Identified a battery charger connected to the port side generator starting battery. The temperature sensor compensator for the battery charger was not installed. The batteries were found discharging water from the vents indicating they are overcharged. Hydrogen gas was observed discharging from the battery vents using a gas detector. Corrosive liquids and solids were observed on the battery terminals. The operator stated that the batteries are replaced very frequently. The battery charger is not installed in accordance with the manufacturer specifications (battery type, temperature sensor, etc.) causing the batteries to overcharge and emit an explosive gas as a by product of overcharging.

The starboard side steel tow cable was found with multiple broken strands and permanent bends in the cable. The hand wheel used to brake the tow cable was heavily corroded and holed. Found a four inch corroded section of the hand wheel holed creating a hazard to personnel using it.

MI found corrosion and a hole in the port side engine room overhead immediately above the generator. The hole measured 2 inches in diameter and contained corroded metal around it. The hole is located next to the fuel vent on the port side main deck inside of the containment.
