After any survey of the ship under regulations 7,8,9 or 10 has been completed, no change shall be made in structural arrangements, machinery, equiptment and other items covered by the survey without the sanction of the administration. . Permanant structural modifications in the form of welded brackets and padeyes were made to carry containers on deck and in cargo holds, after survey without the sanction of the administration.

Personnel responsible for the operation of an incinerator installed in accordance with the requirements of paragraph 6.1 of MARPOL Annex VI shall be trained to impliment the guidance provided in the manufacturer's operating manual. A can, which is not approved by the incinerator's manual was found in the incinerator.

Resolution: MSD Fort Macon rcvd a class report from NKK. Class issued a due date of 30DEC16 for the inop lifeboat engine. See attached documentation for details.
