SANCTUARY

A proper certificate issued by the Federal Communications Commission shall be regarded as evidence that the radio installation, if any, is in compliance with the requirements of that agency. Observed that the FCC Communications Safety Radio Telephony Certificate was expired. Replace the expired certificate with a valid certificate. (46 CFR 184.25-1(b)) [OLD T]

Each life float shall be fitted with a life line and shall be equipped with two paddles, a water light and a painter. Observed one of the aft life floats is missing a floating water light and one of the forward floating water light brackets is broken. Affix a floating water light to the life float and repair the broken bracket. (46 CFR 180.15-1(a)) [OLD T]

At least one ring life buoy must be fitted with a lifeline. Each ring life buoy with a floating waterlight must have a lanyard of at least 3 feet in length, but not more than 6 feet, securing the waterlight around the body of the ring life buoy. Each floating waterlight installed after March 11, 1997, on a vessel carrying only one ring buoy, must be attached to the lanyard with a corrosion-resistant clip. Observed ring life buoy waterlight was not secured around body with a clip, and the lifeline was not in serviceable condition. Secure the waterlight around the body with a clip and replace the lifeline with serviceable replacement. (46 CFR 180.70)

All vessels must have navigation lights in compliance with the applicable sections of the International and Inland Navigation Rules. Observed masthead light and stern light both show incorrect arcs. Replace lights to properly display arcs of lumination. (46 CFR 183.420)

Waste receptacles must be constructed of noncombustible materials with no openings in the sides or bottom. Observed waste receptacles made from plastic or similar materials. Replace bins with noncombustible material. (46 CFR 177.405(f))

Each window, port hole, and its means of attachment to the hull or deck house, must be capable of withstanding the maximum load from wave and wind conditions expected due to its location on the vessel and the authorized route of the vessel. Observed that the central rear window of the bridge was cracked. Repair cracked window. (46 CFR 177.1020)

The COI and any SOLAS Certificates must be posted under glass or other suitable transparent material, such that all pages are visible. Current COI was not present onboard. 46 CFR 176.302

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections... of all watertight closures in the hull, decks, and bulkheads. MI observed foscle void space hatchcover leaking from the deck. 46 CFR 176.802 (a)(2)

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull, protection against fire, means of escape in case of a sudden unexpected casualty, guards and rails in hazardous places, ventilation of enclosed spaces, and necessary facilities for passengers and crew. Capped thru hull penetration in mid ship void just aft of forward bulkhead/port of centerline has excess corrosion where the piping penetrates the hull that could subject the vessel to flooding if it further corrodes. Replace thru hull piping/cap to the satisfaction of attending Marine Inspector.

Exhaust gas must not leak from the piping or any connections. Exhaust leak detected on the starboard engine.

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Dead end wires present in forward lower cabin, starboard side near aft bulkhead.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: Charts of large enough scale to make safe navigation possible. Charts onboard were not up to date.

At each inspection for certification, quarters, toilet and washing spaces…and similar spaces maybe be examined to determine that they are in serviceable and in a sanitary condition. Sink line running along the overhead of the midship void was previously terminated but is still allowing small leak (approx.. 1 drop every 3 seconds). Make permanent repairs at next credit drydock.

Water tight coming that comply with the requirements of Table 171.124 (Partially Protected – 6 inches) must be provided for each opening …that...gives access into the hull. Vessel has an pipe fitting on the starboard aft bulkhead of the cabin that allows water to ingress below the main deck and is located below 6 inches above the deck. 46 CFR 171.124(e)(1) “OLD T”. 705 - Prior to credit drydock.

For fixed-gas fire extinguishing systems, the inspections and tests required by Table 176.810(b) of this section, in addition to the tests required by 46 CFR 147.60 and 147.65. The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. Fireboy service records must be provided.

A vessel must comply with the applicable requirements for any radio...including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission. Station license is missing and must be provided.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: (3) Coast Guard Light List; (4) Tide Tables. Publications are out of date and must be replaced with up-to-date volumes either in hard copy or electronic format.

Each ring life buoy on a vessel must: be approved in accordance with § 160.050 in subchapter Q of this chapter. The grab line shall be 3/8-inch diameter polyethylene, polypropylene, or other suitable buoyant type synthetic material having a minimum breaking strength of 1,350 pounds. Grab line is deteriorated and must be replaced to the specified minimum breaking strength.

Each flare carried to meet this section must be stowed in one of the following: a portable watertight container marked as required by § 185.614 of this chapter, carried at the operating station. Portable watertight container of sufficient size must be provided for flares.

Each item of survival craft equipment must be of good quality, and efficient for the purpose it is intended to serve. Light attached to life float is extinguished/does not actuate as designed. 8 passengers are reduced from total persons until light on life float is in working condition.

A vessel exposed to salt water more than three months in any 12 month period must undergo a dry dock and internal structural examination at least once every two years. Vessel has failed to schedule dry dock and internal structural examination within two years of last dry dock as listed on the COI.

A hatch exposed to the weather must be watertight. Lazarette hatch not watertight. Verify hatch is watertight in presence of Marine Inspector on March 9th, 2021.

Each life float must be fitted with two paddles that are lashed to the life float and must be marked with the name of the vessel. Not all life float paddles are connected to each float or labeled with vessel name. 01 paddle not in operational condition. Correct stowage of all paddles and replace 01 broken paddle.

A vessel not required to have a power driven fire pump must have three 2.5 gallon buckets. Vessel has onboard 2 gallon buckets.

When stowed in tiers, life floats must be kept apart by spacers. No spacers between life floats.

Electrical equipment must be installed to protect passengers and crew from electrical hazards. Exposed dead end wires in accommodation space fire extinguisher cubby. Remove or cap wires.

The general construction of the vessel must be such as to minimize fire hazards. Wall between engine room and midships compartment provides a vapor tight boundary for Fire Boy to effectively put out engine room fire. MI witnessed penetration in bulkhead that was not made vapor tight.

Each ring life buoy must be marked with retro-reflective material. No retro-reflective tape on ring buoy.

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. On February 28th, vessel reported a loss of power/propulsion by possible clogged fuel filter or battery/alternator discrepancy. 46 CFR 182.200

The inspection and maintenance of survival craft shall be logged for review by Coast Guard upon request. The vessel did not have an up-to-date life saving inspection log. The vessel updated the life saving inspection log in the presence of the Marine Inspector. 46 CFR 185.702 (d)

Any vessel of at least 5 net tons which engages in coastwise trade must have a Certificate of Documentation bearing a valid endorsement.

The owner, charterer, master or managing operator shall instruct each crew memeber as to the duties the crewmember is expected to perform in an emergency. Vessel could not provide proof of any training or drills conducted.

Vessel requires EAP policy statement and hotline number IAW 46 CFR 16.401.

10-person & 15-person lifefloats require repair or replacement; also, paddles require markings. - 46 CFR 180.200

Lifering worn/wasted on unmarked side and requires replacement IAW 46 CFR 180.70.

Gasket material around engine room hatch covers worn.
