This part contains requirements for the design, construction, installation, and operation of propulsion and auxiliary machinery, piping and pressure systems, steering apparatus, and associated safety systems. Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Vessel’s generator had a fuel leak on the fuel line four feet from the generator and at the connection of the mechanical fuel pump mounted on the engine block. Repair leaks and satisfactory test run the generator in the presence of the attending marine inspector. 46 CFR 182.100

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable to determine that the vessel and its equipment are suitable for the service in which they are to be employed. Pre-engineered fixed gas fire system not tested due to generator fuel leak. Satisfactory test pre-engineered fixed gas fire extinguishing system in the presence of the attending marine inspector. 46 CFR 176.840

At each initial and subsequent inspection for certification, the marine inspector may require that an abandon ship or man overboard drill be held under simulated emergency conditions specified by the inspector. Man overboard drill not conducted due to not testing the pre-engineered fixed gas fire extinguishing system. Upon successful testing of the pre-engineered fixed gas fire extinguishing system, conduct man overboard drill in the presence of the attending marine inspector. 46 CFR 176.808(g)

At each initial and subsequent inspection for certification, the marine inspector may require that a fire drill be held under simulated emergency conditions to be specified by the inspector. Fire drill not conducted due to not testing the pre-engineered fixed gas fire extinguishing system. Upon successful testing of the pre-engineered fixed gas fire extinguishing system, conduct fire drill in the presence of the attending marine inspector. 46 CFR 176.810(d)

S and L. Each major generator and motor shall be fitted with a nameplate of corrosion-resistant material marked with the following information as applicable: (1) Name of manufacturer. (2) Manufacturer's type and frame designation. (3) Output in KW of watts or horsepower rating. (4) Kind of rating (continuous, intermittent, etc.). (5) Revolutions per minute at rated load.(6) Amperes at rated load. (7) Voltage. (8) Frequency if applicable. (9) Number of phases, if applicable. (10) Type of winding (for direct-current motors). Vessel generator nameplate is missing. Provide generator nameplate with the information described above to the satisfaction of the attending marine inspector. 46 CFR 183.10-5(a) (Old-T)

Fuses must be of the cartridge type only and be listed by Underwriters Laboratories or another independent laboratory recognized by the Commandant. New installation of blade type fuses mounted on lower aft inboard pantry partial wall. Replace with the correct amperage UL cartridge type fuses to the satisfaction of the attending marine fuses. 46 CFR 183.380(l)

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. The vessel’s Radiotelephony Certificate was expired. Renew and have onboard to the satisfaction of the attending marine inspector. 46 CFR 184.502

The discharge ends of vent pipes shall be fitted with removable flame screens or flame arresters. The flame screens shall consist of a single screen of corrosion resistant wire of at least 30x30 mesh. The port side flame screen mesh was damaged and needs to be renewed to the satisfaction of the attending marine inspector. 46 CFR 182.15-35 (d) Old T

A vessel is inspected for compliance with the standards required by this subchapter. Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. The port side main engine fuel return lines and the starboard fuel line coming off the manifold and filter were wasted and need to be renewed in kind to the satisfaction of the attending marine inspector. 46 CFR 176.800 (a)

The number and identification of the items stowed inside, and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets and immersion suits. Vessel carries extended size range lifejackets. Extended range life jackets are not clearly marked. Mark as described below. 1) LIFEJACKETS (or IMO Symbol for Lifejacket), for persons over (blank) inches (or lb.)" or 2)Additional suggested marking at the stowage location is a line inches (same number of inches as above paragraph) above the deck with the text: "Children shorter than this line must use Child-size Life Preservers", to the satisfaction of the attending marine inspector. 46 CFR 185.604(f)

No major repairs or alterations affecting the safety of the vessel with regard to the hull, machinery, or equipment, shall be made without the knowledge and approval of the OCMI, Marine Inspection. Stbd main engine had a crack in the salt water cooling piping elbow. Replace the elbow in kind and prove proper operation in the presence of attending marine inspector. 46 CFR 176.20-1

The following spaces must be equipped with a fixed gas fire extinguishing system, in compliance with § 181.410, or other fixed fire extinguishing system specifically approved by the Commandant. A space containing propulsion machinery. The engine starts while fire-boy is disconnect, solenoid timing is out. Repair and demonstrate proper function to the satisfaction of attending Coast Guard Marine Inspector. 46 CFR 181.400(a)(1)

No major repairs or alteration affecting the safety of the vessel with regard to the hull, machinery or equipment shall be made without the knowledge and approval of the Officer in charge, Marine Inspections. Seats and tables on upper deck must be secured utilizing through bolts. 46 CFR 116.820(b)

For a buoyant apparatus—Approved under approval series 160.010. The color of the buoyant apparatus must be primarily vivid reddish orange. Each buoyant apparatus must have Type II retroreflective material meeting subpart 164.018 of this chapter on each side and end. 46 CFR 180.200(a)(1) 46 CFR 160.010-4(n), (p)

At each initial and subsequent inspection for certification the marine inspector shall examine and test the following items to the extent necessary to determine that they are in proper operating condition, safe electrical condition, and fit for the service for which they are intended. Lighting installed in engine room inoperable. Provide adequate lighting in engine room. 46 CFR 176.25-15 (a)

The minimum capacity of the power fire pump shall be 50 gallons per minute at a pressure of not less than 60 pounds per square inch at the pump outlet. The pump outlet shall be fitted with a pressure gage. During operational test of fire pump, pump would not make pressure shaft just spun. Make 60 psi in presence of attending Marine Inspector. 46 CFR 181.10-1 (e)

A vessel must be provided with a satisfactory arrangement for draining any watertight compartment, other than small buoyancy compartments, under all practicable conditions. 46 CFR 182.500(a)

An exposed hazard, such as gears or rotating machinery, must be properly protected by a cover, guard, or rail. Alternator on Port & STBD engines missing belt guard. 46 CFR 177.960

Life floats and buoyant apparatus must not be stowed in tiers more than 1,220 millimeters (4 feet) high. When stowed in tiers, the separate units must be kept apart by spacers. Insert spacers in between Life floats. 46 CFR 180.137(f)

Upper deck passenger limit not posted at entranceway to upper deck. Provide marking at both sets of stairs showing 47 passengers authorized on upper deck at entranceway to the upper deck.

Port side main engine turbo outlet has expansion joint attached by band clamps. The connection is slipping. Provide sure connection of expansion joint with two clamps.

Fire main gauge numbers worn. Provide legible gauge to ensure adequate fire main pressure.

Drug and alcohol hot-line numbers not posted in common space. Provide drug and alcohol telephones in common space.

Fixed extinguishing system did not secure generator when tested. Prove correct operation in accordance with 46 CFR 176.810.

Port engine outer turbo oil drain was leaking. Repair leak and clean/renew lagging in accordance with 46 CFR 176.804.

Replace Exhaust lagging on both MDE's

Engine room deck plates need replaced.

Ensure compass light is operational prior to night operations.

Repair helm hand pump seal.
