At each initial and subsequent inspection for certification, the owner or managing operator must be prepared to conduct tests and have the vessel ready for inspection of its fire protection equipment, including the following: Inspection of fixed gas fire extinguishing system. During operational test of engine room pre-engineered fire fighting system, engines were unable to restart when activating the override function.

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Electrical distribution panel in wheelhouse is missing labels for several breakers.

On a vessel of at least 26 feet in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in the spaces provided in 46 CFR 182.530(a). The vessel's audible alarm was not functioning.

For a vessel described by 46 CFR 175.110(c), the training program in paragraph (a) of this section must address firefighting proficiency and must include, but need not be limited to— (1) Training in the use and location of firefighting equipment and general firefighting knowledge, including: (i) Location of firefighting appliances and emergency escape routes; (ii) Types and sources of ignition; (iii) Flammable materials, fire hazards and spread of fire; (iv) The need for constant vigilance; (v) Actions to be taken on board; (vi) Fire and smoke detection and automatic systems on board; and (vii) Classification of fire and applicable extinguishing agents. Provide required crew training and document in drill log. 46 CFR 185.420 (b)

Except as noted in paragraph (c) of this section, stations in the maritime service must be licensed by the FCC either individually or by fleet. Vessels FCC station license is expired. Provide proof of current, vailid Station License. 47 CFR 80.13 (a)

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. The following contents are expired: Eyewash saline, Aspirin. Provide first-aid kit with servicible contents. 46 CFR 184.710

Fuel cocks are prohibited for use in fuel lines except those of the solid bottom type with tapered plugs and union bonnets. The vessel did not have the approved fuel cocks connected to its fuel filters. 46 CFR 182.20-40 (a)(4)(OLD T)

Machinery and fuel tank spaces shall be separated from accommodation spaces by watertight and/or vapor tight bulkheads. Engine room had a sewage holding tank vent pipe that penetrated the watertight bulkhead that needs vapor seal. 46 CFR 182.35-1(a)(OLD T)

On a vessel of at least 26 ft in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in normally unmanned spaces. The bilge high level alarms did not indicate which space the high level alarm was going off on. 46 CFR 182.530(a)

The discharge ends of fuel tank vent pipes must be fitted with removable flame screens or flame arresters. The starboard fuel tank vent pipe did not have a removable flame screen. 46 CFR 182.450(e)

Vessels operated beyond the 20 nautical mile limitation, but not more than 100 nautical miles from the nearest land, must be equipped with a Medium Frequency- Digital Selective Calling frequency transmitter. The vessel did not have the corresponding MMSI number programmed within the radio to ensure proper DSC transmission. 47 CFR 80.905(a)(2)

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock, and minimize accidental personnel contact with energized parts. Dead end wire on PME Alternator. Remove unused wire or connect to adequate equipment.

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected al required guards and protective devices must be in satisfactory condition. Excessive amounts of oil seen leaking from PME and SME on fuel injector lines. Determine source of oil and prevent further discharge. 46 CFR 176.830 (a)

The intent of this part is to insure that the main propulsion machinery installed on each vessel is suitable for the vessel and its operation. It is further intended that all machinery be installed and maintained in such a manner as to afford adequate protection against personnel injury. Discovered missing guard over gear driven electric clutch washdown pump on port engine, presenting a potential safety hazard to the crew.

This part is intended to provide a minimum standard of regulations for electrical installations and equipment that will result in a vessel that is reasonably safe from fire caused by and or originating in electrical equipment, and reasonably safe from electrical shock of personnel. Discovered exposed wires in fwd engine room and center compartment, presenting potential hazards to crew.

Each window and its attachment to the deckhouse, must be capable of withstanding the maximum load from wave and wind conditions. Port forward and aft window on deckhouse is cracked. Replace window with glass or other glazing material that will not break into dangerous fragments if fractured. 46 CFR 177.1020

All cable and wire must be installed with metal supports not spaced more than 24 inches apart. Wires in center compartment space is hanging and is required to be adequately supported. 46 CFR 183.340

All electrical installations shall be to the satisfaction of the [name withheld] for light over SSDG is loosely installed causing the light to go on and off when touched. 46CFR 183.01-15

Vapor Barriers must be provided in spaces where vapors are present such as a machinery space. Aft bulkhead port side of Engine room has two inch hole in bulkhead. Seal hole to make vapor tight. 46 CFR 177.405 (e)

Each Life Jacket must be marked with Type I retroflective material approved in 46 CFR 164.018. 5 Adult life jacket retroflective tape is worn. 46 CFR 185.604 (h)

An adult life jacket must be provided for each person carried on board a vessel. Two Kapok life jackets were not serviceable. 46 CFR 180.71 (a)

Each connection to a conductor or terminal part of a conductor must be made within an enclosure. Vessel has installed "loose" 12V bait pump switch (main cabin below decks) in wiring secured with a copper conductor strap. Vsl should use switch box or fabricate panel for mounting switch safely using proper connectors.

Ring life buoys: A vessel must carry one floating waterlight. Vessel's waterlight does NOT float. Vessel needs to replace or repair floating waterlight.

When a marine employer determines that a casualty or incident is, or is likely to become, an SMI, the marine employer must ensure that alcohol and drug testing is conducted (alcohol testing of an individual must be conducted within 2 hours of when the SMI occurred). No alcohol testing strips onboard during inspection. Vsl on 100 mile oceans route, should carry alcohol test strips onboard.

As appropriate for the intended voyage, a vessel must carry Charts, Coast Pilot [name withheld] and Inland Nav Rules. All found to be were dated (2015-16, and Inland Rules version B). Vsl needs to update charts and publications.

Secure/remove dead end wiring on bridge.

Replace worn emergency broadcast placard.

Change stowage location of ring buoy to a space where it is readily accessible and stowed in a way that it can be rapidly cast loose

Store all lifejackets in passenger accessible space

Vessel route is restricted to within 20nm of shore until the single side band is onboard and operational. 47CFR80.905 & 80.913
