Key personnel must complete training in 33CFR101.650(d)(2) by January 12, 2026. Company does not have a CYSO and all key personnel have not yet completed the training as of this inspection. 33 CFR 101.650(d)(4) 705: 60 days

For fixed-gas fire extinguishing systems, the inspections and tests required by Table 176.810(b) of this section, in addition to the tests required by 46 CFR 147.60 and 147.65. The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. If any of the equipment or records have not been properly maintained, a qualified servicing facility may be required to perform the required inspections, maintenance procedures, and hydrostatic pressure tests. Current or satisfactory documentation of maintenance for the installed CO2 system was not made available. Stickers were observed on the CO2 bottle but report of complete system checks were not made available. Provide satisfactory documentation from third-party. 46 CFR 176.810(b)(2)

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Rudders were swung and operated as designed. However, Hydraulic hose fittings and steering ram were noted with excessive corrosion. Replace corroded hydraulic hose fittings as showed and and explained to the master. Refurbish or replace steering ram.

The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. Installed CO2 system did not have evidence of current required annual servicing. CO2 bottle displayed corrosion at bottom of cylinder and bracket due to dissimilar metals. Replace Co2 bottle. Provide current installed system service/inspection records. 46 CFR 176.810(b)(2)

instruct each crew member, upon first being employed and prior to getting underway for the first time on a particular vessel and at least once every three months, as to the duties that the crew member is expected to perform in an emergency. Crew training shall be logged or otherwise documented for review by the Coast Guard upon request. Crew drill training log was not made available. Provide drill training log. 46 CFR 185.420(a)(d)

Each EPIRB, other than an EPIRB in an inflatable liferaft, must be tested monthly,The EPIRB test required by paragraph (a) shall be logged or otherwise documented. and NOAA registration was expired. Make monthly checks available and update NOAA registration.(combined def) 46 CFR 185.728(a),(c)

Machinery and fuel tank spaces must be separated from accommodation spaces by boundaries that prevent the passage of vapors. Main passenger cabin was found to have the ER hatch and p/s coolant resevior accesses non vapor tight. Make vapor tight. 46 CFR 177.405(c)

A number of child size life jackets equal to at least 10% of the number of persons permitted on board must be provided. 04 Child life jackets require maintenance (missing light, clips, and stenciling). Conduct maintenance as required. 46 CFR 180.71(b)

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire. Engine driven fire pump was found with excessive leak. Make necessary repair. 46 CFR 182.100

Provision must be made for closing all supply duct cowls or scoops and exhaust duct discharge openings for a space protected by a fixed gas extinguishing system. Closures did not secure properly in the closed position. Make necessary repair. 46 CFR 182.465(h)

A vessel must be operated in accordance with applicable laws and regulations and in such a manner as to afford adequate precaution against hazards that might endanger the vessel and the persons being transported. Two windows behind operating station were found cracked and reduced visibility. Replace windows. 46 CFR 185.100

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces. Lazarette high water level alarm inoperable at the operating station. 46 CFR 182.530

The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. A tag issued by a qualified servicing organization, and attached to each extinguisher, may be accepted as evidence that the necessary maintenance procedures have been conducted. The fuel shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located, preferably from an accessible position on the weather deck. If the handle to the shutoff valve at the tank is located inside the machinery space, it must be located so that the operator does not have to reach more than 300 millimeters (12 inches) into the machinery space and the valve handle must be shielded from flames by the same material the hull is constructed of, or some noncombustible material. Provide a shield inside the space or move the valve outside the engineering space at the next drydock.

High level alarms were inoperable during testing in the lazarette and engine room spaces.

Observed port main engine exhaust hole and lagging is scorched and corroded.

Both (02) inflatable buoyant apparatuses (IBAs) and hydrostatic release units are expired.

Plywood deck over engine compartment is not vaport tight causing potential for leakage/spread of CO2, flame, and smoke. Make repairs to two cut-out areas above port & starboard engines and deck hatches prior to carrying passengers.

Engine compartment has evidence of signifigant oil leak/mist. Clean engine compartment to discover and make repairs to leaking component/system.

Repair leak in piping penetrating collision bulkhead.

Repair hydraulic steering fluid leak under console.

Adjust watertight door dogs to demonstrate watertight integrity.

Show proper operation of starboard MDE RPM gauge.

Reconnect sewage discharge line.

Demonstrate proper operation of audible bilge alarms for engine room and forward compartments.

EPIRB tests not logged for September or October.

Hole found in bottom plate in way of forward engine room bulkhead at keel.

Wastage found in bottom plate in way of aft bulkhead of engine room and forward bulkhead of lazarette at keel.

Cracks found at transom in way of flange plate both port and starboard side.

Pipe penetrating transom not completely welded.

Verticle stiffner in lazarette found to be cracked a way from bulkhead.

Battery cables penetrating lazarette forwad bulkhead not made with proper stuffing tubes.

Forepead bilge piping valve linkage broken.

Provide servicing report showing servicing and proper operation of fixed gas fire extinguishing system prior to carriage of passengers.

Repair leak in forepeak bilge piping in way of bulkhead penetration.

Repair bridge window.

Repair fwd rail damage and bridge windows. Contact MSD STT to conduct COI exam.
