Provisions shall be made to facilitate cleaning, inspection and maintenance of main propulsion and auxiliary machinery including boilers and pressure vessels. Main engine has excessive fuel oil leaks and lube oil leaks from multiple areas and prime movers for generator sets have excessive fuel oil and lube oil leaks. These conditions have caused the bilges and sludge tanks to be filled with oil causing a hazard to crew and vessel.

Life jackets where moved from marked storage location, to unmarked stowage location. Request audit of the storage of all life saving gear and the location indicated on the general arrangement plan.

Main and emergency switchboards shall be so arranged as to give easy access as may be needed to apparatus and equipment, without danger to personnel. High voltage locker being used as a storage lockers and access to panels in main switchboard not accessible. Storage of combustible paper products are being stored above main switch board, causing a fire hazard.
