Each vessel that operates on the high seas, or that operates beyond three miles from the coastline of the Great Lakes, must have on board a FCC Type Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. Vessel is restricted to routes within 3 nautical miles from shore until a new battery is installed onboard. (46 CFR 180.64)

Each life jacket must be marked with Type I retroreflective material approved in 46 CFR 164.018 or other standard specified by the Commandant. Observed faded, peeling retroreflective tape on lifejackets for 4 adults and 1 child. (46 CFR 185.604(h))

A vessel must carry one floating waterlight, unless it is limited to daytime operation, in which case no floating waterlight is required. Observed that the waterlight’s battery was expired. (46 CFR 180.70)

The painter must be of a dark color if synthetic, or of a type certified to be resistant to deterioration from ultraviolet light. Observed that the painter line of a lifefloat was deteriorated from sun damage. (46 CFR 179.320(a))

Each watertight bulkhead must be sufficient strength to be capable of remaining watertight with a head of water to the top of the bulkhead. Observed that the centerline of forward lazarette bulkhead has a penetration. (46 CFR 179.320(a))

Rails or equivalent protection must be installed near the periphery of all decks of a vessel accessible to passengers or crew. Observed that several port and starboard gunwhale stiffeners were cracked allowing water intrusion and affecting rail rigidity. Repair so as to prevent water intrusion and prevent further deterioration. (46 CFR 177.900(a))

Your vessel must undergo an annual inspection within the 3 months before or after each anniversary date. Vessel has not completed annual COI Inspection within allotted time frame. Complete satisfactory annual COI inspection prior to the carriage of passengers. 46 CFR 176.500(b)(1)

All escape hatches and other emergency exits used as means of escape must be marked on both sides in clearly legible letters at least 50 millimeters (2 inches) high: “EMERGENCY EXIT, KEEP CLEAR”. The markings for the emergency exit were faded.

Remote fuel shutoff stations must be marked in clearly legible letters at least 25 millimeters (1 inch) high indicating purpose of the valve and direction of operation. The markings for the fuel shutoff valves were faded.

Life floats and buoyant apparatus must not be stowed in tiers more than 4 feet high. When stowed in tiers, the separate units must be kept apart by spacers. The life floats on board were stowed in tiers without spacers.

Each life float shall be fitted with a life line, pendants, two paddles, a painter and a light. The water light on one of the life floats did not work properly.

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules. The starboard light was inoperable.

Shutoff valves, installed so as to close against the fuel flow, must be fitted in supply lines at the tank connection. Emergency fuel shutoffs, when operated from the weather deck, does not fully close the ball valves on the port and starboard fuel tanks. Make repairs to ensure full closure. 46 CFR 182.455(b)(4)

Marine employers shall establish a program for the chemical testing for dangerous drugs on a random basis of crewmembers on inspected vessels who: Occupy a position, or perform the duties and functions of a position, required by the vessel’s Certificate of Inspection. Employer was not able to provide proof of enrollment of employees in such program. 46 CFR 16.230(a)

No marine employer shall engage or employ any individual to serve as a crewmember unless the individual passes a chemical test for dangerous drugs for that employer. Employer was not able to provide proof of negative drug tests for employees. 46 CFR 16.210(a)

An interconnected detection and alarm system must: Be installed such that the activation of alarm in one area results in both audible and visual alarms in all areas required by 46 CFR 405(c) to be protected by the interconnected detection and alarm system. Alarm system that is installed does not activate other alarms in the system. Additionally, the audible alarm signal to the bridge is not audible over operational noises. 46 CFR 181.450(a)(2)

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected. Crew utilizes sharp hook gaff for handling and pick-up of Man Overboard that could injure a person in the water. Vessel to provide safer alternative prior to carriage of passengers. 46 CFR 176.830(a)

The owner, charterer, master or managing operator shall instruct each crew member, upon first being employed and prior to getting underway for the first time on a particular vessel and at least once every three months, as to the duties that the crew member is expected to perform in an emergency including, but not limited to, the emergency instructions listed on the emergency instruction placard required by § 185.510 of this part and, when applicable, the duties listed in the station bill required by § 185.514 of this part. (b) For a vessel described by 46 CFR 175.110(c), the training program in paragraph (a) of this section must address firefighting proficiency and must include, but need not be limited to - (Extensively listed in 185.420) - 46 CFR 185.420

Each collision bulkhead required by 179.210 must be constructed IAW with 179.320, except that a collision bulkhead: Must not be fitted with any type of penetration or opening except penetrations may be made if they are located as high and as far inboard as practicable, and they have a means to make them watertight. Hatch in collision bulkhead was found unfastened for an undetermined amount of time. Master secured prior to end of inspection.

Annual inspection. Your vessel must undergo an annual inspection within the 3 months before or after each anniversary date. Vessel is overdue for annual inspection. Complete inspection prior to carriage of passengers.

Vessel that is operating on a Coastwise or Oceans route must have an interconnected fire detection system in compliance with 46 CFR 181.450 installed in all enclosed areas where passengers and crew have routine access, including accommodation spaces and machinery spaces. The vessel is limited to a Limited Coastwise route until the installation of an interconnected fire detection system. 46 CFR 181.405(c)

An adult life jacket must be provided for each person carried on board a vessel. 04 Kapok life jackets were found unserviceable. Life jackets need to be replaced. Vessel is limited to carriage of 34 P.O.Bs.

Vessel operated beyond 20NM but not more than 100NM from the nearest land must be equipped with a MF-DSC frequency transmitter and capable fo transmitting J3E emmission within the band 1710 to 2850 kHZ in addtion to the VHF DSC radiotelephone installation. Vessel is equipped with DSC capable radio but radio is not set up to broadcast GPS location. Radio needs to be connected to GPS. 47 CFR 80.905 (a)(2)

When using DSC techniques, Coast Stations and Ship stations must use maritime mobile service identities assigned by the commission or its designees vessel's current MMSI does not match the new FCC Radio station authorization. Ensure VHF radio displays correct MMSI number. 47 CFR 80.103 (b)

A vessel not required to have a power driven fire pump must have at least three 9.5L (2 1/2 Gals) buckets with an attached lanyard placed so as to be easily available during an emergency. No fire buckets were found onboard. Required fire buckets were replaced onboard.

Chemical testing of personnel must be conducted as required by this subpart and in accordance with the procedures detailed in 49 CFR part 40. Consortium letter provided by owner was expired.

Machinery and fuel tank spaces must be separated from accommodation spaces by boundaries that prevent the passage of vapors. Several holes found on forward bulkhead in engine space.

A vessel must comply with the applicable requirements for any radio and EPIRB installations, including the requirements for a station license and installation certificates to be issued by the FCC, as set forth in 47 CFR part 80. Radiotelephony certificate expired.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: charts of large enough scale to make safe navigation possible. Onboard charts not current.

Each vessel carry sufficient lifesaving equipment to provide for timely rescue and survival of the personnel on board in the event of a casualty to this vessel. Observed severe cracking and deterioration on top of one of the four 12 pax life floats (forward, upper Jim-Buoy). 46 CFR 180.01-5 [Old T]

If the life float does not have a painter attachment fitting, a means for attaching the painter must be provided by a wire or line that encircles the body of the device and has a breaking strength that is at least the breaking strength of the painter. Painter lines were secured to outer grab lines and not around the body of the life float. 46 CFR 180.20-1(a)(3) [Old T]

Rails or equivalent protection shall be installed near the periphery of all weather decks accessible to passengers and crews. Guard rails (port side aft and mid ships) had minor flexing and moving on their mounting bolts when body weight was placed on them and tugging. 46 CFR 177.35-1

Each ring life buoy with a floating water light must have a lanyard of at least 190 millimeters (3 feet) in length, but not more than 1,830 millimeters (6 feet), securing the water light around the body of the ring life buoy. Water light was attached to the outer grab line and not around the body of the ring life buoy. Rectified on the spot. 46 CFR 180.70 (d)(2)

Life preservers shall be of first-class workmanship and shall be free from any defects materially affecting their appearance or serviceability. 01 life preserver found cracked, removed by operator on the spot and replaced with extra PFD. 46 CFR 160.002-4(i)

11109 - Float-free link on both sets of life floats installed in a manner that would prohibit float-free deployment. Properly install float-free links for both life floats. 46 CFR 180.130

04109 - Owner could not provide records of fire, abandon ship drills, and monthly EPIRB tests. Conduct fire and abandon ship drills with CG Marine Inspector and test/log EPIRB. 46 CFR 185.524

07124 - Owner could not provide records of servicing for fire extinguishing equipment onboard. Provide documentation for most recent fire extinguishing equipment service. 46 CFR 176.810

10111 - Owner could not provide suitable back up for electronic charts. Provide back up for charts. 46 CFR 184.420

11118 - Bench seats in main cabin had two locations with placards indicating child life jackets stored within, however, only one location contains child life jackets. Update placards to reflect correct location of child life jackets. 46 CFR 180.78

02108 - Two dead end wires located under helm and one in the lazarette. Properly cap dead end wires. 46 CFR 183.300

09112- First aid kit missing ammonia inhalant. Provide ammonia inhalant. 46 CFR 184.710
