Multiple versions were found onboard with the stowage of gear not IAW most recent version. New copies of the most recent version are to be provided onboard to be verified by RO - 30 day w/ verification of gear stowage as approved.

No welding, burning, cutting, or riveting operations involving the use of fire, flame, spark, or arc-producing equipment should be carried out in any space other than machinery spaces and workshops where fire-extinguishing arrangements are available, except in an emergency and, if in port, with authorization of the port authority. Crew conducts hot work and other on the open main deck area with class 1 cargo aboard. Bench with oxy-acetelyne torch and bench grinder all identified as in use in this area. 74 SOLAS (cons.)VII/3 IMDG Part 7.8.6.3

During rescue boat drill multiple crew members were noted leaning over and working adjacent to the side of the ship with all chain guards removed with no life jackets or fall protection. Ventilators in position 1 or 2 to spaces below freeboard decks or decks of enclosed superstructures shall have coamings of steel or other equivalent material, substantially constructed and efficiently connected to the deck. Multiple areas of full thickness deterioration of steel found in the following locations on the main deck:kick-pipe in the #2 crane breezeway, #3 DB ballast sounding pipe (fiberglass wrapped patch), and a forepeak supply vent. ILLC Reg.19(1)

Ventilator openings shall be provided with watertight closing appliances of steel or equivalent material. Significant deterioration of weathertight closure device gasket channels found at following locations: hold 4 aft exhaust, hold 3 supply, forward pipe tunnel access door, and hold 1 supply. The vessel had numerous dogs, closure devices, and handles on weathertight ventilation closure covers leading to cargo holds and other spaces, which were either deteriorated, missing, or misaligned. Additionally, approximately 10 of these closures had severely deteriorated knife edges and gasket channels. ILLC Reg. 19(4)

All access openings in bulkheads at ends of enclosed superstructures shall be fitted with doors of steel or other equivalent material, permanently and strongly attached to the bulkhead and framed, stiffened and fitted so that the whole structure is of equivalent strength to the unpierced bulkhead and weathertight when closed. The means for securing these doors weathertight shall consist of gaskets and clamping devices or other equivalent means and shall be permanently attached to the bulkhead or to the doors themselves, and the doors shall be so arranged that they can be operated from both sides of the bulkhead. Vessels 01 superstructure weathertight doors, port and stbd, found with deteriorated lower gasket channels. ILLC Reg. 12(1)

The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company. Due to the number and nature of deficiencies found, objective evidence exists that the vessels safety management system is not being implemented properly. An external ISM audit shall be conducted prior to departure. 74SOLAS(20)Ch.IX/3.1 ISM Part A/10.1

Fire drills should be planned in such a way that due consideration is given to regular practice in the various emergencies that may occur depending on the type of ship and the cargo. Each fire drill shall include; checking the operation of fire dampers. No objective evidence was provided to indicate the crew had ever conducted fire drills which included the removal of the 16 installed dehumidifier lines entering and existing cargo holds 1-4 through the fire dampers. Crew stated that ducting has been in place since 2019 without removal. 74SOLAS(97)Ch.III/18.3.11

Rescue boats shall be stowed in a state of continued readiness for launching in not more than 5 mins. Vessels rescue boat aft harbor pin was painted in place, the painter was disconnected, and the frapping line was jammed into the engine mount. Launching took 20 mins. 74 SOLAS (97 cons.) III/14.1
