Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. All lifeboats Fuel shut offs were not operational. Lifeboat 2 was not able to start. Lifeboat 3 fuel line on the port side engine had non maritime fuel hose. Rescue boat on the starboard side had an exhaust leak. 74 SOLAS 20 III/20.2

In passenger ships such units shall give an indication of any fire and its location in any space served by the system and shall be centralized on the navigation bridge or in the main fire control station, which shall be so manned or equipped as to ensure that any alarm from the system is immediately received by a responsible member of the crew. PSCO observed smoke detectors 8-104/5-139/9-108 did not alarm when tested. 74 SOLAS 01 II-2/12.1.2.1

Fire protection systems and fire-fighting systems and appliances shall be maintained ready for use. Two Semi-portable extinguishers (1.4 & 1.11) in the engine room are over pressurized and outside of the green indication. 74 SOLAS 20 II-2/14.1.1

Where "A" class divisions are penetrated for the passage of electric cables, pipes, trunks, ducts, etc., or for girders, beams or other structural members, arrangements shall be made to ensure that the fire resistance is not impaired. Emergency Diesel Generator louvers did not open when testing the EDG. 74 SOLAS 97 II-2/18.1.1

The means of escape, including stairways and exits, shall be marked by lighting or photo luminescent strip indicators placed not more than 300 mm above the deck at all points of the escape route, including angles and intersections. The marking must enable passengers to identify the routes of escape and readily identify the escape exits. Additionally, escape route signs and fire equipment location markings shall be of photo luminescent material or marked by lighting. PSCO observed low level lighting on deck 9 aft were un-serviceable. 74 SOLAS 01 II-2/41-2.4.7

The minimum for integrity of all bulkheads and decks shall be as prescribed in tables 26.1 to 26.4. Vessel modified steeriung gear space of category 10 with the addition of two category 13 storage spaces on the port/starboard side. Table requires boundaries to be A-0.

When the fire extiguishing medium is sorted outside a protected space, it shall be stowed in a room which shall be effectively ventilated. PSCO found the ventilation damper in the deck 11 CO2 room was seized in the open position

Ventilation system of a passenger ship shall be fitted with controls that all fans may be stopped from either two seperate positions. PSCO observed power ventilation switch in port passageway deck 11 wated and warn.

Stowage of survival craft, stbd davit launched life raft was not secured. vessel crew re-attached strap and hydrostatic release.

Electrical installations shall be such that electrical services essential for safety will be ensured under various emergency situations. PSCO observed multiple emergency lighting stations not functional.

Compressed gas sylinders must always be secured and when not in use they must be stowed in a rack. PSCO found multiple compressed gas cylinders that were not secured adequately on deck 12 storage.

The following fire protection systems shall be kept in good order so as to ensure their required performance if a fire occurs: structural fire protection, including fire-resistant divisions, and protection of openings and penetrations in these divisions. The following fire screen doors had approximately a 6mm or greater gap around the sides and top: A60 10.241Bis, A0 10.234, A60 10.241, A60 10240 Bis, and A60 10.240. 74SOLAS 20 II-2/14.2.1

In addition to complying with the specific provisions for fire integrity of bulkheads and decks mentioned elsewhere in this part the minimum fire integrity of all bulkheads and decks shall be as prescribed in tables 26.1 and 26.2. Each liferaft or group of liferafts shall be stowed with a float free arrangement complying with the requirements of paragraph 4.1.6 of the code, so that each floats free and, if inflatable, inflates automatically when the ship sinks. The float free liferafts on both port and starboard side are being blocked by the stowade position of the liferaft davit.

The fire integroty of the divisions shall be maintained at openings and penetrations. The A60 boundary at the fire door (A60 9.208) has a hole that is above the door compromising the integroty of the boundary.

Electrical installations shall be such that the safety of passengers and crew from electrical hazards shall be ensured. The AC flat (station) on deck 9 AFT, had three elxtrical boxes that are a safety hazard.

Stairways and ladders shall be arranged to provide ready means of escape to the lifeboat and liferaft embarkation deck from all passenger and crew spaces. Housekeeping trolleys were found to be secured via straps in the egress route.

The vessel Oily Water Separator is not operational (OOC). Any ship of 10,000 GT & above shall be fitted with an oil filtering equipment complying with paragraph 7 Reg 14 or as per MARPOL Annex I Reg 14.2. For the meantime, vessel oily water shall retained all oily waste onboard or discharge to a recognized discharge facility for proper disposal. Log all events on the official Oil Record Book.

Fire doors in main vertical zone boundaries and stairway enclosures shall have an approximately uniform rate of closure not more than 40s and not less than 10s. Fire screen door 4.63 closes in approximately 2s.
