Exposed hatches shall be provided with efficient watertight cover. Both, the starboard and port engine room escape hatchways had gaskets that were coming off of the hatchway, rendering the hatchways not watertight.

The means for securing the weathertight doors shall consist of gaskets and clamping devices that can be operated from both sides. The gasket on the emergency generator door was worn and the bottom left clamping device was inoperable.
