Each window must be capable of withstanding the maximum load from wave and wind conditions expected due to its location on the vessel and the authorized route of the vessel. Windows were cracked during repair procedures. Replace all broken windows to the satisfaction of the MI prior to credit drydock. (46 CFR 177.1020)

The intent of this part is to insure that the main propulsion machinery installed on each vessel is suitable for the vessel and its operation... for the purpose intended. It is further intended that all machinery be installed and maintained in such a manner as to afford adequate protection against fire, explosion, machinery failure, and personnel injury. MI observed fuel leak from the STBD fuel pump on the vessel’s STBD engine. 46 CFR 182.05-5 (Old T)

The general construction of the vessel shall be as to minimize fire hazards insofar as reasonable and practicable. MI observed 03 cable penetrations in the FWD bulkhead of the engine room and STBD fuel shutoff mount not secure, failing to maintain a vapor tight boundary. 46 CFR 177.10-5 (Old T)

Service use of distress signals shall be limited to a period of 3 years from date of manufacture, and replacement shall be made no later than...after the date of expiration. Vessel had 06 expired flares, replace with red flares to the satisfaction of the MI. 46 CFR 180.35-1 (b) (Old T)

For a vessel described by 46 CFR 175.110(c), the training program in paragraph (a) of this section must address firefighting proficiency. Vessel had limited logs detailing crew training for vessel's oceans route. Crew training program must address the listed firefighting proficiencies to improve crewmember. 46 CFR 185.420(a)

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: U.S. Coast Pilot and Coast Guard Light List. Vessel did not have current Coast Pilot and Light list, procure items to the satisfaction of the MI. 46 CFR 184.420 (a)(2)/(3)

Remote fuel shutoff stations shall be marked in at least 1-inch letters indicating purpose of the valve and direction of operation. No lettering was present, label to the satisfaction of the MI. 46 CFR 185.30-20(Old T)

Life floats and buoyant apparatus, together with their oars and paddles, shall be conspicuously marked with the vessel's name. No labeling was present on life float paddles, label wiht vessel's name to the satisfacition of MI. 46 CFR 185.30-5 (Old T)

A marine employer whose vessel is involved in a casualty or incident that is, or is likely to become, a serious marine incident as defined in § 4.03-2 of subchapter A of this chapter shall comply with the requirements of § 4.06 in subchapter A of this chapter. Drug & Alcohol swabs were expired, replace to the satisfaction of the MI. 46 CFR 185.212

For fixed-gas fire extinguishing systems, the inspections and tests required by Table 176.810(b) of this section, in addition to the tests required by 46 CFR 147.60 and 147.65. The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. If any of the equipment or records have not been properly maintained, a qualified servicing facility may be required to perform the required inspections, maintenance procedures, and hydrostatic pressure tests. Vessel is missing evidence of required pre-engineered fire extinguishing system servicing. Provide evidence of servicing report to Marine Inspector. 46 CFR 176.810 (b) (2)

Through hull fittings and shutoff valves shall be metal. In the case of nonmetallic hulls, materials which will afford an equal degree of safety and heat resistivity as that afforded by the hull may be approved. 01 Thru-hull in fore-peak and 03 thru-hull fittings in Engine Room are plastic and require replacement with metallic thru hull fittings to satisfaction of attending Marine Inspector. 46 CFR 182.40-1 OLD-T

Provision must be made for closing all supply duct cowls or scoops and exhaust duct discharge openings for a space protected by a fixed gas extinguishing system. All closure devices must be readily available and mounted in the vicinity of the vent. 08 Engine Room ventilation holes at bulwarks require readily available closure devices. Provide closure devices to satisfaction of Marine Inspector. 46 CFR 182.465 (h)

Each portable fire extinguisher on a vessel must be of an approved type. 03 Fire extinguishers (1 in pilothouse, 2 in Main Cabin) are invalid unless installed in correct bracket (Part Number listed on extinguisher). Provide proper mounting brackets for 3 fire extinguishers to satisfaction of Marine Inspector. 46 CFR 181.500 (a)

At each initial and subsequent inspection for certification the marine inspector shall examine the bilges and other spaces to see that there is no accumulation of oil or other matter which might create a fire hazard. Engine Room and Laz have excessive amounts of oil and hydraulic fluid in bilges. Clean bilges to satisfaction of Marine Inspector. 46 CFR 176.25-50 OLD-T

(b) For a vessel described by 46 CFR 175.110(c), the training program in paragraph (a) of this section must address firefighting proficiency and must include, but need not be limited to— (1) Training in the use and location of firefighting equipment and general firefighting knowledge, including: (i) Location of firefighting appliances and emergency escape routes; (ii) Types and sources of ignition; (iii) Flammable materials, fire hazards and spread of fire; (iv) The need for constant vigilance; (v) Actions to be taken on board; (vi) Fire and smoke detection and automatic systems on board; and (vii) Classification of fire and applicable extinguishing agents. (2) The drills required by § 185.524, including fire location and fire type; and (3) Emergency egress training for each member of the crew, to occur for all members of the crew— (i) At least monthly while such members are employed on board the vessels; and (ii) Each time a crew member joins the crew of such vessel. Provide required training and log into vessels log book. 46 CFR 185.420 (b)

Alcohol testing of each individual must be conducted within 2 hours. Alcohol testing strips were expired. Vessel restricted to voyages less than two hours.

If the life float or buoyant apparatus does not have a painter attachment fitting, a means for attaching the painter must be provided by a wire or line that: has a breaking strength that is at least the strength of the painter. Bracket securing wire connecting the line of life float to vessel was damaged and unable to secure life float with appropriate breaking strength. 46 CFR 180.20-1(3)(iii) [old-T]

Each officer employed on any vessel subject to this subchapter must have his or her license or merchant mariner credential onboard and available for examination at all times when the vessel is operating. Master’s original MMC not available onboard during inspection.

The discharge ends of vent pipes shall be fitted with removable flame screens or flame arresters. Starboard side fuel tank vent pipe is damaged, port side fuel tank vent pipe is missing. 46 CFR 182.20-35(d) [old-T]

Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. Drill records were not available for review.

At each initial and subsequent inspection for certification and reinspection the marine inspector shall inspect all lifesaving equipment to determine its serviceability. Starboard life float water light inoperable.

Employers must ensure that they and their crewmembers meet the requirements of this part. Captain was not listed on covered employee list provided by consortium. Provide updated list to reflect all crewmembers serving onboard.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date charts of large enough scale. Owner was missing charts. Provide current charts.

A vessel not required to have a power driven fire pump must have at least three 2/5 gallon buckets with an attached lanyard. The words FIRE BUCKET must be stenciled in a contrasting color. Two fire buckets were missing. Provide 3 fire buckets onboard within 7 days.

Employers must maintain records of chemical tests and must make these records available to Coast Guard officials. Records for negative test results for one year were not available. Provide records of test records for employees.

Where the station power does not exceed 1500 watts peak envelope power, the operator must hold a marine radio operator permit or higher class license. Master's operator permit expired 30MAR06. Provide proof of valid marine radio operator permit. 47 CFR 80.159

Each vessel certificated to operate on an oceans route in cold water must be provided with life floats of an aggregate capacity that will accommodate at least 100 percent of the total persons permitted on board. Port life float must be replaced due to a gash revealing the life float's core material. Vessel's passenger count is reduced to 22 passengers.

The master of the vessel shall ensure that the EPIRB's battery is replaced after it is used, or before the date required by the FCC regulations in 47 CFR part 80, whichever comes sooner. Vessel is restricted to operating within 3 NM of shore.

The float-free link required by 180.130 (a)(1) must be of proper strength for the size of the life float or buoyant apparatus as indicated on its identification tag. Both life floats need new weak links with proper identification tag indicating strength specification.

A vessel must carry one floating water light, unless it is limited to daytime operation, in which case no floating water light is required. The light attached to the ring buoy is inoperable. Vessel is restricted to daytime operation.

At all inspections special attention shall be paid to ensure that no fire hazards exist and that guards or protective devices are provided in all hazardous places. Vessel does not have a means to close supply vents to engine room in the event of a fire. Cite: 46 CFR 176.25-10 (b)

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date charts of large enough scale to make navigation possible. Charts on board are not up-to-date or of large enough scale. Cite: 46 CFR 184.420

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. Ammonia Inhalants are expired and must be replaced in vessel's First Aid Kit. 46 CFR 184.710

Nameplates shall be provided for each piece of apparatus to indicate clearly its service. Breakers on distribution panel in the engine room must be labeled to indicate the service of each breaker. 46 CFR 111.30-1 (h)

The inspection and maintenance of the equipment listed in paragraph (a) of this section shall be logged or otherwise documented for review by the Coast Guard upon request. Lifesaving equipment maintenance records were not available for inspection. 46 CFR 185.702 (d)

Any vessel of at least five net tons which engages in coastwise trade must have a Certificate of Documentation (COD) bearing a valid endorsement appropriate for the activity in which engaged. Vessel COD is currently expired. 46 CFR 67.7

The flame screen for the starboard emergency fuel shutoff reach rod was not properly secured. The valve handle must be shielded from flames by a noncombustible material. The owner will replace screws to properly secure flame screen. 46CFR182.455(b)(4)

Prove proper operation of p/s fuel oil shutoffs prior to operation.
