Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. When repairs are made during an emergency, the owner, managing operator, or master shall notify the OCMI as soon as practicable after such repairs or alternations are made. Vessel's lazarette had 8-10 inches of water from leaking rudder post packing. Vessel O/O is aware and daily inspection is required in Helm Connect. Vessel is required to keep performing daily inspections, monitor and pump bilge as needed, and to notify Coast Guard if situation worsens. Rudder packings shall be replaced NLT September FY24 drydock.

A vessel of more than 19.8 meters (65 feet) in length must carry at least one rescue boat unless the cognizant OCMI determines that:(1) The vessel is sufficiently maneuverable, arranged, and equipped to allow the crew to recover a helpless person from the water. Vessel's person recovery davit shackle did not fit pulley rope carabiner. Vessel replaced the shackle to fit the carabiner. Deficiency corrected on site.

A voltmeter and an ammeter, which can be used for measuring voltage and current of a generator that is in operation, must be provided for a generator rated at 50 volts or more. For each alternating current generator, a means for measuring frequency must also be provided. Switchboard voltmeters were not reading generator voltage. Vessel replaced fuse with test SAT.

Portable and semi-portable extinguishers must be inspected and maintained in accordance with NFPA 10 (incorporated by reference, see § 175.600 of this chapter) as amended. The portable CO2 fire extinguisher serving as the fixed extinguisher for the paint locker was found with a deteriorated discharge hose and with an out of date hydrostatic test date for the cylinder. Provide cylinder within hydrostatic test dates with a serviceable discharge hose.

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances, including the following: (1) Inspection of all accessible parts of the exterior and interior of the hull, the watertight bulkheads, and weather decks. A crack was found in the transverse weld between the main deck and the transverse structural support for the bow stanchion. Grind and reweld.
