, together with the nature of any...alterations contemplated. Two smaller generators (port and starboard)replaced the single generator (located at the centerline) that used to be on the vessel. Plan review conducted by the Marine Safety Center (MSC) is required for the 02 new generators. Once obtained, email the Project Number from the MSC to SDB. If the plans are approved by the MSC, contact SDB to schedule a Marine Inspector(s) to attend the vessel to operationally test the 02 generators.

Each vessel must meet the applicable requirements in Subchapter S of this chapter. There have been weight changes since the purchase of the vessel, such as installation of 02 generators and changing the sewage tank. Submit a record of weight changes to the Marine Safety Center to determine if a new stability test is required. Cite: 46 CFR 177.13-1 "OLD T"

A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a drydock examination and an internal structural exam at least once every two years. [name withheld] missed their DD/ISE date 30NOV2022. Must complete a DD/ISE prior to carrying passengers.

MI observed all life floats that were daisy chained between each other were not long enough. when deployed the life float would be in contact with each other. Require VSL to make the connecting lines longer. 46cfr180.137(e)(3) 50c

MI observed a water light on the life float seal failed and took on water. Require VSl to replace seal. 46cfr180.175(d) 50c

MI observed no hatches at captive chains. Require VSL to attach captive chain to all hatches. 46cfr179.360(b) 50c

MI observed fuel shutoffs were not labeled. Require VSl to label the fuel shutoffs. 46cfr185.608 50c

MI observed the port engine did not have guards over the fly wheel and alternator. Requiring to put a guard in place of all exposed and hazardous places. 46cfr177.35-15(a) 50c

Mi observed no amp meter on any of the battery charges. Requiring VSl install amp meter or battery charger with AMP meter. 46cfr183.05-20(f) 50c

MI observed no interlock switch on switch board preventing the shore tie power from coming on with the generator on line. Require interlocker for switch board to not be able to energize shore power and gen power at the same time. 46cfr183.01-10(a) 50C

Mi observed inside the accommodations area that there were holes/ cut outs made in the framing, allowing water in and possible trapping it in the accommodation area. Require VSl permanently seal those cut outs that will make it water tight not allowing water inside the accommodations area. 46cfr177.13-1 50C

To maintaina valid Certificate of Inspection you must complete your annual inspection within the periods specified in 176.500(b)(1) and your Certificate of Inspeciton must be endorsed. Per 46 CFR 176.502, vessel shall not carry passengers until a satisfactory annual inspection is compeleted.

All observed unsafe practices and hazardous situations be corrected. Hydraulic non-metallic piping for steering failed. Replace piping. 46 CFR 176.25-50 (Cite not listed in MISLE)

Each vessel operating on an oceans route in warm water be provided life floats for 100% total persons on board. One 25-person life float unsatisfactory. Replace life float. Vessel is restricted to 75 total persons on board.

An adult life jacket must be provided for each person. One life jacket unsatisfactory. Replace life jacket. Full carriage of passengers is dependent on correction of item #2.

Vessel's current route requires an EPIRB. EPIRB battery is expired. Replace battery. Vessel is restricted to not more than 3 NM offshore until replaced.

Suitable guards shall be installed in way of all exposed machinery. Starboard engine PTO missing guard. Install guard.

All electrical installations shall be to the satisfaction of the [name withheld] room lighting receptacle junction box is loose; repair junction box. Operator console has dead-end wires; terminate properly. 46 CFR 183.01-15 (Cite not listed in MISLE)

Radio installations shall be in accordance with 47 CFR 80. Vessels over 20 meters require a Bridge-to-Bridge Certificate. Obtain certificate. 46 CFR 184.25-1 and 47 CFR 80.1001

A vessel must carry adequate and up-to-date charts and Coast Guard Light List. Vessel did not maintain up-to-date chart corrections or light list. Provide chart corrections and light list.

A penetration in a watertight bulkhead must be made watertight. Observed hydraulic line penetration through the collision bulkhead. Provide proof that the penetration is watertight.

Vessels operated beyond 20 nautical miles but not more than 100 nautical miles from the nearest land must be equipped with a VHF-DSC radiotelephone installation and a MF-DSC frequency transmitter. Both the vessel's VHF radios lacked DSC capability, as well as the SSB radio. Install 02 VHF-DSC radios and 01 MF-DSC radio. 47 CFR 80.905(a)(1) & (2).

A vessel of more than 65 feet must carry a minimum of three life buoys. Ring life buoy mounted outside of wheelhouse is unserviceable. Replace ring life buoy with one approved IAW 46 CFR 160.050.

Electrical installations on vessels shall be such that the safety of paasengers, crew, and vessel from electrical hazards will be assured. Identified dead end cable STBD side engine room from electrical panel. Remove dead end cable.

Auxiliary machinery shall be suitable in type and design for the purpose intended. Identified plastic zip ties being used as a bracket. Install permanent bracket to secure generator exhaust outlet.

Propulsion machinery shall be suitable for operation at constant marine load. Identified loose shaft packing bolts on STBD main engine. Tighten shaft packing.

The construction and arrangement of a vessel must allow the safe operation of the vessel. Identified corrosion around access to sewage discharge valve on fantail. Submit repair proposal for review and approval.

Replace expired hydrostatic release for EPIRB.

Replace wiring to fresh water pump.

Reconfigure weak-link on both painters for float-free arrangement.

Repair 07 adult lifejackets and replace 01 adult lifejacket.

Re-stencil 02 liferings on main deck.

Replace flexible fuel line hose to generator.

Install clamps to fuel lines to fuel pump on generator.

Repair rudder installation to make rudder stops effective.

Make permanent repairs to weld fractures found under stbd rudder bracing at frames. Crop/renew frame work. Send written repair proposal prior to starting work. Make rounds to inspect cracked welds prior to getting underway and upon return from sea. Log rounds/imspections in log book.

Vessel must have 02 alcohol test stripes on board. Vessel must be able to test within 2 hours of incident.

Passenger count on top deck must be marked.

Hydraulic line penetration through collision bulkhead must be watertight.

Lifejacket stowage must be marked.

Alternator belt on port side engine must be replaced.

Hotstart junction box on port engine must be replaced.
