At least one ring life buoy must be fitted with a lifeline. If more than one ring life buoy is carried, at least one must not have a lifeline attached. Each lifeline on a ring life buoy must: (1) Be buoyant; (2) Be of at least 18.3 meters (60 feet) in length; (3) Be non-kinking; (4) Have a diameter of at least 7.9 millimeters (5/16 inch); (5) Have a breaking strength of at least 5 kilonewtons (1,124 pounds); and (6) Be of a dark color if synthetic, or of a type certified to be resistant to deterioration from ultraviolet light. Life ring's line was worn and brittle. 46CFR180.70(c)

The hailing port of the vessel must be marked on some clearly visible exterior part of the stern of the vessel. Vessel had the wrong hailing port. 46CFR67.123

The Coast Guard was made aware that the vessel hit the bottom of the waterway while overtaking another vessel in the Saint Lucie Canal causing potential damage below the waterline. Prior to movement, a damage survey must be completed by an attending CG Marine Inspector. Additionally, all repairs, if applicable, must be completed to the satisfaction of an attending CG Marine Inspector.

Tubular gauge glasses, if fitted to diesel fuel tanks, must be of heat resistant materials, adequately protected from mechanical damage, and provided at the thank connections with devices that will automatically close in the event of rupture of the gauge or gauge lines. Vessel's main diesel fuel tank is equipped with a tubular sight gauge that is constructed of plastic vinyl tubing with manual ball valves at the tank connections. Remove current sight glass arrangement and cap valves or install proper tubular gauge glass arrangement to the satisfaction of the attending Marine Inspector prior to vessel movement. 46 CFR 182.440(a)(7)

Each ring life buoy with a floating waterlight must have a lanyard of at least 3 feet in length, but not more than 6 feet, securing the waterlight around the body of the ring life buoy. Vessel's ring life buoy equipped with a floating light located on the second deck has a lanyard of less than 3 feet in length. 46 CFR 180.70(d)(2)

07109 - Fixed CO2 System The following spaces must be equipped with a fixed gas fire extinguishing system...a space containing propulsion machinery. During semi-annual maintenance and servicing, the third party inspection agency reported that the automatic discharge functions of the fixed carbon dioxide fire extinguishing system serving the engine compartment were inoperable. In the interim, vessel may operate in accordance with owner's letter dated 25JAN23, requiring 01 additional crewmember in excess of what is required on the COI, who will be stationed in the engine compartment and follow the provided "STARSHIP III Engine Room Watch Duties" procedures. Operation is also contingent upon Coast Guard receipt of confirmation from the servicing company that the automatic discharge components of the CO2 system have been isolated and that the remote pull station for the system located at the entrance is operational. Vessel rep must either repair the system to a fully operational status with an accompanying third party servicing tech report attesting as such, OR submit a proposal for replacement of the system for review/approval by the deficiency due date of 12MAY23. 46 CFR 181.400(a)(1) 50c

02103 - Stability When necessary for safe operation, the cognizant OCMI may place specific stability restrictions in a stability letter or on the Certificate of Inspection of a vessel... Vessel installed new potable water tank in last drydock without approved drawings or calculations by MSC. Vessel is reduced to 146 passengers until MSC has reviewed and approved calculations for stability changes. 46 CFR 178.210(c) 50c

Due to the removal of four plastic potable water tanks and installation of a single aluminum potable water tank, IAW the vessel's Stability Ltr H1-1502747 you must provide verification to the Marine Safety Center (MSC)that the vessel's lightship parameters have not changed, as presently outfitted.

The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located. MI observed remote fuel shutoff for the main fuel tank did not fully close the center valve at the tank. Rectify within 3 days. 46 CFR 182.455(b)(4)

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. MI observed flexible hose installed on the supply side of the bow thruster engine raw water cooling system was bulging in multiple locations along its length. Replace within 7 days. 46 CFR 182.100

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. MI observed pipe section just prior to the strainer for the generator cooling piping was showing signs of significant corrosion. 46 CFR 182.100

Cooking and heating equipment must be suitable for marine use. MI observed vessel utilized portable open flame heaters for keeping food warm on the serving line. 46 CFR 184.200

Each ring life buoy must be marked in clearly legible block capital letters with the vessel's name. MI observed ring life buoy on the second deck was missing lettering on the required marking. Rectify within 7 days. 46 CFR 185.604(b)

Stop valves and check valves are installed in the piping to isolate the systems during simultaneous operation and prevent possible flooding through the bilge system. MI conducted operational test of bilge system with both the engine room and steering compartment not drawing a suction when bilge pump was energized. Identified both check valves in both compartments stuck closed not allowing suction. Crew got both check valves loose, then showed proper operation. 46 CFR 182.520(i)(3)

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. MI observed missing quantity of aspirin. Crew retrieved additional aspirin to meet compliance. 46 CFR 184.710

Exhaust gas must not leak from the piping or any connections. MI noted a salt buildup on stbd engine exhaust system. 46 CFR 182.430(b)

Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. MI noted child and adult size life jackets stored in the same area without partition or separation. 46 CFR 180.78(a)(5)

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date charts of large enough scale to make safe navigation possible, tide tables, and current tables. MI noted vessel's charts were not updated and vessel did not have tide and current tables. 46 CFR 184.420(a)

At least one ring life buoy must be fitted with a lifeline. If more than one ring life buoy is carried, at least one must not have a lifeline attached. MI noted that all 3 life ring buoys had lifelines attached. 46 CFR 180.70(c)

Each vessel that operates on the high seas, or that operates beyond three miles from the coastline of the Great Lakes, must have on board a FCC Type Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. MI noted EPIRB was restricted from being float free by overhead on the 3rd deck aft of the operating station. 46 CFR 180.64

A vessel must be provided with a satisfactory arrangement for draining any watertight compartment...under all practicable conditions. Overboard dewatering pipe discovered on the forward port side of the engine room found corroded. 46 CFR 182.500 (a)

Cable and wire for power and lighting circuits must: be listed by Underwriters Laboratories (UL), as UL boat or UL Marine cable. Cable used for welding found leading from the battery space to the DC panel. 46 CFR 183.340(d)(2)

Electrical equipment on a vessel must be installed and maintained to minimize accidental personnel contact with energized parts. Loose uncapped wiring found behind the operating station and on the starboard side of the fly bridge. 46 CFR 183.200(c)

A portable or temporary electric cord or cable must be constructed and used in compliance with the requirements of 111.60-13 in subchapter J of this chapter for a flexible electric cord or cable. Multiple power strips leading to permanent fixtures discovered in battery space. 46 CFR 183.340(r)

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date charts of large enough scale to make safe navigation possible. Nautical charts found out-of-date since 2011. 46 CFR 184.420(a)(1)

Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. A/C discharge hose on the starboard side of the sewage tank compartment found not capped or permanently enclosed. 46 CFR 182.100

To maintain a valid Certificate of Inspection, you must complete your annual inspection within the periods specified in §176.500(b)(1) and your Certificate of Inspection must be endorsed. Vessel exceeded 3 month time frame to complete annual inspection.

Whenever damage or deterioration to hull plating or structural members that may affect the seaworthiness of a vessel is discovered or suspected, the cognizant OCMI may conduct an internal structural examination in any affected space including fuel tanks, and may require the vessel to be drydocked or taken out of service to assess the extent of the damage, and to effect permanent repairs. The OCMI may also decrease the drydock examination intervals to monitor the vessel's structural condition. Vessel ran aground, suspected to have sustained hull and/or internal structural damage. IAW: 46 CFR 176.600 (d)

A device must be provided to automatically shut down power ventilation servicing the protected space and engines that draw intake air from the protected space prior to release of the extinguishing agent into the space. Starboard engine room ventilation shut down flap is inoperable. Repair or replace. - 46CFR181.410(b)(10)

Batteries must be mounted in trays. Battery mount for #2 generator is loose. Repair. - 46CFR183.350(e)

Vsl is broadcasting: Greater/smaller than expected vessel length, no dimension information (AIS Antenna Position ABCD values equals 0), and not broadcasting an AIS Ship Type.

Renumber life jacket containers for appropriate numbers iaw 46CFR185.604(f).

Install GPS on VHF-FM radio iaw 46CFR184.502.

Provide a bilge suction strainer no less than three times the area of the bilge pipe in the tank room IAW 46 CFR 182.510(b).

The port main engine was not in compliance with 46 CFR 182.420 (a). The cooling pump was inoperable.
