46 CFR 182.455(b)(4) Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines. MI observed the port side remote fuel shutoff valve was inoperable.

46 CFR 180.137(b) The float free line on an inflatable buoyant apparatus must be secured to the painter at one end and to the vessel on the other end. MI observed the float free link and hydrostatic release installed improperly such that the apparatus would not float free.

46 CFR 181.420 A pre-engineered fixed gas fire extinguishing system must automatically shut down all power ventilation systems and all engines that draw intake air from within the protected space. MI observed that while the main engines did shut down upon activation of the system, the engines were able to restart without the override switch being activated. Owner/Operator shall provide a report from a qualified technician attesting to the satisfactory operation of the system upon repair.

46 CFR 181.500(b) A portable fire extinguisher located in the vessel’s operating station must be rated 10-B:C. MI observed the extinguisher in the operating station was 1-A and therefore undersized.

46 CFR 176.810(d) At each initial and subsequent inspection for certification, the marine inspector may require that a fire drill be held under simulated emergency conditions to be specified by the inspector. During inspection the required crew were not available to conduct drills.

46 CFR 176.808(g) At each initial and subsequent inspection for certification, the marine inspector may require that an abandon ship or man overboard drill be held under simulated emergency conditions specified by the inspector. During inspection the required crew were not available to conduct drills.

46 CFR 185.420(d) Crew training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill log book was not made available to the MI during the inspection.

46 CFR 180.70(c) At least one life ring buoy must be fitted with a lifeline. The lifeline connected to the ring buoy was frayed, compromising the integrity and strength of the line, and line was attached to ring buoy with a clip that was stuck in the open position.

46 CFR 184.710 A vessel must carry either a first-aid kit or a kit with equivalent contents and instructions. MI observed the first aid kit on board was expired.

46 CFR 183.420 All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules. MI observed the anchor light’s arc of visibility was impeded by a deck light installed on top of the pilothouse. Vessel is restricted to operations during daytime with 3 NM of visibility only.

46 CFR 181.450 The vessel is required to have an interconnected detection and alarm system installed such that the actuation of alarm in one area results in both audible and visual alarms in all areas.

46 CFR 177.35-15 Suitable covers, guards, or rails shall be installed in way of all exposed and hazardous places such as gears, machinery, etc.

46 CFR 111.01-1 Electric installations on vessels must ensure protection of passengers, crew, other persons, and the vessel from electrical hazards. MI observed the following electrical hazards. 1. Dead end wires on the overhead of the pilothouse 2. Generator harness with butt connectors that are exposed and in need of heat shrink. 3. A battery in the machinery space bilge that is not connected to anything.

46 CFR 176.810(b)(1)(ii) The owner, operator, person-in-charge, or a designated member of the crew must conduct monthly inspections of portable fire extinguishers required by NFPA 10. There were no monthly inspections logged by the vessel.

46 CFR 185.510(a) The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing the actions to be taken in the event of fire, heavy weather, or man overboard conditions.

46 CFR 183.430 Each vessel must be equipped with at least two operable portable battery lights. One of these lights must be located at the operating station and the other at the access to the propulsion machinery space.

46 CFR 176.306 The stability letter must be poster under glass or other suitable transparent material, such that all pages are visible, at the operating station of the vessel. There was no stability letter posted on the vessel.

46 CFR 176.302 The Certificate of Inspection must be posted under glass or other suitable transparent material, such that all pages are visible. There was no COI posted on the vessel.

46 CFR 16.401(b) An EAP training program must be conducted for the employer's crewmembers and supervisory personnel. Supervisory personnel must receive at least 60 minutes of training. There was no record of training on the EAP program available for inspection.

46 CFR 180.210 (b) A vessel of not more than 19.8 meters (65 feet) in length is not required to carry a rescue boat unless: The cognizant OCMI determines that the vessel is designed, arranged, or involved in operations so that the vessel itself cannot serve as an adequate rescue craft. Vessel's rescue ladder does not meet the specific guidelines as per Sector St. Petersburg's Work Instruction MPS-WI-SECSP, Rescue Platforms as an Alternative to Rescue Boats on Small Passenger Vessels. Specifically, MI's observed that the rescue ladder was not equipped with an appropriate platform landing of at least 36 X 36 inches. Ammended on 14OCT25 to: The cognizant OCMI has determined that the vessel is designed, arranged, or involved in operations such that it cannot serve as an adequate rescue craft. During a demonstration of helpless person recovery, the Marine Inspector observed that the recovery method was unsatisfactory. The vessel shall propose procedures and demonstrate a safe and effective method for recovering a helpless person from the water. This deficiency was amended on October 14th, 2025 as explained within letter 16710/25-176. Action is required by November 13th, 2025. This deficiency was amended on November 14th, to code 701c

Each vessel operating on a coastwise route in warm water must be provided with life floats to acommodate 100% of toal number of persons onboard. Vessel only has 1 life float accommodating 22 people. Passenger count is limited to 20 until a second life float is onboard. Total persons allowed is now 22. 46 CFR 180.204 705(c)

A visual and audible alarm at the operating station must be in place. The E/R and steering compartment high water alarms are not working. Must be fixed prior to passengers. 46 CFR 182.530 701(c)

The master shall conduct sufficient drills and give sufficient instructions ensure that all crew members are familiar with their duties during emergencies. Upon arriving to new to home port, vessel master and crew shall obtain new to zone inspection and successfully run abandon ship, man overboard, and fire fighting drills in the presence of attending Coast Guard Marine Inspector prior to the carriage of passengers.

A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a drydock examination and an internal structural examination at least once every five years. Vessel is overdue for the required dry dock inspection required by 46 CFR 176.600 and is restricted to carrying passengers until a drydock is completed.

Connections must be made to battery terminals with permanent type connectors. Spring clips or other temporary type clamps are prohibited. Inspector observed battery connections to terminals held in place by alligator clips and no cover intact. Operator to fix.

All observed unsafe practices, fire hazards, and other hazardous situations must be corected. All required guards and protective decies must be in satisfactory condition. Vessel missing machinery guards on main diesel engines and generator.

Electric equipment must be maintained to protect from electrical hazards. Vessel had exposed wires and A/C drain water leaking onto electrical components. Operator to repair.

A vessel of at least 26 feet a visual and audible alarm must be provided at the operating station to indicate a high water level in each unmanned space. Vessels visual bilge alarm and pump indicators weren't operable. Operator to repair.

Marine employers shall establish programs for the chemical testing for dangerous drugs on a random basis of crewmembers on inspected vessels. Vessel could not provide satisfactory proof of random drug test program for all crew members. Operator to enroll crew in program.

Electric equipment must be maintained to protect from electrical hazards. Vessel had exposed wires on bridge. Operator to remove or cover exposed wires.

At every vessel inspection all observed unsafe practices, fire hazards and other hazardous situations must be corrected. Vessel had batteries, empty containers, dead end wires, pipe wrench and lighters adrift. Operator to secure gear in appropriate places.

Provisions must be made for closing all supply ducts, supplies or scoops and exhaust duct discharge openings for a space protected by a fixed gas extinguishing system. Engine room was not made vapor tight. Operator to correct.

All vessels must have navigation lights that are in compliance with the applicable sections of the Int'l and Inland Navigation rules. Anchor light is inoperable.

Propulsion machinery must be suitable in type and design for propulsion requirements of hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Engine mount bolt sheared, to be replaced.

A suitable metal marine type strainer, meeting the requirements of the engine manufacturer, must be fitted in the fuel supply line in the engine compartment. Operator to install fixed mechanical plug.

Vessels described by 46 CFR 175.110(c) must have an interconnected fire detection system in compliance with § 181.450 installed in all enclosed areas where passengers and crew have routine access, including accommodation spaces and machinery spaces. Vessel did not have installed interconnected fire detection system; operator to rectify.

The operator shall have each hydrostatic release unit in compliance with the servicing and usage requirements. Inflatable buoyant apparatus hydrostatic release expired. Operator to replace with new release unit.

Each life float and buoyant apparatus must be stowed as required and secured to the painter at one end and to the vessel on the other end. release mechanism was mounted upside down and note secured to IBA mounting bracket. Operater to mount release mechanism upright and secure to painter and vessel.

A vessel not required to have a power driven fire pump by § 181.300 must have at least three 9.5 liter (2-1/2 gallon) buckets, with an attached lanyard satisfactory to cognizant OCMI, placed so as to be easily available during an emergency. The words "FIRE BUCKET" must be stenciled in a contrasting color on each bucket. Marine Inspector noted only two buckets present, without lanyards, and not labeled. Operator to procure the third bucket, fit with lanyards, and labeled.

The minimum number of portable fire extinguishers required on a vessel must be acceptable to the cognizant OCMI. Vessel is certificated to have three portable fire extinguishers. Marine inspector noted extinguisher in forward salon area was not adequately charged. Operator to replace with charged extinguisher or recharge existing extinguisher.

Each vessel that operates on the high seas, or that operates beyond three miles from the coastline of the Great Lakes, must have on board an FCC Type Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. Marine Inspector noted the vessel's EPIRB registered to the wrong vessel. Operator to register EPIRB to correct named vessel.

The name of the vessel must be marked or painted in clearly legible letters on each Emergency Position Indicating Radio Beacon (EPIRB). Operator to label EPIRB.

Each life jacket, immersion suit, and ring life buoy must be marked in clearly legible block capital letters with the vessel's name. Marine Inspector noted life jackets and ring buoy marked with vessel's old name. Operator to mark the life jackets and ring buoy appropriately.

Placards containing instructions for donning and use of life jackets aboard vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers. Marine inspector noted no instructions on vessel. Operator to mount placard for model of life jacket found on board.

Each vessel must be equippred with at least two operable portable battery lights. One of these lights must be located at the operating station and the other at the access to the propulsion machinery space. Marine inspector noted no portable light at the operating station. Operator to procure and install portable light.

Remote fuel shutoff stations must be marked in clearly legible letters at least 25 millimeters (1 inch) high indicating purpose of the valve and direction of operation. Marine Inspector noted the vessel's fuel shutoff valves not marked with direction of operation. Operator to label shutoffs.

A vessel must be fitted with ground tackle and mooring lines necessary for vessel to be safely anchored or moored. The ground tackle and mooring lines provided must be satisfactory for size of the vessel. Marine inspector noted a cleat for mooring missing on bow of vessel. Operator to affix a cleat.

A vessel must comply with applicable design, equipment, personnel, procedures, and record requirements of 33 CER parts 151. Marine Inspector observed the Waste Management Plan with the name of another vessel. Operator to Ensure Garbage Management plan is updated.

Marine Inspector observed a drain in the engine room dripping water onto the mounting plate for the starboard engine. Operator to route drain to bilge away from machinery.

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to inspect of all machinery essential to the routine operation of the vessel. Marine Inspector noted missing starboard engine turbo filter cover. Operator to replace cover.

Deck rails must be designed and constructed to withstand a point load of 200 pounds applied at any point in any direction, and a uniform load of 50 pounds per foot applied to the top rail in any direction. Marine Inspector observed a broken railing on the bow. Operator to secure railing to the deck; no passengers allowed on the bow until rectified.

Every vessel subject to Part III of Title III of the Communications Act must have a detailed inspection of the radio installation by an FCC—licensed technician once every five years and be issued a Communications Act Safety Radiotelephony Certificate. Marine inspector observed no radiotelephony certificate on board. Operator needs to receive required FCC inspection and obtain the certificate prior to operating beyond 3NM.

Portable fire extinguishers shall be installed and located to the satisfaction of the [name withheld] bracket in Pilot House. Install fire extinguisher in approved bracket in Pilot House. 46 CFR 181.30-10(b)

FRP vessel op'ing on limited coastwise route in cold water must have life float for 100% of persons onboard. Inflatable Buoyant Apparatus (IBA) absent. Replace and properly stow the IBA currently out for servicing. 46 CFR 180.205(b)

Propulsion machinery shall be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. The vessel owner or operator must pay the annual vessel inspection fee each year on or before the vessel's user fee anniversary date, unless the fee has been prepaid under 46 CFR 2.10-105. Vessel has not paid user fee's. Vessel rep will provide proof of payment. 46 CFR 2.10-101(b)

The fuel shutoff valve shall be operable from outside the tank compartment, preferably from an accessible position on the weather deck. Vessel is missing a reach rod to assist in the accessibility of the fuel shut off valves from the weather deck.

Each survival craft must be stowed so that when the vessel sinks the survival craft floats free and, if inflatable, inflates automatically. The vessel's liferaft is missing a hydrostatic release.

Each ring life buoy must be marked in clearly legible block capital letters with the vessel's name. The ring buoy is not marked.

Each ring life buoy must be marked with Type II retro reflective material approved in 46 CFR 164.018. The reflective tape on the ring buoy is worn.

For sternlights and masthead lights and at 22.5 degrees abaft the beam for sidelights, the minimum required intensities shall be maintained over the arc of the horizon up to 5 degrees within the limits of the sectors prescribed in Rule 21. Vessel's stern light and masthead light are inoperable.
