Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines, one at the tank connection and one at the engine end of the fuel line to stop fuel flow when servicing accessories. The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located, preferably from an accessible position on the weather deck. If the handle to the shutoff valve at the tank is located inside the machinery space, it must be located so that the operator does not have to reach more than 300 millimeters (12 inches) into the machinery space and the valve handle must be shielded from flames by the same material the hull is constructed of, or some noncombustible material. Marine Inspector identified temporary repairs on the ball valve handles to connect fuel shutoff reach rod. Conduct permanent repairs to the satisfaction of the attending MI. 46 CFR 182.455(b)(4)

At each inspection for certification and at every other vessel inspection the bilges and other spaces may be examined to see that there is no excessive accumulation of oil. Marine Inspector identified excessive accumulation of oil below hydraulic tank in midship storage void. Identify source of leak and conduct repairs to the satisfaction of the attending Marine Inspector (MI). 46 CFR 176.830(b)

All cable and wire must: Be installed with metal supports spaced not more than 610 millimeters (24 inches) apart, and in such a manner as to avoid chafing and other damage. Marine Inspector identified that cables and wires throughout the engine room, were not properly supported by metal supports. Install metal supports to all cables and wires to the satisfaction of the attending Marine Inspector. 46 CFR 183.340(b)(4)

Electrical equipment on a vessel must be installed and maintained to: Prevent electrical ignition of flammable vapors. Marine Inspector identified dead-end wires below house switch cabinet. Conduct repairs to the satisfaction of the attending Marine Inspector. 46 CFR 183.200(d)

At a minimum, each placard must notify the reader of the operating requirements contained in §§ 151.67 through 151.73 as they apply to that ship. The following requirements should also be prominently stated:(1) The discharge of all garbage is prohibited into the navigable waters of the United States and into all other waters except as specifically allowed;(2) The discharge of all forms of plastic into all waters is prohibited; (3) person who violates the above requirements is liable for civil and/or criminal penalties; and(4) Regional, state, and local restrictions on garbage discharges also may apply. Marine Inspector identified that the garbage placard on board contained insufficient information to meet the requirement. Replace garbage placard with one that meets the requirements of 33 CFR 151.59(c). 33 CFR 151.59(c)

The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing the actions to be taken in the event of fire, heavy weather, or man overboard conditions. Vessel was missing required postings at the operating station. Post emergency instructions to the satisfaction of the MI. 46 CFR 185.510(a)

Vessel does not have emergency instructions as required. Vessel will need to create and post emergency instructions to the satisfaction of the Marine Inspector. 46CFR185.510(a)

All escape hatches and other emergency exits used as means of escape must be marked on both sides in clearly legible letters at least 50 millimeters (2 inches) high: “EMERGENCY EXIT, KEEP CLEAR” Vessel's fwd crew accommodation space emergency exit to the bow marking was the same coloration of the hatch, and marked on only one side. Vessel most add the lettering to the accommodation side and make sure both sides are contrasting to the satisfaction of the Marine Inspector. 46CFR185.606

A power-driven vessel underway shall exhibit: (iv) A stern light. Vessel's stern light was inoperable, at the beginning of the inspection. Vessel representative satisfactorily fixed the stern light, cleared on the spot. 33CFR83.23(a)(iv)

Whenever damage or deterioration to hull plating or structural members that may affect the seaworthiness of a vessel is discovered or suspected, the cognizant OCMI may conduct an internal structural examination in any affected space including fuel tanks, and may require the vessel to be drydocked or taken out of service to assess the extent of the damage, and to effect permanent repairs. Vessel sustained damage to hull after allision with 02 docked nearby vessels and dock piling. Vessel requires external and internal structural exam to determine extent of damage and effect permanent repairs prior to movement. 46 CFR 176.600 (d)

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. During transit from slip to fuel dock, vessel experienced loss of ability to control ENG throttles resulting in allision with nearby vessels. Complete cause analysis, repairs, and test of ahead/astern operation to satisfaction of attending Marine Inspector prior to movement. 46 CFR 182.200

The master of a vessel shall have examined and tested the steering gear, signaling whistle, propulsion controls, and communication systems of the vessel prior to getting underway for a voyage, except that such examination and testing need not be conducted more than once in any 24 hour period. Vessel suffered damage from allision with a dock while returning from the fuel dock. Owner stated that propulsion controls were sabotaged. Master of vessel shall perform examinations of all times at all stations IAW this regulation. 46 cfr 185.320

Vapor barriers must be provided where insulation of any type is used in spaces where flammable and combustible liquids or vapors are present, such as machinery spaces and paint lockers. Bulkhead penetration between the lazarette and engine room is not vapor tight. Effect repairs to the satisfaction of the attending marine inspector.

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. Vessel's Safety Radiotelephony Certificate was not onboard the vessel.

A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a drydock examination and an internal structural at least once every two years. By USCG Policy, a drydock examination and internal structural exam is complete when the work list assigned by the Marine Inspector is completed. Vessel has not completed work list item #2 for broken windows in the pilot house and has been operating with passengers for approximately four months past the drydock expiration date. 46 CFR 176.600(c)(1)

Vessels described by 46 CFR 175.110(c) must have an interconnected fire detection system in compliance with § 181.450 installed in all enclosed areas where passengers and crew have routine access, including accommodation spaces and machinery spaces. Vessel does not have installed detectors in required spaces by 27Dec2022. Vessel has been limited to a Limited Coastwise route until detector installation has been verified by a Marine Inspector. 46 CFR 181.405(c)

A vessel on which a pre-engineered fixed gas fire extinguishing system is installed must have the following equipment at the operating station an audible alarm that sounds upon discharge. Pre-engineered system's audible alarm failed to actuate at a sufficient decibel to provide clear signal to operator.

Each life jacket must be approved in accordance with either § 160.002, § 160.005, or § 160.055 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.155 or 160.176. Three Kapok PFD's were deemed unfit for continued use based on requirements listed under Subchapter Q.

A vessel must be provided with a satisfactory arrangement for draining any watertight compartment, other than small buoyancy compartments, under all practicable conditions. Nonmetallic flexible 3" bilge line in engine space and mid compartment are deteriorated and missing a secondary securing clamp (mid compartment).

Nonmetallic hose may be used for wet exhaust systems provided it is especially adapted to resist the action of oil, acid, and heat, has a wall thickness sufficient to prevent collapsing or panting, and is double clamped where practicable. Port main diesel engine exhaust only had one hose clamp. Add second hose clamp. 46CFR182.430(e)

Except as otherwise provided in this section, rails or equivalent protection must be installed near the periphery of all decks of a vessel accessible to passengers or crew. Equivalent protection may include lifelines, wire rope, chains, and bulwarks, which provide strength and support equivalent to fixed rails. Bulwark door only has a bottom hinge holding it in place. Install top hinge as original. 46CFR177.900(a)

Switches other than those mounted on the switchboard shall be of the enclosed type. Outlet found in vicinity of midship void without cover. Replace cover. 46CFR183.05-15(d)

Each window, port hole, and its means of attachment to the hull or deck house, must be capable of withstanding the maximum load from wave and wind conditions expected due to its location on the vessel and the authorized route of the vessel. Starboard pilothouse door window has cracked window. 46CFR177.1020

A vessel exposed to salt water more than three months in any 12 month period must undergo a drydock examination and internal structural exam at least once every two years. Operator did not make vessel available for January 2021 drydock. 46 CFR 176.600(c)(1)

Three lifejackets were found unserviceable and removed from the vessel.

Vsl carrying one ring buoy, floating light must be attached to the lanyard with a corrosion resistant clip. Floating light was tied to ring buoy body, clip added on spot.

Emergency fuel shut off valves must be marked.

Hydrostatic release unit is expired. 46 CFR 180.64

Vessel does not have a lifejacket placard does not accurately reflect each type of PFD that is carried onboard. 46 CFR 185.516

Operator failed to schedule a follow up inspection to rectify outstanding deficiencies.

Bilge pump that operates off of the starboard engine does not have guards/protection surrounding the belts - 46 CFR 182.100

The construction and arrangement of a vessel must allow the safe operation of the vessel, giving consideration to provisions for a seaworthy hull. A hose for the bait tank in the engine room had a small gouge, providing for a small leak of water. - 46 CFR 177.100

Vessel shall complete satisfactory dry dock inspection prior to carrying passengers. 46CFR176.600(b)(1)
