TODD G suffered a loss of steering and self reported to Marine Safety Unit St. Thomas. MI attended the vessel while technician was on board. Technician stated the steering control box switch at the operating station was replaced with the incorrect type and caused the fuse to blow. As a result, steering was lost. The technician replaced the steering control box switch again with the correct switch. MI conducted steering test from the bridge and it was discovered the rudder angle indicators did not work from either operating station. MI conducted steering test from the deck above rudder room and witnessed rudders swing full port/starboard, test was satisfactory. Provide report (worksheet) from technician stating the root cause and analysis for loss of steering to MSU St. Thomas. This shall also include the repairs that were made with parts listed. Prove proper operation of rudder indicators at both operating stations no later than December 18, 2025. Provide CG-2692 to MSU ST. Thomas.

Testing equipment was brought to the vessel and Attending MI’s witnessed successful testing. Vessel operators need to have available at all subsequent inspections; Canned smoke for testing of smoke detectors and a heat sourced designed for the testing of heat detecting head units. If your vessel's Certificate of Inspection is renewed less often than annually, the marine inspector must check the settings of the safety or relief valves on all pressure vessels, except cargo tanks, twice every 5 years. No more than 3 years may elapse between any check and its immediate predecessor. Engine room air flasks relief valves have not been tested recently. Attending MI’s witnessed the testing of both port and starboard air flasks in the after portion of the engine room. When pressurized the starboard relief valve released at 165 PSI well under the 200 MAWP of the air flask. The port relief valve failed to release automatically or manually and needs to be replaced. It’s recommended that the vessel operators establish some type of documented tracking to ensure tests are completed twice in 5 years The unloader relief valves on the compressor heads should also be tested within the same time parameters to to verify proper operations. 46CFR 182.330 46CFR 61.10-10 (i)

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection. In accordance with the COI, "The loading ramp shall be locked in the up and closed position while the vessel is underway." Marine Inspectors observed the TODD G was operating with the loading ramp in the unlocked partially open position and was missing a padeye and locking pin on the STBD side of the ramp.
