Before the ship leaves port and at all times during the voyage , all life saving appliances shall be in working order and ready for immediate use. The chain used to connect the rescue boat was found with significant wastage and the rescue boat was improperly positioned in its davit causing it to list approximately 10 degrees to the outboard side. Appropriate precautions shall be taken during loading and transport of cargo units and cargo transport units on board ro-ro ships, especially with regard to the securing arrangements on board such ships and on the cargo units and cargo transport units and with regard to the strength of the securing points and lashings. A majority of the "D" rings forward of the vessel's rescue boat on the weather deck where found wasted or excessively corroded.

Per SOLAS 74 (2020 Cons) Ch III Reg 31.2, cargo ships shall carry at least one rescue boat complying with the requirements of section 5.1 of the Code. A lifeboat may be accepted as a rescue boat, provided that it and its launching and recovery arrangements also comply with the requirements for a rescue boat. MSU Saipan received notification from the master of the USNS Charlton stating that the rescue boat was damaged during a monthly lowering/operation drill. The master further clarified that the damage appears to be cosmetic as the FRP is fractured/holed under the sea painter retention hook, located on the bow. The rescue boat is operational and can be raised/lowered as per regulations. The damage does not impair the rescue boat's seaworthiness. The USNS Charlton intends to transit with the damaged rescue boat from Saipan to Korea for final repairs to be completed by an authorized repair company. Provide an ABS Class report to CG MSU Saipan and notify CG Activities Far East when repairs are completed to the satisfaction of the attending surveyor. SOLAS 74 (2020 Cons.) Ch III Reg 20.2

A fireman's outfit shall consist of: [.5] an axe to the satisfaction of the Administration. The firemans' outfits onboard are presently not provided with individual axes/hatches suitable for attaching to the firefighter's belt and used as part of the personal equipment. *It is understood that there are discussions between MSC HQ and CG HQ regarding the number of firemans' outfits that require these axes, given that MSC vessels often have far more than the SOLAS required number. At a minimum, axes to be provided for the SOLAS req'd number.

The Machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to moving parts, hot surfaces and other hazards. The design shall have regard to materials used in conjunction, the purpose for which the equipment is intended, the working conditions to which it will be subjected and the environmental conditions on board. Vessel crew reported that during a routine maintenance inspection of the stbd GT, erosion and blade damage were discovered on the stage 1 & 2 blade tips of the GT. Vessel is to limit the use of the stbd GT for emergency usage only until permanent repairs have been affected to the satisfaction of the attending ACP class surveyor or the CG Marine Inspector.

74 SOLAS (14) I/II Stair tower door 7-84.52 is out of adjustment and does not close properly. Door is hard to open when closed. Adjust door to the satisfaction of RO.

S/W leaked on fire main pipe forward of metallic expansion joint in B deck, frame 45 overhead. VSL alternate plan is to utilize forward sprinkle sea water pump to provide fire main pressure to fire stations forward of damage pipe until permanent repairs are complete.

Maintenance, testing and inspections shall be carried out based on the guidelines developed by the organization and in a manner having due regard to ensuring the reliability of fire sighting systems and appliances. Annual servicing has not been conducted on the rolling galley fire door since 2017. 74SOLAS(14) II-2/14.2.2.1 50A

Make permanent repairs to the fire and gas detection system in cargo holds and engine spaces. Repairs and operational testing to be completed to the satisfaction of OCMI.

Make repairs to the pneumatic valve for the engine room Low pressure CO2 system. Repairs and operational testing to be completed to the satisfaction of OCMI. Vessel authorized to proceed to anchorage with additional measures in place per the risk assessment plan. Vessel will not depart the port for voyage until permanent repairs are made.

Replace expired self activating smoke signals on both bridge wings

Re-service 30 second time delays on the fixed CO2 system that are not within manufacture specifications.

Provide manufacture and service company specifications attesting to the acceptable activation range of fixed system time delays. Information should include acceptable percent over and under setting on time delay.

Prove proper operation of Over speed trips for Port and STBD Main Engines.

Shop test #1 and #2 Start air flask relief valves and prove proper operation.

Replace supply air duct for air handling unit located Forward, Frame 73 STBD side.

Prove proper operation of Cargo hold OWS.

Prove proper operation of Boilers and all related Safeties Prior to departure.

Ensure all inflatable liferafts are certified and onboard ship prior to departure.

Ensure all pyrotechnics and line throwing appliances are valid and onboard prior to departure.

Ensure Lifeboat davits and Falls are in compliance prior to departure.

Ensure all provisions for Lifeboats are onboard and Valid prior to departure.

Ensure Fire and Lifeboat drills are conducted prior to departure.

Ensure Hospital is fully supplied with the proper Med’s and other first aid items.

Prove proper operation of starboard reduction gear. Compliance may be achieved by providing documentation of successful sea trial test results including: a) Gear tooth contact sketches (at 100%); b) Bearing temp readings for reduction gear and line shafts; and c) Lube oil clear and bright tests

Reinstall gripes on all lifeboats to the satisfaction of attending marine inspector. Until complete, the use of web-type cargo straps is authorized in lieu of gripes to secure lifeboats.
