A 18.5 feet long fractured was found in the structural starboard side stringer, at center-under the superstructure. Clean the fracture and repair to provide a strong bonding. 46 CFR 177.100. Code 17C.

Wood rot was found in the 1st, 2nd and 6th ring-frame bottoms starboard side forward hull. Crop and renew. 46 CFR 177.100 COde 17C.

Crop and renew a section of 22" by 14" of damaged, and wood rot found in the starboard side of the forward operating station controls. 46 CFR 177.100/ Code 17C.

Missing 02 support longitudinal frames missing (top & bottom) at the top of the 1st ring-frame, closer to the bulkhead in the starboard side hull forward. 46CFR177.100/ Code 17C.

Repair a fracture found at the top with the deck in the 1st ring-frame inner side starboard side hull forward, closer to the bulkhead, as identified. 46CFR177.100/ Code 17C.

The 2nd ring-frame in the starboard side hull forward was found loose.46CFR177.100/ Code 17C.

Wood rot was found below the forward center window of the operating station. Crop and renew. 46CFR177.100/ Code 17C.

Wood rot was found in the top vertical frame closed to the bulkhead, and also at the bottom plywood of the under deck, in the portside forward hull, right below the access hatch. 46 CFR 177.100 Code 17C.

Missing a support longitudinal frame missing at the top of the 2nd ring-frame, in the portside hull forward as identified. 46CFR177.100 Code 17C.

Electrical cables for the starboard side forward hull submersible bilge pump were found submerged in residual water. Secure cables as high as practicable above the bilges. 46 CFR 183.340 Code 16C.

An aft railing stanchion base in the starboard side was found loose. 46CFR177.100 Code 16C.

One of the portside batteries under the bench in the operating station was found with more than 04 cables connected into a terminal. Not more than 4 connection per terminals are allowed. ABYC E-11/ 46 CFR 183.340/ 46 CFR 183.350/ Code 16c.

Dead-end wires were found under the operating station. 46 CFR 183.200/ Code 16C.

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull, protection against fire, means of escape in case of a sudden unexpected casualty, guards and rails in hazardous places, ventilation of enclosed spaces, and necessary facilities for passengers and crew. Make the vessel ready for inspection in dry dock, to determine extent of damage, and structural condition of hull and internal structural frame, following a marine casualty. 46 CFR 177.100, Code 17C.

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull, protection against fire, means of escape in case of a sudden unexpected casualty, guards and rails in hazardous places, ventilation of enclosed spaces, and necessary facilities for passengers and crew. A railing stanchion support in the upper deck, starboard side midship was found lose. 46 CFR 177.100.

A vessel of more 65 feet in length must also have navigation lights that meet UL 1104, “Standards for Marine Navigation Lights,”. All navigation light-bulbs (side, stern, anchor and all around) shall be replaced to UL1104 standard to the satisfaction of an attending USCG Marine Inspector. 46 CFR 183.420.

Each vessel must have adequate emergency lighting fitted along the line of escape to the main deck from all passenger accommodation spaces. 01 main deck stbd side 1st forward lamp, and 01 under deck lamp stbdside on top of dampers were found inoperable.46 CFR 183.432 (a)

Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. Port and stbd side batteries were found to have installed more than 4 cables to each terminal poles. Connections on all batteries should be limited to no more than 4 per terminal. 46 CFR 176.800(a) & ABYC E-11

Electrical equipment on a vessel must be installed and maintained to:(a) Provide services necessary for safety under normal and emergency conditions;(b) Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock;(c) Minimize accidental personnel contact with energized parts; and(d) Prevent electrical ignition of flammable vapors. A ground cable was found disconnected in forward engine room portside next to the charger, and multiple dead-end wires were found under the operating station. 46 CFR 183.200

A submersible electric bilge pump may be used as a power bilge pump shall be listed by an independent laboratory as meeting the requirements in UL 1113. Submersible bilge pumps aren't UL 1113. Prove that current submersible bilge pumps are in compliance with UL, or replace with a UL approved. 46 CFR 182.520

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull. An upper transverse support wood frame located in the portside steering compartment was found rotten. Crop and replace with a new frame; and The wood located at port and starboard, inside of the outlet of the ventilation dampers in the engine room was found decayed. Sand to good wood and apply a wood preserver until next dry dock. 46 CFR 177.100

Provision must be made for closing all supply duct cowls or scoops and exhaust duct discharge openings for a space protected by a fixed gas extinguishing system. Port and starboard fire ventilation dampers failed to close upon actuation. 46 CFR 182.465 (h).

The marine inspector may require any part or all of the propeller shafting to be drawn for examination of the shafting and stern bearing of a vessel whenever the condition of the shafting and bearings are in question.The marine inspector may conduct a visual examination and may require nondestructive testing of the propeller shafting whenever the condition of shafting is in question. Portside shaft log was found with penetrations after removing the cutless bearing. Submit a repair proposal for approval of the USCG Sector San [name withheld] Office addressing the repairs to be conducted, process and materials, prior repairs being conducted. Repairs shall be verified by an attending USCG Marine Inspector. 46 CFR 176.670(a) & (b).

The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. If any of the equipment or records have not been properly maintained, a qualified servicing facility may be required to perform the required inspections, maintenance procedures, and hydrostatic pressure tests. Vessel pre-engineered CO2 system was serviced by a contracted technician 01MARCH2024, only a certificate was provided without evidence of what testing or verification was performed. Provide a servicing report of what type of inspection or verification was conducted to the CO2 system. 46 CFR 176.810 (2), code 16C.

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. Both VHF radios amp meters were found to have the back up battery to be expired upon testing. 17 CFR part 80 and 46 CFR 184.502, code 16C.

On a vessel of more than 65 feet in length, the minimum capacity of the fire pump must be 50 gallons per minute at a pressure of not less than 60 psi at the pump outlet. The pump outlet must be fitted with a pressure gauge. The pressure gauge at the outlet of the pump indicates "0" (no pressure) upon opening the flow valve. 46 CFR 181.300 (b).

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. An oil leak from an unknown source was identified to be leaking at the starboard side generator. Identify the source of leak for correction and clean the oil covering the generator. 46 CFR 176.830, code 16C.

Exhaust gas must not leak from the piping or any connections. An exhaust pipe leak was found in the starboard side exhaust engine pipe, with missing insulation. 46 CFR 182.430, code 16C.

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. Dead-end wires were found in the starboard side engine room under the fuel filters and below a sender unit. 46 CFR 176.830, code 16C.

All hatches and openings in the hull, except loading doors, of a vessel must be kept tightly closed except when being used. Port and starboard sides access hatches that leads to the steering apparatus at the stern of the vessel were found to have closing handles broken. Replace broken handles and prove proper operation of both hatches. 46 CFR 185.330(a), code 16C.

All fuel tanks must be electrically bonded to a common ground. Portside fuel tank ground was found covered in aluminum salt deposits, including the tank top. Clean tank top and electrical ground cables. 46 CFR 182.440, code 16C.

Connections must be made to battery terminals with permanent type connectors. Three (03) wing nuts were identified on the terminal connections.

A submersible electric bilge pump may be used as a power bilge pump provided that a positve shutoff valve is installed at the hull penetration. The bilge shutoff valves are currently installed after a pipe elbow and not at the hull penetration.

The color of the life floats must be primarily vivid reddish orange. Found three (03) - 22 passenger life floats were discolored and had cracks around the corners.

A penetration in a watertight bulkhead must be made watertight. Identified several cable penetrations on the port and starboard side passing through watertight bulkheads that were not made watertight.

A self priming, power driven fire pump must be installed on each vessel of more than 65ft in length. On a vessel of more than 65ft in length, the minimum capacity of the fire pump must be 50 gpm at a pressure of not less than 60psi at the pump outlet. The vessel has an electric fire pump that reaches a max of 30psi.

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. The owner or managing operator shall submit drawings, sketches, or written specifications describing the details of any proposed alterations to the cognizant [name withheld] to the port hull caused by anchor and chain becoming rapped around propeller was observed by marine inspectors. Anchor penetrated hull and allowed for water intrusion into rudder compartment. Damage to the propeller was also observed on multiple fins. Submit repair proposal for review by OCMI.

Where rigid non-metallic material (plastic) is permitted for use in piping systems its through hull fittings must be metallic. All bilge discharge hose connections and sewage discharge connections are fitted to plastic through hull fittings.

A submersible bilge pump may be used as a power bilge pump required by Table 182.520(a) provided that a positive shut-off valve is installed in the hull penetration. The vessel is fitted with RULE 3700 electric submersible bilge pumps approved by the MSC at plan review. All bilge discharge hose connections are clamped to the through hull fitting and are missing a shut-off valve.

At each inspection all observed unsafe practices must be corrected and and all required guards and protective devices must be in satisfactory condition. Both Main Diesel Engines are missing machinery guards around rotating machinery that may cause an injury to a crew member.

On a vessel in which the number of passengers permitted in the upper deck is limited by stability criteria the maximum number of passengers permitted must be indicated by a durable marking of at least 1 inch numbers and letters at the entranceway to that deck. The second deck’s passenger limitation is not marked.

A vessel must carry nautical charts appropriate for the intended voyage. Several nautical charts on board the vessel were not corrected and up-to-date.

At each vessel inspection all observed hazardous situations must be corrected. The port hull shaft log area (at frames 70-74) shows severe deterioration and objective evidence shows that there is water leaking and intrusion from this area.

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. The OCMI may require an inspection and testing whenever a repair or alteration is undertaken. Vessel shall submit repair proposals for damaged portions of the vessel and receive permission from Coast Guard before commencing repair work. Repairs shall be made to the satisfaction of Coast Guard Marine Inspectors prior to putting vessel in service.

A vessel with stability calculations issued under CFR Title 46, Subchapter S, must be fitted with permanent draft marks or have permanent loading marks placed on each end of the vessel.

A pre-engineered fixed gas fire extinguishing system must automatically shut down all power ventilation systems and all engines that draw intake from the space. The generator does not shutdown upon actuation of the fireboy system.

The vessel must be fitted with a FCC type accepted general marine radar system for surface navigations with a radar screen mounted at the primary operating station. Vessel does not have radar onboard. Vessel is restricted to carry 49 passengers until deficiency is cleared.

The vessel's pump outlet must be fitted with a pressure gauge. The fire pumps onboard do not have adequate pressure gages.

All cable wire must be installed with metal supports spaced not more than 24 inches apart. The cable in the engine compartments are supported by a plastic cable run.

MSDs must be secured to prevent accidental discharge of sewage. MSDs in stbd and prot compartments were found unsecured.

The instillation of the portable fire extinguishers must be to the satisfaction of the OCMI. The fire extinguisher in the operating station is not secured.

When stowed in tiers, life floats must be kept apart by spacers. Stacked life floats did not have spacers.

A suitable marine type strainer meeting the requirements of the marine manufacturer must be fitted in the fuel supply line. Marine Inspectors found plastic in fuel filters.

A vessel must carry adequate and up-to-date nautical publications. The vessel does not have the US Coast Pilot or current tables.

A vessel must carry appropriate day shapes. The vessel was found to be missing three balls and one diamond.

The vessel Master reported that the vessel had lost the port side rudder while in Palomino Island. Vessel owner/operator is to provide a report to the USCG, Sector San Juan, addressing the repairs to be conducted on the vessel and when the vessel will be taken out of the water to effect repairs.

Prior to operation repair/replace overheated port side shaft bearing and any associated parts that have been affected by the overheating.

Must notify U.S. Coast Guard Marine inspector 1, 3, and 6 months from this date to complete internal inspection of repairs made.

Repair delaminated sections on port and starboartd hulls

Replace zinc on propeller shaft.

Replace worn cutless bearings.

Make proper repairs to the port side bulkhead and deck.

Make proper repairs/replace to the port side motor to the satisfaction of an attending marine inspector.

Make proper permanent repairs to starboard fire main piping in way of the leak in the piping at the main deck penetration.

Mark vessel's name on the lifefloat paddles.

Make proper repairs to the port main diesel engine (lube oil pump failed causing an overheating condition) to the satisfaction of an attending marine inspector.

Replace leaking fire hose on port side.

Relocate EPIRB installation to a float-free position.

Make proper repairs to the satisfaction of an attending marine inspector prior to the carriage of passengers.

Provide automatic bilge pump operating indicator light at operating station.

Repair chaffed hydroulic hose in way of steering gear system.

Prior to carrying passengers for hire, vessel must undergo drydock inspection in the presence of a qualified Marine Inspector.

Make repairs to fractured bridge window prior to carrying passengers for hire.

Conduct underway tests of propulsion and steering systems with Marine Inspector prior to carrying passengers for hire.

Determine cause of propulsion system failure and correct the same prior to carrying passengers for hire.
