Through hull fittings and shutoff valves must be metallic. In the case of nonmetallic hulls, materials that will afford an equal degree of safety and heat resistivity as that afforded by the hull may be approved. A plastic through hull fitting with no valve on the stbd sideshell within the engine room space was found with duct tape and sealant squeezed inside. Provide a wooden plug that can be placed in the through hull, in an emergency, the plug should be stowed immediately adjacent to the thru hull. This is accepted as a temporary means to prevent any water ingress, if needed.

Place a metallic through hull fitting and shutoff valve on the identified starboard through hull or provide a repair proposal to permanently remove the through hull.

Gauges to indicate engine revolutions per minute (RPM), jacket water discharge temperature, and lubricating oil pressure must be provided for all propulsion engines installed in the vessel. The gauges must be readily visible at the operating station. No lubricating oil pressure gauge was available at the operating station.

Each item of lifesaving equipment determined by the Marine Inspector to NOT be in serviceable condition must be repaired or replaced. The grab line on the ring buoy is in need of replacement due to UV deterioration.

For the purpose of flexibility only, flexible hose may be installed in lengths not to exceed 30" if installed with a suitable compression type connection (hose clamp). Install a hose clamp on the port main diesel engine (MDE) jacket water expansion line and crank case breather line. Install a hose clamp on the stbd MDE crankcase breather line.

The owner or managing operator shall have the vessel ready for inspections of machinery including all machinery essential to the routine operation of the vessel including cooling systems, replace the missing port jacket water expansion tank cap.

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons and the vessel from electrical hazards including fire, caused by or originating in electrical equipment, and electrical shock. Replace the cracked fire/bilge switch cover plate.

A hatch exposed to the weather must be watertight. Repair or replace the cracked engine room water tight hatch.

At each initial and subsequent inspection of a vessel the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances, including the following; Masts and similar arrangement constructed on the hull and on a sailing vessel all spars, standing rigging, running rigging, blocks, fittings, and sails. Although the vessel hauls out and down rigs every year, the last documented Coast Guard attendance and inspection of the masts when unstepped was 2012. In accordance with NVIC 02-16, wood masts, spars and appendages should be unstepped once every 10 years. Prior to the vessel operating in 2024, the vessel will need to down rig, unstep and make available for inspection for the Coast Guard all masts, spars, standing rigging, running rigging, blocks, fittings, and sails.

On a vessel of at least 26' in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each unmanned space. Found the visual indicators inoperable for each compartment's high level alarm. While underway, the crew is directed to make visual inspection of each compartment on a half hour schedule & make an appropriate entry in the vessel's log book. 46 CFR 182.530(a)

All cable and wire must be installed with metal supports spaced not more than 24 inches apart, and in such a manner as to avoid chafing and other damage. Wiring on aft bulkhead of forward tool space is supported with plastic zip ties and not the proper metal supports. Replace to the satisfaction of CG marine inspector.

Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers. No life jacket donning placard onboard.

Shut off valves must be fitted in the fuel supply lines, one at the tank connection and one at the engine end of the fuel line. Valve at the tank connection not properly connected.

A submersible electric bilge pump must have a positive shutoff valve installed at the hull penetration. No valve is installed in the engine room.

Flexible non-metallic hose may be used in a pneumatic system if each penetration is made using an acceptable metallic through bulkhead fitting that is welded or otherwise attached to the bulkhead and a metallic shut off valve must be installed at the bulkhead. Port side head space has a flexible hose going directly through the bulkhead.

Deck rails must be designed and constructed to withstand a point load of 200lbs. applied at any point in any direction. Vessel stanchions are loose.

All unfired pressure vessels must be installed to the satisfaction of the OCMI. Provide manufacturer documentation to ensure pressure vessel meets type approval.

The design, materials, and construction of standing rigging must be suitable for the intended service. Inner forestay deck attachment found with lead to load misalignment. Correct path to load on the inner forestay. Provide nondestructive testing (NDT) for the deck attachment. Replace bolts on the this attachment.

Portable fire extinguishers must be inspected and maintained in accordance with NFPA 10. Monthly inspections may be conducted by the owner, operator, person in charge, or a designated member of the crew. No monthly fire extinguisher inspections have been conducted.

Chainplates (8) at weather deck show signs of potential failure points. Clean to bright, provide a report on company letterhead that attests to the condition and continued serviceability of each. Vessel shall not operate under sail prior to satisfaction of this requirement.

MOB drills not completed due to hazderous weather conditions. Complete in spring before passenger operations.

First aide kit missing contents.

Provide reflective material for the ring buoys

Provide valid COD for 2015/2016

Provide proper medication for first aid kit.

Properly seal all through penetrations from engine space to aft compartment.

Provide proper ops of override for CO2 system onboard along with servicing report.

Total persons reduced to 60 by 185 AAWPP weight ratio. Awaiting stability test.

The stbd side #6 and #3 deck rails were damaged during an allision.

Bumkin on vsl was cracked/splintered during an allision.

Aft stbd quarter of vessel received cosmetic damage during an allision

Clean corroded battery terminals.

Remove PVC wire tray in engine room and replace with metal wire hangers.

Plug hole in fwd engine room bulkhead.

The forepeak deck hatch was found to have a crack in the polyethylene plastic component and accompanying aluminum frame.

Properly install bilge alarm system required by 46CFR 182.530 prior to operating with passengers in the 2006 season.

Provide line diagram/instructions for fire/bilge system NLT 22SEP05.

Provide proper retro reflective tape on ringbuoy.

Provide proof of proper registration

Provide proof of enrollment in a drug and alcohol program

Splice the life line around the vessels life ring, not just the line.

Address dead-end wires in engine room and lazarette.

Provide proof of enrollment in a drug and alcohol monitoring program.

Provide prof of testing and servicing of the Fixed Fire Fighting system.

Provide a spanner wrench for the fire station.

Provide updated charts for operating area.

INSTALL A PRESSURE GAUGE ON THE DISCHARGE OF THE FIRE PUMP IAW 46 CFR 181.300(B).

INSTALL FIRE PUMP OPERATING SWITCH AT OPERATING STATION IAW 46 CFR 181.300(E).

Provide original copy of COD.

Prove proper operation of fixed fire fighting system.

Prove proper repairs were made to fire main piping a last dry-dock as stated in inspection note dated 20Apr01.

Install new firemain piping IAW 46 CFR 182.710 and to the satisfaction of the attending marine inspector prior to operating in the spring of 2002.

Vessel rigging not in place. Notify attending Marine Inspector upon rerigging of vessel for rigging inspection, or provide inspector with report from third party rigging surveyor.

Prove updated chart of operating area.

Prove proper operation of aux steering.

Conduct man overboard drills,

Install visual indicator at operating station for automatic bilge pump or remove.

INSTALL EMERGENCY LIGHTING IN BELOW DECK PASSENGER SALON AS PER APPROVED PLANS.
