MI identified section of flexible hose on the generator which did not have sufficient double clamping. 46 CFR 182.720(e)(3)(v)(B)

At each initial and subsequent inspection for certification, the owner or managing operator must be prepared to conduct tests and have the vessel ready for inspection of its fire protection equipment, including the following: Inspection and testing of all smoke and fire detection systems, including sensors and alarms. MI identified a smoke detector in the wheelhouse with a faulty battery. 46 CFR 176.810(a)(7)

Each ship transporting more than six passengers for hire equipped with a radiotelephone station in accordance with Part III of Title III of the Communications Act must carry a radio operator who meets the following requirement: Where the station power does not exceed 250 watts carrier power or 1500 watts peak envelope power, the radio operator must hold a marine radio operator permit or higher class license. Provide proof marine radio operator permit to MI. 47 CFR 80.159 (e)(1)

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. MI identified expired first aid kit; secondary first aid kit was also missing some of the required contents. A first-aid kit may be considered acceptable if it meets all of the requirements of ISO 18813:2006(E) paragraph 4.12. 46 CFR 184.710

An internal structural examination conducted in compliance with § 176.600 may be conducted while the vessel is afloat or out of the water and consists of a complete examination of the vessel's main strength members, including the major internal framing, the hull plating and planking, voids,... A crack was found across the wood stiffener between the center hatch and port engine hatch near the forward end where a cut had been made to install the new engines. Present acceptable proposal for permanent repair and make corrections prior to credit of next drydock 2027. 46 CFR 176.610(b)

A hatch exposed to the weather must be watertight. Water intrusion was found through a crack between the center hatch and port engine hatch where a cut had been made through wood stiffener to install the new engines. Prior to credit drydock, waterproof to mitigate water intrusion to wood stiffener pending permanent repair. 46 CFR 179.360(a)

A penetration in a watertight bulkhead must be as high and as far inboard in the bulkhead as practicable, and made watertight. MI observed a cable transit pipe on upper starboard corner of bulkhead in lazarette that was not watertight. 46 CFR 179.320(c)

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. MI identified a dead end wire in the upper starboard corner of the lazarette. Remove or terminate prior to credit drydock. 46 CFR 183.200(b)

A vessel not required to have a power driven fire pump by § 181.300 must have at least three 9.5 liter (2 1/2 gallon) buckets, with an attached lanyard satisfactory to the cognizant OCMI, placed so as to be easily available during an emergency. The words “FIRE BUCKET” must be stenciled in a contrasting color on each bucket. Fire buckets were too large. Make fire buckets 2.5 gallons, to the satisfaction of the marine inspector. 46 CFR 181.610

Except as otherwise provided in this section, machinery and fuel tank spaces shall be separated from accommodation spaces by watertight and/or vapor tight bulkheads of double diagonal wood, marine plywood, steel plate, or equivalent construction. Hole found between engine room and accomodation space. Make hole vapor tight to the satisfaction of the marine inspector. 46 CFR 182.35-1 "OLD T"

For fixed-gas fire extinguishing systems...The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. Owner did not have servicing report for Fireboy system. Provide report to marine inspector. 46 CFR 176.810(b)(1)(iv)

An adult life jacket must be provided for each person carried on board the vessel. 09 kapok life jackets were found to be unserviceable and need to be replaced. Vessel currently has 21 Adult life jackets on board. Personnel allowance on board the vessel while underway shall be reduced to 21 persons until remainder of appropriate life jackets is obtained.

Electrical equipment used in a machinery space must be drip proof. 02 plug in outlets in the fwd machinery space need covers to protect the outlets from moisture.

A pre-engineered fixed fire extinguishing system must automatically shut down all engines that draw intake air from within the protected space. Fire-boy did not shut down port and starboard engine.

An inspection for renewal of a Certificate of Inspection is conducted to determine if the vessel is in satisfactory condition, fit for the service intended, and applies with all applicable regulations. Vessel not cleared to carry passengers. Operator notified dispatch that the vessel will not be ready by COI renewal date.

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. Repairs or alterations that affect the safety of the vessel include, but are not limited to: replacement, repair, or refastening of deck or hull planking, plating, and structural members; repair of plate or frame cracks; damage repair or replacement, other than replacement in kind, of electrical wiring, fuel lines, tanks, boilers and other pressure vessels, and steering, propulsion and power supply systems; alterations affecting stability; and repair or alteration of lifesaving, fire detecting, or fire extinguishing equipment. Operator installed new engines and generator without approval from the OCMI. 46CFR176.700(a)

Remove oxidation or replace port/stbd navigation light lenses.

Provide shields for the port & stbd emergency fuel shutoff valves which isolate the valve handle from the protected space. 46 CFR 182.455(b)(4)
