Monthly checks of the lifeboats on the vessel were logged as complete onboard the vessel's electrical log, however, PSCOs observed all six lifeboats onboard the vessel with non-functional / disconnected fuel shutoff valves. In addition, the vessel's #2 lifeboat was found not operational. Condition of the lifesaving appliances were not reported to the company. Based on the deficiency #2, an additional external audit of the safety management system audit is required. 74 SOLAS 20 IX/3.1 ISM Code A.10.1 & ISM Code A.9.1

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. All lifeboats Fuel shut offs were not operational. Lifeboat 2 was not able to start. Lifeboat 3 fuel line on the port side engine had non maritime fuel hose. Rescue boat on the starboard side had an exhaust leak. 74 SOLAS 20 III/20.2

In passenger ships such units shall give an indication of any fire and its location in any space served by the system and shall be centralized on the navigation bridge or in the main fire control station, which shall be so manned or equipped as to ensure that any alarm from the system is immediately received by a responsible member of the crew. PSCO observed smoke detectors 8-104/5-139/9-108 did not alarm when tested. 74 SOLAS 01 II-2/12.1.2.1

Fire protection systems and fire-fighting systems and appliances shall be maintained ready for use. Two Semi-portable extinguishers (1.4 & 1.11) in the engine room are over pressurized and outside of the green indication. 74 SOLAS 20 II-2/14.1.1

Where "A" class divisions are penetrated for the passage of electric cables, pipes, trunks, ducts, etc., or for girders, beams or other structural members, arrangements shall be made to ensure that the fire resistance is not impaired. Emergency Diesel Generator louvers did not open when testing the EDG. 74 SOLAS 97 II-2/18.1.1

The means of escape, including stairways and exits, shall be marked by lighting or photo luminescent strip indicators placed not more than 300 mm above the deck at all points of the escape route, including angles and intersections. The marking must enable passengers to identify the routes of escape and readily identify the escape exits. Additionally, escape route signs and fire equipment location markings shall be of photo luminescent material or marked by lighting. PSCO observed low level lighting on deck 9 aft were un-serviceable. 74 SOLAS 01 II-2/41-2.4.7

74SOLAS(97)III/16.2 EACH LIFEBOAT SHALL BE PROVIDED WITH AN APPLIANCE WHICH IS CAPABLE OF LAUNCHING AND RECOVERING THE LIFEBOAT. STARBOARD NUMBER 5 LIFEBOAT COULD NOT BE PROPERLY RECOVERED, DUE TO THE TRAVELING WIRE COMING FREE FROM PULLEY. LIFEBOAT COULD NOT BE PROPERLY SECURED FOR SEA. Action code: 15 a c

General arangement plans shall be permanently exhibited for the guidance of the ships officer. Fire Control Plan does not show modifications of steering gear room to include the addition of two category 13 spaces on the port and starboard side.

The minimum for integrity of all bulkheads and decks shall be as prescribed in tables 26.1 to 26.4. Vessel modified steeriung gear space of category 10 with the addition of two category 13 storage spaces on the port/starboard side. Table requires boundaries to be A-0.

When the fire extiguishing medium is sorted outside a protected space, it shall be stowed in a room which shall be effectively ventilated. PSCO found the ventilation damper in the deck 11 CO2 room was seized in the open position

Ventilation system of a passenger ship shall be fitted with controls that all fans may be stopped from either two seperate positions. PSCO observed power ventilation switch in port passageway deck 11 wated and warn.

Stowage of survival craft, stbd davit launched life raft was not secured. vessel crew re-attached strap and hydrostatic release.

Electrical installations shall be such that electrical services essential for safety will be ensured under various emergency situations. PSCO observed multiple emergency lighting stations not functional.

Compressed gas sylinders must always be secured and when not in use they must be stowed in a rack. PSCO found multiple compressed gas cylinders that were not secured adequately on deck 12 storage.

The following fire protection systems shall be kept in good order so as to ensure their required performance if a fire occurs: structural fire protection, including fire-resistant divisions, and protection of openings and penetrations in these divisions. The following fire screen doors had approximately a 6mm or greater gap around the sides and top: A60 10.241Bis, A0 10.234, A60 10.241, A60 10240 Bis, and A60 10.240. 74SOLAS 20 II-2/14.2.1

In addition to complying with the specific provisions for fire integrity of bulkheads and decks mentioned elsewhere in this part the minimum fire integrity of all bulkheads and decks shall be as prescribed in tables 26.1 and 26.2. (a) There was a substantial amount of combustibles found within A/C space 5.112, not a category 13 space (b) Fuel oil samples were located in the carpenter and plumber storage on deck 12, a pace categorized as a 13 instead of a category 14. solas 2001 II-2/26

ANY REQUIRED AUTOMATIC SPRINKLER SHALL BE CAPABLE OF IMMEDIATE OPERATION AT ALL TIMES. VESSEL FOUND WITH 9 SPRINKLER HEADS IN THE CASINO, AND 2 SPRINKLER HEADS IN THE DRESSING ROOM ON DECK 7 LEAKING. ADDITIONALLY TWO SPRINKLER HEADS ON DECK 4 AT FRAMES 96 & 98 WERE BENT BEYOND DESINGED OPERATIONAL LIMITS. SOLAS 74(97)II-2/12.1.1

Each liferaft or group of liferafts shall be stowed with a float free arrangement complying with the requirements of paragraph 4.1.6 of the code, so that each floats free and, if inflatable, inflates automatically when the ship sinks. The float free liferafts on both port and starboard side are being blocked by the stowade position of the liferaft davit.

The fire integroty of the divisions shall be maintained at openings and penetrations. The A60 boundary at the fire door (A60 9.208) has a hole that is above the door compromising the integroty of the boundary.

Electrical installations shall be such that the safety of passengers and crew from electrical hazards shall be ensured. The AC flat (station) on deck 9 AFT, had three elxtrical boxes that are a safety hazard.

Stairways and ladders shall be arranged to provide ready means of escape to the lifeboat and liferaft embarkation deck from all passenger and crew spaces. Housekeeping trolleys were found to be secured via straps in the egress route.

The vessel Oily Water Separator is not operational (OOC). Any ship of 10,000 GT & above shall be fitted with an oil filtering equipment complying with paragraph 7 Reg 14 or as per MARPOL Annex I Reg 14.2. For the meantime, vessel oily water shall retained all oily waste onboard or discharge to a recognized discharge facility for proper disposal. Log all events on the official Oil Record Book.

Fire doors in main vertical zone boundaries and stairway enclosures shall have an approximately uniform rate of closure not more than 40s and not less than 10s. Fire screen door 4.63 closes in approximately 2s.

The lifejackets shall not impede entry into the lifeboat or seating including operation of seatbelts in the lifeboat. To be rectified next U.S. port after sailing foreign.

Have vessel's classification approve of temporary repairs in regards to using water vice lube oil for shaft seal/bearing. If necessary vessel must be issued a condition of class to address the seal/bearing issue.

0750: Plans and booklets shall be kept up to date, any alterations being recorded. Fire Control Plan shows door in engine dept changing room. Room also contains washer and dryer. Room is labeled as pantry. 97 SOLAS REG 20 (1); 50ac

0750- Manually operated call points shall be provided as necessary throughout special category spaces and one shall be close to each exit from such spaces. Manual call point outside Canyon Ranch spa club near elevators Deck 12 was not operating. SOLAS 97 REG 37 1.4.2 (10c)

0750:The doors in A class divisions shall be self closing and have a uniform rate of closure of no more than 40 seconds and no less than 10 seconds. The men and women's Sauna doors (A0 12.258, 12.259) close in about 4 seconds & door latches did not properly work. Solas 97 Reg 30 (10c)

Fire Screen Door 7.179BIS is not effectively closing therefore failiing to meet the class "A" boundary requirements.

Photoluminescent lighting is wasted on deck 4, starboard side cooridor leading to control room.

0750; F/D throughout the VSL need adjustment to ensure proper closing to maintain proper fire boundaries. Doc., of for all doors is maintained by the Safety Officer. There appears to be a disconnect between when the doors are inspected and when repaired. Door 4-93 has been condemned but the F/P still shows it active. Inspection, and maintenance guidelines under ISM are in question, 70 A/B

0610; Minor cracks, shell damage and window damage was found on vessels tenders. Very little of no documentation was found, which identified the damages or suggested when repairs were going to be made. No reports were sent to the company. Minor failure of ISM between crewmembers and Senior Staff, 10.

Remove combustible materials from all fan rooms.

SOLAS II-2 requires A-60 boundary for life saving equipment. Verify glass on Deck 7 and installed sprinklers meet A-60 equivalency.

Ensure egress routes are clear throughout engine room, specifically port and starboard store rooms in auxillary space.

Provide audit report of all FSD's and their proper operation. Numerous doors found closing outside of time parameter (10-40 sec) and several of sampled FSD's were not closing properly.

It was determined that the vessel was not upholding its safety management system. The oil content meter, according the CHENG, had never been callibrated.

Bilge piping system in the engineering spaces does not match approved piping diagram. Bilge piping currently allows for multiple paths to by pass the installed oil filtering equipment. Provide Class/Flag report stating that the machinery space bilge piping is approved and in compliance with MARPOL Annex I. MARPOL Annex I/15.

Vessel required to launch starboard side lifeboats within 30 days.

Hole in #6 lifeboat on the starboard side, aft below the rub rail. Clear to the satisfaction of Class. SOLAS 74 Chapter III Regulation 20.2

#5 and #6 forward lifeboat gripes frayed in the eye and severely detiorated. Clear to the satisfaction of Class. SOLAS 74 Chapter I Regulation 11(a).

#5 lifeboat brake does not re-apply when released form the lifeboat. Clear to the satisfaction of Class. 74 SOLAS Chapter III Regulation 29(j)/Regulation 15.2.

Leak in hydraulic pipe connection at tank on the davit system for the #3 lifeboat. Clear to the satisfaction of Class. SOLAS 74 Chapter III Regulation 20.2.

Liferaft captains demonstrated lack of knowledge for liferaft equipment. Clear to the satisfaction of Class. SOLAS 74 Chapter III Regulation 19.4.2.

Fire detection system in Rooms 1100-1124 indicate wrong location at control panel. Clear to the satisfaction of Class. SOLAS 74 Chapter II-2 Regulation 13 Section 1.6.

Prove proper operation of starboard rudder in followup mode (logic card-printed circuit board burned out) preventing rudder from operation. Prior to departure from next U.S. port.

Provide a continuous means of recording for echo depth sounder w/in 30 days to the satisfaction of class. 33CFR164.35(i)

WTD 15 closes too slowly. Adjust closure rate of WTD prior to departure. SOLAS 74, II-1, 15.7.7

QAWTD leading out to the forward mooring deck failed light test. Prior to departure, adjust the door & provide documentation form RINA surveyor.

Semi-WTD 50 is in operable and has been inoperable for the past month according to the ship's records & parts are on order. Provide documentation from RINA addressing this door prior to departure.

Fire door 241 on deck 10 fails to latch. Repair latch prior to departure.
