Auxiliary machinery of the internal combustion piston type must comply with the provisions of this part. A portable gasoline generator was found stored unsecured above the passenger deck canopy. This unauthorized installation poses stability risks by altering the vessel's lightship condition and vertical center of gravity (VCG). It additionally increases fire hazards from unapproved gasoline machinery and constitutes improperly secured equipment.

Repairs or alterations to the hull that affect the safety of the vessel must not be made without the approval of the cognizant [name withheld] decay was identified at the upper terminus of the starboard transverse frame IWO aft portion of the starboard water tank, where it meets the deck beam and clamp. Adjacent frames were intact without observed decay. The owner plans to complete extensive repairs/renewals at the next scheduled drydock for the frames and deck beams along the starboard side between the cabin to the bow, to match previous repairs completed for the remainder of the vessel. Prior to commencing any structural work, the owner/operator must submit a written repair proposal to the cognizant OCMI for review and approval. The proposal must detail the full scope of the intended work, the repair methods and materials to be used. All repairs must be completed to the satisfaction of the attending marine inspector by the next scheduled drydock.

Each connection to a conductor or terminal part of a conductor must be made within an enclosure. MI observed wire terminal strip mounted to forward bulkhead in engine room.

On a vessel of at least 26 feet in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in spaces with a through-hull fitting below the deepest load waterline, machinery bilge spaces, bilge wells, shaft alley bilge, or other spaces subject to flooding from sea water piping within the space. The vessel's forward high bilge level alarm was inoperable, and the midship high bilge level alarm's visual indicator did not illuminate.

Waste receptacle must be constructed of non-combustible materials with no openings in the side or bottom. The waste receptacles on the vessel's passenger deck were found to be made of plastic.

A vessel not required to have a power driven fire pump by 46 CFR 181.300 must have at least 3 2.5 gallon buckets, with an attached lanyard satisfactory to the cognizant [name withheld] of the vessel's three fire buckets was found to be missing a lanyard.

The owner or operator must provide satisfactory evidence of the required servicing of fire extinguishers to the marine inspector. A tag issued by a qualified servicing organization, and attached to each extinguisher, may be accepted as evidence that the necessary maintenance procedures have been conducted. At the time of inspection, the operator was not able to produce evidence of satisfactory servicing for fire extinguishers.

Marine employers shall establish programs for the chemical testing for dangerous drugs on a random basis of crewmembers on inspected vessels who: Occupy a position, or perform the duties and functions of a position, required by the vessel's Certificate of Inspection. Crew members were not included on the consortium. Members not included shall not perform duties onboard until registered in the consortium.

A submersible electric bilge pump may be used as a power bilge pump required by Table 182.520(a) only on a vessel of not more than 19.8 meters (65 feet) in length carrying not more than 49 passengers, other than a ferry, provided that: A positive shutoff valve is installed at the hull penetration. Vessel did not have installed shutoff valves.

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. Vessel did not have appropriate supplies which were not expired.

An adult life jacket must be provided for each person carried on board a vessel. There are 03 lifejackets with holes in them and thus not suitable for service.

Electrical equipment on a vessel must be installed and maintained to provide services necessary for safety under normal and emergency conditions. There is an exposed wire creating an electrical hazard on the forward edge of the canopy.

Electrical equipment on a vessel must be installed and maintained to minimize accidental personnel contact with energized parts. The 02 electrical outlets in the pilot house lack protective covers.

Electrical equipment exposed to the weather must be watertight. The stern light cover is broken and not watertight.

A portable or temporary electric cord or cable must be constructed and used in compliance with the requirements of § 111.60-13 in subchapter J of this chapter for a flexible electric cord or cable. The temporary electrical cord utilized in the pilot house does not meet the subchapter J requirements.

Remote fuel shutoff stations must be marked in clearly legible letters at least 25 millimeters (1 inch) high indicating purpose of the valve and direction of operation. There is not fuel shutoff valve marking.

Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The vessel does not have records of completed training.

Fire extinguishers in good condition and charged, but out of inspection date by two weeks. Inspection is scheduled for 2 days from date of COI inspection. Provide properly serviced fire extinguishers by 6/21/2019.

Charts not updated. Provide updated charts for operational area.

CO2 Cylinders must be hydor tested every 12 years. Fixed CO2 was last hydro tested in 2005.

Make engine compartment gas tight to ensure effectiveness of fixed CO2. Temp arrangements are authorized for 2 weeks.

Make collision blkd water tight. There is a 2”x3” opening on the port side, top of blkd

Conduct annual service of 01 B-II portable fire extinguisher.

Reattach and label remote fuel shutoff.

Replace floating waterlight, restricted to daytime operations.

Masthead is inoperable, replace.

Provide 60 feet of lifeline, dark in color or UV resistant.

Provide vessel name on port bow.

Provide 1 5 gpm hand bilge pump.

Provide 3 2.5 gallon fire buckets, labeled "FIRE BUCKET", with lanyard able to reach waterline.

Missing 01 Adult life jacket, provide 01 lifejacket. Vessel total persons on board restricted to 49.

Fuel fill missing bonding in hose section. Install bonding strap.

Vessel to complete annual inspection prior to carrying passengers.

Vessel to conduct underway sea trial in presence of marine inspector to verify watertight integrity following repairs to starboard side. Prior to carrying passengers.

Due to extensive repairs identified during 2015 hull exam and subsequent repair proposals dated June 5, 2015 and March 9, 2016, vessel is required to follow and complete items listed on repair proposal dated March 9, 2016 prior to operating after October 31, 2016.
