All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules. Marine Inspector identified that the masthead light was inoperable. Conduct repairs to the satisfaction of the Marine Inspector. 46 CFR 183.420

Unless other means are provided to ensure that a potential waste receptacle fire would be limited to the receptacle, waste receptacles must be constructed of noncombustible materials with no openings in the sides or bottom. Marine Inspector identified that some waste receptacles on board the vessel were made of combustible materials. Replace waste receptacles with those constructed of noncombustible materials. 46 CFR 177.405(f)

Each rescue boat and ring life buoy must be marked with Type II retro reflective material approved in accordance with 46 CFR 164.018 or other standard specified by the Commandant. Ring buoy missing reflective tape and was corrected during exam.

The bilge suctions shall be fitted with suitable strainers having an open area not less than three times the area of the bilge pipe. ENG room bilge suction is insufficient size. Provide bilge suction strainer that meets this regulation to satisfaction of attending Marine Inspector. 46 CFR 182.25-5 (b) "OLD-T"

The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel to determine the presence of alcohol in the system of each individual who was directly involved in the SMI. Test strips on vessel are expired. 46 CFR 4.06-15

The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing the actions to be taken in the event of fire, heavy weather, or man overboard conditions. Vessel missing complete emergency instructions posted. Add missing heavy weather and man overboard instructions. 46 CFR 185.510 (a)

When operating a vessel on a body of water where the discharge of treated or untreated sewage is prohibited by the Environmental Protection Agency under 40 CFR 140.3, the operator must secure each Type III device in a manner which prevents discharge of treated or untreated sewage. Acceptable methods of securing the device include - (1) Closing each valve leading to an overboard discharge and removing the handle; (2) Padlocking each valve leading to an overboard discharge in the closed position; (3) Using a non-releasable wire-tie to hold each valve leading to an overboard discharge in the closed position. OVBD Discharge is currently secured with a plastic tie wrap. Provide an acceptable method of securing the valve to satisfaction of attending Marine Inspector. 33 CFR 159.7 (c)

For a vessel described by 46 CFR175.110(c), the training program in paragraph (a) of this section must address firefighting proficiency and must include all items referenced in this part. Provide required training and log into vessels training/drills logs. Federal Register/V.86 No. 245 46 CFR 185.420

"All vessels shall be provided with a satisfactory arrangement for draining any watertight compartment" Forward void missing bilge strainer. 46 CFR 182.25-1

"For fixed-gas fire extinguishing systems, the owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector." Vessel missing documentation of Global Fire report. 46 CFR 176.810

"A vessel must be fitted with ground tackle and mooring lines necessary for the vessel to be safely anchored and moored. The ground tackle must be satisfactory and subject to the approval of the cognizant OCMI." Corroded anchor chain shackle linkage. 46 CFR 184.200

"All cable and wire must be installed in accordance with paragraph 14.h of ABYC E-8 and paragraph 15.h of ABCY E-9)." Multiple cable and wire transits connecting to battery bank. Current set up does not meet ABYC standards. 46 CFR 183.340 Submit plans to OCMI for plan review.

Each survival craft must be secured to the vessel by a painter with a float-free link permanently attached to the vessel. Vessel's painter was improperly attached to the float free link for the inflatable buoyant apparatus.

On a vessel of at least 26 feet in length, a visual and audible alarm must be provided at the operating station to indicate a high water level. Vessel's forepeak void did not have an audible bilge alarm.

Attachment plugs for portable lamps, tools, and similar apparatus operating at 100V or more, must have a grounding pole and a grounding conductor in the portable cord. Vessel's plug in 120V space heater in the accommodation space is not grounded and is unsuitable for marine application. 46 CFR 183.10-20(o).

The person in command of a documented vessel must have on board that vessel the original Certificate of Documentation currently in effect for that vessel. COD on board listed vessel under previous name of New Salmon Queen and was expired. 46 CFR 67.313

Every vessel subject to Part III of Title III of the Communications Act must have a detailed inspection of the radio installation by an FCC-licensed technician in accordance with §80.59 once every five years. FCC radiotelephony did not have EPIRB listed as radio installations on board the vessel. 47 CFR 80.903

Man overboard drills and training shall be logged or otherwise documented at least once within a 3 month period. The drill entry shall include the date of the drill and training; and general description of the drill scenario and training topics. 46 CFR 185.520

As appropriate for the intended voyage, a vessel must carry adequate and up to date charts of large enough scale to make safe navigation possible. Vessel did not have up to date charts onboard. 46 CFR 184.420(a)(1)

Each life jacket carried on a vessel on oceans, coastwise, or Great Lakes route, must have a life jacket light. 01 life jacket onboard was worn and inoperable. 46 CFR 180.75(a)

Flexible nonmetallic hose used in vital piping systems must meet SAE J-1942 or be specifically approved by the Commandant. The flexible fuel hoses and priming bulbs on the main engine fuel filters are not of an approved type. Remove permanent installation of hoses and install metallic cap to fitting to the satisfaction of the Marine Inspector. 46 CFR 182.720(e)

A bilge suction must be fitted with a suitable strainer having an open area not less than three times the area of the bilge pipe. The strainers for the installed bilge system do not have an open area of three times the bilge pipe. Install bilge strainers that meet the requirements of 46 CFR 182.510 to the satisfaction of the Marine Inspector. 46 CFR 182.510(b)

An EAP training program must be conducted for the employer's crew members and supervisory personnel. The Captain did not receive at least 60 minutes of EAP training. Execute training requirements IAW 46 CFR 16.401 to the satisfaction of the Marine Inspector. 46 CFR 16.401(b)

All marine employers must submit drug testing program data required by 49 CFR 40.26. The vessel could not provide proof of MIS submission. Provide proof that the MIS Data Collection Form IAW 46 CFR 16.500 has been submitted. 46 CFR 16.500(b)

Each vessel certificated to operate on a coast wise route in cold water must be provided with inflatable buoyant apparatus of an aggregate capacity that will accommodate at least 100% of the total persons permitted on board. As per the vessel's COI, the IBA on board only has a capacity to accommodate 71% of the total persons permitted on board. Provide survival craft to meet requirements of 46 CFR 180.204 by the next Annual Inspection. *Formal notification letter to follow. 46 CFR 180.204(b)

Vessel's fixed bilge system is inoperable. 46 CFR 182.500

Vessel does not have closure devices for supply duct openings for the engine room which is protected by a fixed gas fire extinguishing system. 46 CFR 182.465(h)

Lighting fixtures are being used as connection boxes/outlet to supply other equipment. 46 CFR 183.410

The belt on the forward part of the port engine is exposed and is a hazard. 46 CFR 177.960
