"On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces..." MI's observed the Middle and AFT bilge high level visual alarms not energizing when actuated. Operator has not responded to clear this def. Issuing prior to pax to have this cleared.

"Gauges to indicate engine revolutions per minute (RPM), jacket water discharge temperature, and lubricating oil pressure must be provided for all propulsion engines installed in the vessel. The gauges must be readily visible at the operating station." MI's observed the oil pressure gauge at the operating station was not reading pressure while engine was running.

"A vessel must have one or more ring life buoys as follows:(2) A vessel of more than 7.9 meters (26 feet) in length, but not more than 19.8 meters (65 feet), must carry a minimum of one life buoy of not less than 610 millimeters (24 inches) in diameter;" "At least one ring life buoy must be fitted with a lifeline. If more than one ring life buoy is carried, at least one must not have a lifeline attached." "Each floating waterlight installed after March 11, 1997, on a vessel carrying only one ring buoy, must be attached to the lanyard with a corrosion-resistant clip. The clip must have a strength of at least 22.7 kilograms (50 pounds), and allow the waterlight to be quickly disconnected from the ring life buoy." "Each life jacket, immersion suit, and ring life buoy must be marked in clearly legible block capital letters with the vessel's name. The marking is not required on a life jacket carried to meet a temporary need for additional life jackets, if the life jacket has the name of another vessel or company marked on it. For an immersion suit, the name of the person to whom the immersion suit is assigned is an acceptable alternative to the name of the vessel." MI's observed ring life buoy lifeline and floating light not attached to ring life buoy. Ring life buoy was not marked with vessel's name.

"Remote fuel shutoff stations must be marked in clearly legible letters at least 25 millimeters (1 inch) high indicating purpose of the valve and direction of operation" MI's observed both fuel shutoff valves were not marked.

"Provision must be made for closing all supply duct cowls or scoops and exhaust duct discharge openings for a space protected by a fixed gas extinguishing system. All closure devices must be readily available and mounted in the vicinity of the vent." MI's observed natural ventilation cowls for the engine compartment could not be secured.

"At each inspection for certification and at every other vessel inspection, quarters, toilet and washing spaces, galleys, serving pantries, lockers, and similar spaces may be examined to determine that they are serviceable and in a sanitary condition." MI's observed water in bilge from MSD tank.

Each door, hatch, or scuttle, used as a means of escape, must be capable of being opened by one person, from either side, in both light and dark conditions. The method of opening a means of escape must be obvious, rapid, and of adequate strength. Handles and securing devices must be permanently installed and not capable of being easily removed. A door, hatch or scuttle must open towards the expected direction of escape from the space served. Cabin door does not open towards the expected direction of escape.

"Your vessel must undergo an annual inspection w/in 3 months before or after each anniversary date." Vessel missed its annual inspection date and must apply for a new COI.

A VESSEL OF MORE THAN 26 FEET IN LENGTH, BUT NOT MORE THAN 65 FEET, MUST CARRY A MINIMUM OF ONE LIFE BUOY OF NOT LESS THAN 24 INCHES IN DIAMETER. EACH RING LIFE BUOY ON A VESSEL MUST BE IN GOOD CONDITION IN ACCORDANCE WITH 46 CFR 160.050 IN SUBCHAPTER Q. AT LEAST ONE RING LIFE BUOY OPERATING IN ALL DAY OPERATIONS, SHALL BE FITTED WITH ONE FLOATING WATERLIGHT APPROVED IN ACCORDANCE WITH 46 CFR 161.010 IN SUBCHAPTER Q. MI DISCOVERED THAT 01 RING LIFE BUOY (STBD SIDE MAIN DECK) WAS IN SUBSTANDARD CONDITION WITH MULTIPLE TEARS/EXPOSED FLOATATION MATERIAL. MI DISCOVERED ON THE SAME RING LIFE BUOY THAT IT'S ATTACHED FLOATING WATERLIGHT'S BATTERY WAS INOPERABLE. 46 CFR 180.70 (a)(2) 46 CFR 180.70 (d)

EACH LIFE FLOAT MUST BE FITTED WITH A FLOATING WATERLIGHT APPROVED UNDER APPROVAL SERIES 46 CFR 161.010. MI DISCOVERED THAT THE LIFE FLOAT'S FLOATING WATERLIGHT'S BATTERY WAS INOPERABLE. 46 CFR 180.175 (f)(4)

STORAGE FOR LIFE JACKETS MUST BE APPROPRIATELY MARKED WITH THE CORRECT AMOUNT OF LIFE JACKETS. MI DISCOVERED THAT THE STORAGE OF THE VESSEL'S LIFE JACKETS DID NOT MATCH THE AMOUNT OF FOUND LIFE JACKETS FOUND IN EACH STORAGE CONTAINER. 46 CFR 180.78

PLACARDS CONTAINING INSTRUCTIONS FOR THE DONNING AND USE OF THE LIFE JACKETS ABOARD THE VESSEL MUST BE POSTED IN CONSPICUOUS PLACES THAT ARE REGULARLY ACCESSIBLE AND VISIBLE TO THE CREW AND PASSENGERS. PROVIDE 2 ADULT LIFE JACKETS TO MATCH THE CURRENT LIFE JACKET PLACARD OR PROVIDE PLACARDS FOR THE 2 DIFFERENT LIFEJACKETS.

A VESSEL NOT REQUIRED TO HAVE A POWER DRIVEN FIRE PUMP MUST HAVE AT LEAST THREE 9.5 LITER (2 1/2 GALLON) BUCKETS, WITH AN ATTACHED LANYARD SATISFACTORY TO THE COGNIZANT [name withheld] AS TO BE EASILY AVAILABLE DURING AN EMERGENCY. THE WORDS “FIRE BUCKET” MUST BE STENCILED IN A CONTRASTING COLOR ON EACH BUCKET. PROVIDE 3 FIREBUCKETS.

A VESSEL MUST CARRY EITHER A FIRST-AID KIT APPROVED UNDER APPROVAL SERIES 160.041 OR A KIT WITH EQUIVALENT CONTENTS AND INSTRUCTIONS. FOR EQUIVALENT KITS, THE CONTENTS MUST BE STOWED IN A SUITABLE, WATERTIGHT CONTAINER THAT IS MARKED “FIRST-AID KIT”. A FIRST-AID KIT MUST BE EASILY VISIBLE AND READILY AVAILABLE TO THE CREW. PROVIDE NEW FIRST AID KIT.

THE COI AND ANY CERTIFICATES MUST BE POSTED UNDER GLASS OR OTHER SUITABLE TRANSPARENT MATERIAL, SUCH THAT ALL PAGES ARE VISIBLE, IN A CONSPICUOUS PLACE ON THE VESSEL WHERE OBSERVATION BY PASSENGERS IS LIKELY. EACH EPIRB, OTHER THAN AN EPIRB IN AN INFLATABLE LIFERAFT, MUST BE TESTED MONTHLY, USING THE INTEGRATED TEST CIRCUIT AND OUTPUT INDICATOR, TO DETERMINE THAT IT IS OPERATIVE. PROVIDE DOCUMENTATION OF MONTHLY EPIRB TEST.

THE FLOAT-FREE LINK IS DESIGNED TO BE BROKEN BY THE BUOYANT FORCE OF THE LIFE FLOAT OR BUOYANT APPARATUS SO THAT THE FLOAT OR APPARATUS BREAKS FREE OF A VESSEL THAT SINKS IN WATER DEEPER THAN THE LENGTH OF THE PAINTER. THE NUMBER AND IDENTIFICATION OF THE ITEMS STOWED INSIDE, AND THEIR SIZES, MUST BE MARKED IN CLEARLY LEGIBLE LETTERS AND NUMBERS ON EACH CONTAINER FOR LIFE JACKETS. LABEL CORRECTLY THE AMOUNT OF LIFE JACKETS IN STORAGE CONTAINERS OR PUT THE ACCURATE NUMBER MATCHING THE CURRENT NUMBER IN THE CONTAINER.

THE OWNER OR MANAGING OPERATOR MUST PROVIDE SATISFACTORY EVIDENCE OF THE REQUIRED SERVICING TO THE MARINE INSPECTOR. PROVIDE SERVICING REPORT FOR FIRE PROTECTION EQUIPMENT.

THE MARINE EMPLOYER MUST HAVE A SUFFICIENT NUMBER OF ALCOHOL TESTING DEVICES READILY ACCESSIBLE ON BOARD THE VESSEL. PROVIDE ALCOHOL TEST STRIPS.

Repairs to the machinery that affect the safety of the vessel must not be made without the approval of the cognizant OCMI. Vessel suffered a loss of propulsion on or about 5/18/2018. Operator must identify cause of loss of propulsion and notify the local USCG Marine Inspection Office of intended repairs. Vessel must conduct satisfactory sea trials in the presence of a USCG Marine Inspector prior to carrying passengers.

Repair 12 person life float IAW manufacture specification and install life flat IAW NVIC 01 83. Vessel is restricted to 1 mile from shore until the Friday before memorial day.
