Repair/replace damaged starboard side reduction gear and prove proper operation in the presence of a USCG Marine Inspector prior to the carriage of passengers.

Conduct man overboard, fire, security and anchor drill in the presence of a USCG Marine Inspector prior to the carriage of passengers.

Conduct sprinkler test on vehicle decks in the presence of a USCG Marine Inspector prior to the carriage of passengers.

Unable to complete STBD MDE PTSP's due to reduction gear failure. Complete PTSP's prior to carriage of passengers.

Repack stuffing tubes on aft upper deck electrical wiring to A/C units.

STBD side aft door on accommodation deck is not closing properly/bottom of door is rubbing on deck preventing door from shutting on its own. Prove door operates properly.

Remount speaker on the port stack.

Replace cage on lighting globe in galley walk-in cooler.

Repair/replace dist panel door in galley.

Repair sprinkler head bracket stbd side under ramp midship

Provide evidence that the main fire pump operates as designed and reaches 50 or above PSI

Sprinkler pumps that can be used to provide required water PSI were not readily available.

STBD Main Diesel Engine coolant deliver pump leaking. Provide evidence that the pump has been repaired/replaced to stop the leak

L/O pressure switch above the STBD Main Diesel Engine coolant pump is leaking. Provide evidence that the pump has been repaired/replaced to stop the leak.

Replace worn fire hose.

Remove paint from waterlight.

Prove proper operation of smoke detector D18 Parts are backordered until January.

Prove proper operation of Low Lube Oil alarm. Parts on order.

Replace port engine tach on port bridge wing. Currently inoperable.

Fire panel has error code z90 for several spaces. Clear code, test detectors, provide report. This was cleared while on board.

The raw water inlet and discharge lines for #2 generator were found wasted IWO heat exchanger, make repairs.

The wiring for stbd M/E exhaust temp sensors were found pulled from junction box. Return wires to junction box.

Repair gasket on stbd M/E reduction gear cooler. Gasket found to be leaking.

#2 generator high water temp sensor found inoperable during testing. Install new. New sensor installed and tested sat, cleared while on board.

The STBD reduction gear cooling lines have 2 soft patches. Renew piping IWO soft patches.

Vessel is authorized for *PUMS on COI, however, the vessel is not fitted with an engineer assist alarm or dead man alarm. Vessel PUMS endorsement revoked, amending COI.

The alarm indicator for the "STBD main high oil" is out in ECR. Replace bulb.

Complete the following tests at the 1st quarterly re-inspection: Boiler automation tests Control Air Receiver Relief Valve

Make repairs and prove proper operation of the #3 generator reverse power relay prior to operation.

Make repairs and prove proper operation to CO2 piping (for bow thruster siren). MI's witnessed leaking of CO2 from behind installed fire insulation boundary.

Make repairs to fire door on stbd fwd main car deck (to stairwell). Wire re-inforced glass is cracked.

Provide lithium ion (AA) batteries IAW floating electric waterlight instructions. MI's found alkaline batteries in place.

Make updates to VRP with new employees name/contact info, and remove employees no longer in charge. MI's found that Mario is still listed in contacts and he no longer works at Bridgeport & Port Jeff Ferries.

Doors in stairway enclosures and main vertical zone bulkheads shall be of the self-closing type. Fire doors from car decks do not latch closed. Adjust doors to self-close (some need handle adjustments, and some tension). 46 CCFR 72.05-25(b)(9)

Vessels manual fire alarm system stations must be of an approved type. The pull station located outside the crew space on aft starboard side was unable to be tested due to corrosion within the locking mechanism. Repair/replace pull station and test for proper functionality.

Repair lighting in voids 5 & 7, currently not working.

Prove proper operation of #2 generator reverse power relay.

Prove proper operation of port main engine shutdown for CO2 discharge.

Fire Detectors must be located in all spaces except those having little or no fire risk. While conducting a quarterly re-inspection MI’s identified two heat detectors (only 1 heat operational of 3) in the bow thruster space and one heat detector in the steering compartment that showed Z90 invalid reply on the fire detection panel. MI’s conducted inspection of the heat detectors and found them damaged and hanging from the mounting bracket. Vessels crew could not say how long they were like that but admitted they have been that way for “a while”. This indicates the crew and company management are not proactively managing the vessel. Vessel to prove proper operation of heat detectors in steering compartment and bow thruster space. 46 CFR 76.27-80(b)

The inspector shall examine the bilges in machinery spaces to see there is no accumulation of oil which might create a fire hazard. During inspection of the bow thruster space it was noted that there was approximately 20 inches of oil and water in the bilge. At least 3 inches of oil was on top of the water. On the COI (3/30/21) inspection similar conditions were found and documented in the engine room. This indicates the crew and company management are not proactively managing the vessel. Remove oil and clean bilges in bow thruster space. 46 CFR 71.25-45

The position of the rudder must be shown at the alternative steering station. Rudder angle indicator at emergency steering station is not operational. This was documents as a CG-835 at the COI (3/30/21) and not corrected and is overdue. No repairs have been made to date and MI’s were never contacted requesting an extension. This presents a risk to safe navigation in an emergency. Prove proper operation rudder angle indicator. 46 CFR 77.05-1 to 113.40-5

No flammable or combustible liquids may be stored in any service space. During inspection of the MSD space a large trash can was found with approximately 20 gallons of hydraulic fluid that has been drained from the elevator space and is being stored. Remove hydraulic oil from MSD space. 46 CFR 147.45(b)

The inspector shall examine the bilges in machinery spaces to see there is no accumulation of oil which might create a fire hazard. During inspection of the engine room a leak for the stbd main engine lube oil cooler was found with a leak on the flange in way of the gasket. The leak appeared to be on going due to staining. There was approximately half gallon of lube oil underneath the cooler. Remove excess oil and repair leak in lube oil cooler. 46 CFR 71.25-45

Oil valves should be readily accessible from outside the compartment which the valves are located. During the inspection of the car deck MI's identified vehicles parked over the emergency fuel shut offs for the bow thruster space. In this condition the fuel shut offs would not be able to be closed in an emergency. Provide corrective action plan to ensure emergency fuel shut offs remain accessible at all times. 46 CFR 56.50-60(d)(1)

Crop and renew escape hatch in sewage void.

Fire screen door leading from galley to service area will not latch. Adjust door so when the door is released it will close and latch.

During engine room examination the engine room was found to have excessive oil in the bilge. Remove oil and water from bilge.

Prove proper operation of rudder angle indicator in emergency steering space.

Prove proper operation of #1 SSDG high water temp. alarm.

Prove proper operation of STBD M/E high gear oil alarm.

Prove proper operation of #1 bow thruster gear oil pressure alarm.

Overhaul and prove proper operation of escape hatch from MSD space.

Make permanent repair to hole in main car deck port side aft in way of stanchion tube for removable fence.

Repair hydraulic leak in operating station for engine room water tight door.

provide lagging for any piping in the engine room over 150 degrees as noted.

Replace high water temperature sensor for #2 bow thruster prime mover

Replace computer card in alarm panel for the low gear oil alarm on the stbd main engine

make pressure vessels in E/R available for inspection.

Re-hang light fixture located in the steering gear void. Light fixture was hanging by the electrical supply wire.

Provide lagging on all piping on the port and starboard main engine oil coolers that are in excess of 150 degrees.

Weld up holes in work bench adjacent to the engine room water tight door to prevent the actuation handle from being held in the open position by a screw driver or other object. Engine room water tight door was found rigged in such a way as to prevent the door from being closed.

Old sewage treatment plant needs to be cleaned out and secured due to leaking.

Remove old sewage treatment plant at next scheduled dry-dock

Remove and properly dispose of all oily waste water from bilge in engine room and MSD space.

Engineer assist alarm needs to be installed in the engineer's state room.

Company must provide full size paper copies of approved plans related to the crew cabin additions and modifications. This requirement is noted on each MSC approval letter as a condition of approval in the electronic commerce program.

A transformer is installed under the port side stairs to the uppermost deck in order to step down 240v to 120v. Ground detection must be installed on the newly installed isolated 120v distribution system as per 46 CFR 111.05-21(d). Plans must be updated and approved by MSC prior to system being placing in operation.

Install properly calibrated time delays for the Bolier, EDG and Paint Locker Spaces

Properly install H2O sensor on #2 generator

Fire detection system is alarming on heat detector #59 intermittently and pull station #28 continually. The fire detection system also has a ground fault.

Fire alarm bell in pilot house has been disabled by removing the contact maker and turning the bell around. Prove proper operation of fire alarm bell.

Repair or replace port and stbd side shell watertight doors and prove proper operation.

Replace wasted sprinkler piping as needed and conduct test in presence of CG MI.

Repair or replace alarm for engine room water tight door.

Remove dead end wire on port aft car deck.
