Sufficient intact stability shall be provided in all service conditions so as to enable the ship to withstand the final stage of flooding of any one main compartment which is required to be within the floodable length. Vessel could not demonstrate cross-flooding devices were operating as intended. 74 SOLAS (04) II-1/8.1.1

Furniture in stairway enclosures shall be limited to seating. It shall be fixed, limited to six seats on each deck in each stairway enclosure, be of restricted fire risk, and shall not restrict the passenger escape route. Furniture shall not be permitted in passenger and crew corridors forming escape routes in cabin areas. In addition, lockers of non-combustible material, providing storage for safety equipment required by regulations, may be permitted. Portable, non-fixed hand-sanitizing station identified in passenger stairways. 74 SOLAS 01 II-2/41-2.4.10

Fire hoses shall be of non-perishable material approved by the Administration and shall be sufficient in length to project a jet of water to any of the spaces in which they may be required to be used. Each hose shall be provided with a nozzle and the necessary couplings. In interior locations in passenger ships carrying more than 36 passengers, fire hoses shall be connected to the hydrants at all times. Pin hole leak identified in charged fire hose during Fire Drill (deck 6). 74 SOLAS 97 II-2/4.7.1

General arrangement plans shall be permanently exhibited for the guidance of the ship’s officers, showing clearly for each deck, the control stations, the various fire sections, enclosed by “A” class divisions, the sections enclosed by “B” class divisions together with particulars of the fire detection and fire alarm systems, the sprinkler installation, the fire extinguishing appliances, means of access to different compartments, decks, etc., and the ventilating system, including particulars of the fan control positions, the position of dampers and identification numbers of the ventilating fan serving each section. The secondary means of escape from the Cagny restaurant is not clearly identified on the means of escape plan. 74 SOLAS 20 II-2/15.2.4.1

Every crew member with assigned emergency duties shall be familiar with these duties before the voyage begins. Stairway guide on embarkation deck, (deck 6) was unfamiliar with their duties regarding muster station locations. 74 SOLAS 20 III/19.2.1

Electrical installations shall be such that electrical services for safety will be ensured under various emergency conditions. Emergency light in AC station (deck 4, aft through FSD 40590A) was missing cover and lightbulb. 74 SOLAS 04 II-1/40.1.2

Above the bulkhead deck there shall be at least two means of escape from each main vertical zone or similarly restricted space or group of spaces at least one of which shall give access to a stairway forming a vertical escape. Cagny restaurant (CAT 8 public space) secondary means of escape (aft) hidden behind a large curtain, means of escape is fitted with a thumb screw in the direction of escape. 74 SOLAS 97 II-2/28.1.2

Passenger ships engaged on international voyages which are not short international voyages shall carry lifeboats complying with the requirements of regulation 42, 43, 0r 44 on each side of such aggregate capacity as will accomadate not less than 50% of the total number of person onboard. #1 starboard lifeboat experienced propulsion casualty during abandonship drill due to sea painter line wrapping around the lifeboat prop. Due to inoperable lifeboat, a reduction in total number of persons to 3000 in accoradance with short term PSSC issued by DNV. 74 SOLAS (97) III/20.1, 50ac

The following fire protection systems shall be kept in good order so as to ensure their required performance if a fire occurs: structural fire protection, including fire-resistant divisions, and protection of openings in these divisions. The following fire screen doors required adjustment: FSD 4046A, 58507A, and 80270A. The following sliding door required adjustment: 70200C. 74 SOLAS 20 II-2/14.2.1.1

A fixed fire detection and fire alarm system of an approved type shall be installed, complying with the requirements of regulation 13 and so installed and arranged as to provide smoke detection. Spaces having little or no fire risk such as voids, public toilets and similar spaces need not be fitted with fixed fire detection. PSCOs observed port and starboard Dive Equipment Lockers to be modified and used as Category 13 spaces per updated Fire Control Plan; lockers now store towels and life jackets presenting a fire risk. Crew removed contents and were instructed that the spaces remain empty until proper fire detection is installed. 74 SOLAS 97 II-2/36.2

Oil fuel lines shall be screened or otherwise suitably protected to avoid as far as practicable, oil spray or oil leakages onto hot surfaces, into machinery air intakes and other sources of ignition. Several fuel pipe flanges have been found without a spray-shield tape or fire protection lagging in both fuel purifying units.

All escape route signs and fire equipment location markings shall be marked by lighting or photo luminescent material. PSCOs observed intermittently functioning emergency escape sign near Cabin 60004. 74 SOLAS 97 II-2/28.1.10

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons onboard. Hose in pool machinery space had leaked into the space and created a potentially hazardous atmosphere.

07105 - Fire doors/openings in fire-resisting divisions - Holdback hooks not subject to Central Control station are prohibited. 20+ fire doors did not register as closed at Central Control Station when centrally released. 74 SOLAS (97 CONS) II-2 / 30.4.3

The vessel reported failure of the transitional uninterrupted power supply, which has now been bypassed to facilitate inspection and troubleshooting.

0799 - (other) fire fighting equipment - "A" class divisions shall be so constructed as to be capable of preventing the passage of smoke and flame to the end of the one-hour standard fire test. Fire doors 70384B and 70367A would not prevent the passage of smoke and flame to the end of a one-hour fire test. - 74SOLAS(97 cons)II-2/3.3

74SOLAS (2001) II-2/26.1 - The minimum fire integrity of all bulkhead and decks shall be as prescribed in table 9.1 and 9.2. Identified the crew gym had numerous drilled penetrations through the deck.

74SOLAS IV/16.2 - In passenger ships at least one person qualified in accordance with paragraph 1 shall be assigned to perform only radio communication duties during distress incidents. Ships designated GMDSS operator performed other duties during distress incident.

Crew did not follow established checklist resulting in flooding of comparment #12, while cleaninig the strainer to the seachest.

Generator #4 is inoperable provide class report attesting to proper operation.

74 SOLAS ('14 cons) Ch.III/19.2.2.1 - Every crewmember with assigned emergency duties shall be famliar with these duties before the voyage begins. Personnel in charge of bakery was not familiar with fire fighting procedures or release of fixed CO2 suppression. Additionally vice commanders of tender boats were unfamiliar with duties.

74 SOLAS ('97 cons) Ch. II-2/26.4 - In approving structural fire protection details, the Administration shall have regard to the risk of heat transmission at intersetions and terminal points of required barriers. Replace missing insulation in tailor shop and deck 4 chemical storage locker.

74 SOLAS ('97 cons) Ch II-2/30.1 - All openings shall be provided with permanently attached means of closing which shall be effective for resisting fires. Several fire doors found inoperable and unable to maintain class "A" boundary protection.

Within the perimeter of such stairways enclosures only lockers of non-combustable material are permitted. Vessel has created unapproved storage space housing combustable material in crew stairway S-C12 on deck 6A.

0799 Fire doors in MVZ bulkheads and stairway enclosures shall be self-closing and have a rate of closure between 10-40 seconds. FSD 10570B did not close when activated from local release. 74 SOLAS (97) CH II-2/30.4.1

Passenger ships shall have a daytime signalling lamp to communicate by light during day and night using an energy source not solely dependent upon the ship's power supply. The power source for daytime signalling lamp did not power lamp as designed. 74 SOLAS (09) V/19.2.2.2

ICLL 66 Annex1, Reg 16(4) The arrangements shall ensure that the tighness can be maintained in any sea condition. Door on deck 5 stbd aft does not maintain weathertight integrity.

SOLAS09ConsEd CHII-2/Reg36.2 & 12.1.1 Passenger ships carrying more than 36 passengers shall be equipped with an automatic sprinkler capable of immediate operation. Deck 4, FZ 5 showing heavy salt buld-up making immediate operation not possible.

SOLAS09ConsEd CHII-2/Reg10.2.3.1.1 In passenger ships carrying more than 36 passengers, fire hoses shall be connected to the hydrant at all times. fire hose in engine room, aft sewage treatment room, was found not connected to the hydrant.

33CFR 164.80(a)(1) & .25(a)(3)/CG Moc Pol Lrt 2-05 The emergency generator automatic start capability and testing under load shall be completed on a weekly basis. Found that tests under load were only conducted on a monthly basis.

0710. "A' Class Fire doors shall be of slef-closing type and capable of closing with an angle of inclination of up to 3.5 degress opposing closure. During fire drill, 22 fire screen doors did not register as closed on panel. In addition, the following doors were verified as not closing 55093A, 90270A, 00516A, 30367, 30466C, 48404F, 48502B. SOLAS 74 CH II-2/Reg 30.4.1

0950. The Condition of the ship and its equipment shall be maintained after any survey of the ship no change shall be made in the equipment covered by the survey without the sanction of the administration. The following light fixtures were found broken, Deck 5 outside paint locker, Steering Gear starboard, Locker 80381, deck 8. All three Light Fixtures have been replaced. SOLAS 74 (09) CH.I/11

0960. Storage of combustible materials is prohibited in spaces within a space categorization (3). Found storage of wooden lockers, dirty clothes, metal lockers, washer and dryer in a corridor outside of paint locker in deck 5. SOLAS 74 (2001) CH. II-2/ Reg 26.2(3)

0750. Storage of combustible materials is prohibited in spaces with a space categorization (10). Cardboard boxes were found in the AC room (deck 10) and the electronic locker #10295A. SOLAS 74 (2001) CH. II-2/ Reg 26.2

0960. Room in room construction is an arrangement where occupants of a space are not provided direct access to a means of escape. A corridor or lobby from which there is only one route of escape shall be prohibited. There was no direct exit from the sauna on deck II( no direct access to means of escape). SOLAS 74 (2001) Ch. II-2/ Reg 28.1.4

0715. A fixed fire detection and fire alarm system shall be so installed and arranged as to provide smoke detection in service spaces, control stations and accomodation spaces, including corridors stairways and escape routes within accomodation spaces. smoke detector on deck 6, starboard side in front of passenger cabin #9225 is inoperable and does not work. SOLAS 74 (2001) Ch.II-2/Reg 7.5.2

0725. The sprinklers shall be resistant to corrosion by marine atmosphere. Multiple Sprinkler heads throughout the vessel are corroded and full of lint. SOLAS 74 (2001) Ch.II-2/ Reg 12.2.4

0930. An efficient bilge pumping arrangement shall be provided, capable of pumping from and draining any watertight compartment other than a space permanently appropriated for the carriage of freshwater, ballast water, oil fuel or liquid cargo and for which other efficient means of pumping are provided under all practical conditions. Space "WTC 7" does not have an installed bilge pumping system.

2550. The company should establish procedures to ensure the ship is maintained with the requirements established by the company. Section G402.03 of the ship's SMS states "keep as much water out of the bilges as possible." However, examiners found 2-3 feet of standing water in "WTC 6" and Ch. Eng could not provide records demonstrating the last time they were pumped. SOLAS (2009) XI-1/1 ISM 10.1

0960. The following basic principle underlies the regulations: protection of means of escape. In the exercise room, Deck II forward, A plant was blocking one of the means of egress. SOLAS 74(97 Cons) Ch II-2/Reg 2.2

0599. For determining the appropriate fire integrity standards to be applied to boundaries, such spaces are classified according to their fire risk in table 9.1. The BOS'N store room/workshop is a normally unoccupied space, however the crew has placed a computer, refrigerator, and several couches in the space, violating the relevant space categorization rules. SOLAS (97 Cons) Ch.II-2/ Reg 26

2510- Safety and Environmental Policy- The Safety management system shall be maintained in accordance with the provisions of the ISM code. Passenger cabin evacuation during the ships drills were found to be inconsistent with ships procedures. Evacuation cards were found on all doors however some doors were found completely open and held back by wedges, some doors were found shut... see narrative.

0710- Fire Prevention- The contents of a space must be appropriate for the space category as per the vessels SFP Plan. Found Flammable liquids inappropriately located in Spa chemical locker deck 11 Starboard 11282B which is not a category 14 space. Spa manager indicated liquids present were more than "Daily Use" SOLAS 09 CH 11-2 Reg 9 2.2.5.2.2

0700 - Fire fighting appliances: Fire doors in MVZ, bulkheads, galley boundaries and stairway enclosures shall be self-closing and capable of closing with an angle of inclination of up to 3.5 degrees opposing closure. Fire doors 50166B and 13491A did not completely shut. Solas 2004 Cons Ed, Chap II-2, Reg 9.4.1.1.4.

0750-Fire Prevention- Storage of combustibles materials is prohibited in spaces with a space categorization (10). Large amount of card board boxes containing Insulation were found AC Space 10491B. SOLAS 74 (2009) II-2/9.2.2.3.2 (10)

Escape routes shall be maintained in safe condition, clear of obstacles. Emergency escape door 7037A in casino is jammed and very hard to open, SOLAS74 2009 Con'ed II-2/13.1.2

0750- Fire prevention- Doors and Door frames in B class divisions and means of securing them shall be provide a method of closure which shall have resistance to fire testy procedures code. B class door 80360A is broken and does not close fully or lock. SOLAS 74 (2009) CH II-2 Reg 13.1.2

0960 -Means of escape-Additional aids for escape shall be provided as necessary to ensure accessibility, clear marking and adequate design for emergency situations. Emergency exit signage is blocked by casino machine can't be seen from center of casino. SOLAS 74 (2009) CH II-2 Reg 13.1.3

During the vessel's fire drill, all fire screen doors were remotely closed from the bridge, however the panel indicated that several of the doors were still open. PSCOs and the Master checked several of the doors that were indicated open and the doors were verified closed. The vessel shall ensre the proper operation of the control panel and is required to provide a vrew mbr to verify FSDs closed

Provide statement of class that all refrigerated spaces above 5 degress celcius are smoke detected vice heat detected IAW fire control plans. LEGACY COMPONENT DESC: Smoke Detectors
