Whenever a defect is discovered of which affects the safety of the ship, the master or owner of the ship shall report at the earliest opportunity to the Administration, nominated surveyor, or recognized organization responsible for issuing the relevant certificate. Main engine charge air cooler was found with two 3"x3" inserts welded over corroded steel. Vessel unable to provide evidence of report as required. After any survey of the ship has been completed, no change shall be made in the machinery without the sanction of the administration. Resolution: Vessel's Chief Engineer was able to replace the broken spring starting mechanism with a spare. The Chief Engineer demonstrated the proper operation after the repairs were made.

All lifesaving appliances shall be ready for immediate use. The liferafts were incorrectly installed in regards to painter line and hydrostatic release.

Resolution: Vessel's deck officers rearranged the painter lines and hydrostatic release units so the liferafts would operate properly in the event of an emergency.
