Starboard F/O inlet hose into filter bank found to be deteriorating and twisted. Port F/O inlet hose into filter bank found chaffed. Replace both F/O hoses with chaffing gear installed. 46 CFR 143.205 (a)

Each towing vessel must have a reliable means to provide notification when an emergency condition exists, or an essential system develops problems that require attention. MDE alarm continuously alarmed while underway. Investigate reasons, rectify and test. 46 CFR 143.230 (a)

Emergency lighting must be provided for all internal crew working and living areas. Automatic, battery-operated with a duration of no less than 2 hours. Pilot house emergency lighting illuminated then immediately dimmed. Replace with new battery and test. 46 CFR 143.410 (b)

Each towing vessel equipped with rudder(s) must have a means to monitor and control the position of the rudder(s) at each operating station. Both primary and flank rudder angle indicators were showing opposite of actual angles. Make repairs to show correct angle indication. 46 CFR 143.225 (b)

Towing vessels must meet the communications requirements of 33 CFR part 26 and 33 CFR 164.72, as applicable. Vessels VHF did not have MMSI or GPS programmed in. Program in the MMSI and GPS for DSC. 46 CFR 140.715 (a)

A current magnetic compass deviation table or graph or compass comparison record for the steering compass, in the wheelhouse. No deviation table. Complete deviation table and post in wheelhouse. 33 CFR 164.35 (c)

A towing vessel subject to this sub chapter and choosing the Coast Guard inspection option, or required to have the Coast Guard inspection option, must undergo an annual inspection within 3 months before or after the COI anniversary date. Vessels anniversary date was January 6th, and vessel is past 3 months from said date. Vessel shall not operate towing cargo until an annual inspection has been completed. 46 CFR 137.200 701c

Vessel representatives notified our office that the stbd shaft had broken. Repair to the satisfaction of the attending marine inspector. 46 CFR 143.205(a) 17C

A towing vessel must carry a combination of day and night visual distress signals indicated in Table 141.370 of § 141.370 for specified areas where the vessel operates. Vessel is required 3 day and 3 night flares, but vessel had 3 day flares that were past the expiration date. 46 CFR 141.375

All vessel equipment must be used in accordance with the manufacturer's recommended practice and in a manner that minimizes risk of injury or death. Vessel's portable fire extinguishers were not mounted in accordance with manufacturer's instructions. 46 CFR 140.610(f)

Decks and bulkheads designed to be watertight or weathertight must be maintained in that condition. Port side main deck door was not weathertight and needs adjustment. 46 CFR 140.610(f)

The master must ensure that all lifesaving equipment is properly maintained and ready for use at all times. MI identified retro-reflective material on life ring buoys was faded. 46 CFR 141.230

An installation or multiple installations of VHF-FM radios as prescribed by part 26 of this chapter and 47 CFR part 80, to maintain a continuous listening watch on the designated calling channel, VHF-FM Channel 13 (except on portions of the Lower Mississippi River, where VHF-FM Channel 67 is the designated calling channel), and to separately monitor the International Distress and Calling Channel, VHF-FM Channel 16, except when transmitting or receiving traffic on other VHF-FM channels or when participating in a Vessel Traffic Service (VTS) or monitoring a channel of a VTS. (Each U.S. towing vessel of 26 feet (about 8 meters) or more in length, except a public vessel, must hold a ship-radio-station license for radio transmitters (including radar and EPIRBs), and each operator must hold a restricted operator's license or higher. Each towing vessel certificated under the provisions of this subchapter must be in full compliance with the terms of the COI. Vessel is overdue for annual inspection. 46 CFR 136.200(b)

The officer in charge of a navigational watch must conduct a navigational assessment for the intended route and operations prior to getting underway. This assessment must consider, at a minimum, the items outlined in 46 CFR 140.635(a)(1)-(13). MI observed no record of navigation assessment for the vessel operations. 46 CFR 140.635(a)

Vessels may use a consortium to implement the required drug testing requirements. MI observed vessel enrollment in consortium had expired in 2019. 46 CFR 16

Emergency instruction must be conducted annually for the required elements of 46 CFR 140.420(d)(1)-(5). MI observed that no instruction had been completed for the launching of a skiff, which was used as emergency equipment for abandon ship. 46 CFR 140.420(d)(1)

Refresher training of the items located in 46 CFR 140.515 must be repeated annually. MI observed that no refresher training had been conducted for the crew. 46 CFR 140.515(d)

Prior to assuming duties related to the safe operation of a towing vessel, each crewmember must receive the training outlined in 46 CFR 140.645(a)(1)-(6). MI observed that no evidence of the required navigation safety training being conducted. 46 CFR 140.645

Vessels with a TVR must ensure the required entries located in 46 CFR 140.915 (1)-(10) are being recorded. MI observed that the vessel had none or missing records of the following: 1. Personnel records 2. Safety orientation 3. Record of drills and instruction IAW 140.420 4. Navigation assessment 5. Navigation safety training 46 CFR 140.915

The owner or managing operator must implement procedures for reporting unsafe conditions and must have records of those conditions being reported. MI observed that the captain of the vessel had reported an unsafe condition, water was found in the stbd side fuel tank prior to dry dock, and there was no record of this condition being recorded. 46 CFR 140.505(a)

Each towing vessel must be fitted with a general alarm that is capable of notifying persons in the engine room, and has installed a supplemental flashing red light that is identified with a sign. Vessel did not have a red flashing light for the general alarm in the engine room. 46 CFR 143.235

Essential systems or equipment must be regularly tested and examined. Each towing vessel must perform the applicable tests in Table 143.245(b). Vessel had no record of testing alarm setpoints for propulsion and auxiliary machinery alarms, as described in 46 CFR 143.230. 46 CFR 143.245(b)

Each lifejacket must have a light approved under series 161.012 or 161.112 securely attached to the front shoulder area of the lifejacket. MI found 01 lifejacket marker light to be expired. 46 CFR 141.340(g)(1)

At least one lifebuoy must have a lifeline, secured around the body of the lifebuoy. If more than one lifebuoy is carried, at least one must not have a lifeline attached. MI found lifelines spliced to all lifebuoys onboard. Remove 01 lifeline. 46 CFR 141.360(c)(1)

A towing vessel must carry adequate and up-to-date charts, maps, and nautical publications for the intended voyage. Vessel did not have up-to-date charts onboard. 46 CFR 140.705(b)

Decks and bulkheads designed to be watertight or weathertight must be maintained in that condition. MI observe a cable pass-through fitting on the engine room aft bulkhead above the hydraulic lines that is not watertight. 46 CFR 140.610(f)

Each owner or managing operator must develop procedures to routinely examine, maintain, and replace capstans, winches, and other machinery used to connect the towing vessel to the tow. MI observed the forward port and starboard cable rollers seized. 46 CFR 143.460(e)

The owner or managing operator of a towing vessel must establish procedures to address fall overboard prevention and recovery of persons in the water. MI observed during the drill the crew member did not have a safety tether to prevent falling overboard during the Man Overboard Recovery. In addition, the crew member utilized a line handling hook which has two sharp spikes on the end. 46 CFR 140.425(a)

The officer in charge of a navigational watch must conduct a navigation assessment for the intended route and operations prior to getting underway. Vessel did not have a Navigation Assessment onboard resulting in additional non-confinities to include AIS reporting a status of “Sailing Underway” while the vessel was moored. 46 CFR 140.635(a)

The results of crew changes, and examination, testing, failure and subsequent repair or replacement of vessel's equipment must be recorded and logged. MI discovered that the vessel was missing various log entries for required operations, to include: Watch/crew changes (when applicable), pre-underway checks, machinery tests, inspecting navigational equipment, towing gear, and fire fighting equipment. 46 CFR 140.400 46 CFR 140.615 46 CFR 140.620 46 CFR 140.820 46 CFR 142.240 46 CFR 143.245

Each towing vessel must have a fire-detection system installed to detect engine room fires. The fire-detection alarm panel was inoperable under primary power and emergency power. The crew members were unaware of the inoperable fire-detection panel. 46 CFR 142.330(a)

To stop the flow of fuel in the event of a fire or break in the fuel line, a remote fuel shutoff valve must be fitted on any fuel line that supplies fuel directly to a propulsion engine or generator prime mover. The fuel shutoffs for the generator, port and starboard main engines are found inoperable when tested by the crew. 46 CFR 143.260(b)

MI observed an exhaust leak on the port main engine when energized. The exhaust smoke filled the engine room compartment endangering personnel within the space. 46 CFR 143.205(a)

Electrical systems and equipment must function properly and minimize system failures and fire and shock hazards. Identified a battery charger connected to the port side generator starting battery. The temperature sensor compensator for the battery charger was not installed. The batteries were found discharging water from the vents indicating they are overcharged. Hydrogen gas was observed discharging from the battery vents using a gas detector. Corrosive liquids and solids were observed on the battery terminals. The operator stated that the batteries are replaced very frequently. The battery charger is not installed in accordance with the manufacturer specifications (battery type, temperature sensor, etc.) causing the batteries to overcharge and emit an explosive gas as a by product of overcharging. 46 CFR 143.400(a)

Pressure vessels must be externally examined annually. MI observed fractured welds and substantial corrosion on two of the three pressure vessel foot brackets for the 200 MAWP pressure vessel. The P/V foot brackets are welded to the deck in the engine room. The welds were found fractured in between the deck and brackets. 46 CFR 143.300(b)

The owner, managing operator, master or officer in charge of a navigational watch of a towing vessel must ensure the following: the strength of each component used for securing the towing vessel to the tow and for making up the tow is adequate for its intended service. The starboard side steel tow cable was found with multiple broken strands and permanent bends in the cable. The hand wheel used to brake the tow cable was heavily corroded and holed. Found a four inch corroded section of the hand wheel holed creating a hazard to personnel using it. 46 CFR 140.801(a)

A towing vessel must be maintained and operated so the watertight integrity and stability of the vessel are not compromised. MI found corrosion and a hole in the port side engine room overhead immediately above the generator. The hole measured 2 inches in diameter and contained corroded metal around it. The hole is located next to the fuel vent on the port side main deck inside of the containment. 46 CFR 140.605(b)

All towing vessels must be operated in a manner that minimizes the risk of down-flooding and progressive flooding. The master must ensure that all hatches, doors, and other openings designed to be watertight or weather-tight function properly. MI observed approximately one foot of standing water in the port and starboard forward voids due to poor gasket maintenance and failed gaskets. 46 CFR 140.610(b)

The following systems must be equipped with gauges at the machinery location: Auxiliary generator engine lubricating oil pressure, auxiliary generator engine RPM and auxiliary generator engine cooling water temperature. The vessel owner installed a new auxiliary generator. The gauge panel was found installed approximately six feet away from the generator and the gauge panel is not permanently secured to prevent shifting. 46 CFR 143.230(c)

Electrical systems and equipment must function properly and minimize system failures and fire and shock hazards. Welding cable found installed for all battery conductors and starting conductors for the main engine and aux generator. Welding cable thermal protection is not sufficient to prevent fire hazards for conductors in an engine room. 46 CFR 143.400(a)

Electrical connections must be suitably installed to prevent them from coming loose through vibration or accidental contact. MI observed plastic zip ties used to support the port engine air intake. 46 CFR 143.400(e)

Each towing vessel must be equipped with navigation lights, shapes, and sound signals in accordance with the International Regulations for Prevention of Collisions at Sea (COLREGS) or 33 CFR part 84 as appropriate to its area of operation. One towing mast light was found extinguished. 46 CFR 140.720

A towing vessel must carry a combination of day and night visual distress signals indicated in Table 141.370 of § 141.370 for specified areas where the vessel operates. One visual distress signal was found expired and the remaining expire in 30 days. 46 CFR 141.375(a)

A towing vessel of 26 ft or more in length and more than 600 HP engaged in commercial service must have a functioning AIS. Found AIS with error code. AIS is not functioning as per manufacturer. 33 CFR 164.46(b)(ii)

The owner or managing operator must implement procedures to identify and mitigate health and safety hazards. Five emergency deck lights were found inoperable and are implemented in the vessel's health and safety hazards to prevent falls overboard. 46 CFR 140.510(a)

Employers must maintain records of chemical testing as provided in 49 CFR 40.333 and must make these records available to Coast Guard officials upon request. Records of current drug/alcohol program where not available for inspection. 46 CFR 16.260 (a)

A skiff used as a survival craft does not require Coast Guard approval but must: Be marked in accordance with §§ 199.176 of this chapter. The number of persons for with the boat is equipped must be clearly marked, preferably on the bow, in permanent characters. The skiff is not marked with the number of people it is equipped for. 46 CFR 141.330(f)

Portable fire extinguisher monthly inspections required by NFPA 10 may be conducted by the owner/operator, PIC, or crew. Monthly inspection of portable extinguishers are not completed or logged. 46 CFR 142.240(a)(1)

Each lifebuoy with a floating light must have a lanyard 3 feet in length but not more than 6 feet in length, securing the light to the body of the ring buoy. Found wasted lanyard on starboard ring buoy. Replace in kind. 46 CFR 141.360(C)(4)

Each lifebuoy must be approved under approval series 160.050 or 160.150. Found port and starboard lifebuoys on bridge wings missing SOLAS reflective material. Replace reflective material on ring buoys. 46 CFR 141.360(b)(1)

A towing vessel must carry adequate and up-to-date nautical charts and nautical publications for the intended voyage. Navigation chart last updated in 2017. Provide new navigation chart. Provide notice to mariners for updates to navigation chart. Provide 2020 and 2021 tide charts. Provide 2020 Coast Pilot. 46 CFR 140.705(a)

A towing vessel of 26 ft or more and more than 600 hp engaged in commercial service shall have an operational AIS. AIS at operating station showed a fault and the master was unable to use the AIS. Repair or replace as original. 33 CFR 164.46 (b)(1)

Marine employers shall establish programs for the chemical testing for dangerous drugs on a random basis of crewmembers on inspected vessels. The selection of crewmembers for random drug testing shall be made by a scientifically valid method, the minimum annual percentage rate for random unannounced drug testing shall be 50 percent of covered crew members. 46 CFR 16.230

Each towing vessel must have a reliable means to provide notification for high bilge levels, visible and audible at the operating station. High bilge alarm in engine room found inoperable. Repair or replace in kind. 46 CFR 143.230

All owners and operators of an existing towing vessel must have a valid COI prior to the specified phase-in period. This vessel is one of the 25% of the vessels that has not obtained a valid COI. The vessel shall be made available for a Coast Guard COI inspection.

DEFICIENCY CODE 01199 All owners and operators of an existing towing vessel must have a valid COI prior to the specified phase-in period. This vessel is one of the 25% of the vessels that has not obtained a valid COI. The vessel shall be made available for a Coast Guard COI inspection within 90 days. If an inspection is not completed prior to this date, the action for this deficiency shall be changed to a 60 (Rectify deficiencies prior to movement) and the vessel may no long be permitted to operate. REPLACE THE INTERIOR FLUORESCENT LIGHTS WITH PROPER MARINE FIXTURES OR PRE-VIOUSLY-APPROVED FIXTURES AND REPLACE WIRES LEADING TO THESE UNIT WITH AP- PROPRIATE WIRE LEADS. THESE LIGHTS ARE TO BE DISCONNECTED FROM THEIR POWERSOURCE UNTIL SUCH REPLACEMENTS ARE COMPLETED.

MAKE SOLID AND SECURE THE COVERS TO ALL ON BOARD ELECTRIC HEATERS.

PROVIDE A PROPER BRACKET FOR THE FIRE EXTINGUISHER IN THE PILOT HOUSE OR REPLACE THE FIRE EXTINGUISHER WITH ONE THAT WILL FIT THE BRACKET. THIS ITEM IS TO BE COMPLETED PRIOR TO OPERATING THE VESSEL OR THE DATE OF COM- PLIANCE, WHICHEVER COMES FIRST.

DISASSEMBLE THE STEERING SYSTEM AS DIRECTED TO FACILITATE INSPECTION.

REMOVE THE OIL FROM THE ENGINE ROOM BILGE.

PROVIDE GUARDS FOR THE MAIN ENGINE SHAFT COUPLINGS.

PROVIDE EVIDENCE AS TO THE CAPACITY OF THE VESSEL'S MARINE SANITATION DEVICE.

PROPERLY WIRE THE VESSEL'S MARINE SANITATION DEVICE.

PROPERLY WIRE THE GENERATOR ALARM CABLE, REMOVING THE OPEN SPLICE.

REMOVE THE HALYARD CLIPS FROM THE LIFE RINGS AND ATTACH THE LINE PERMANENTLY TO THE LIFE RINGS.

REPLACE THE LIFE RING THAT HAS THE WATER LIGHT ATTACHED.

PROPERLY REPAIR THE VESSEL'S ANCHOR LIGHT.

THE VESSEL'S DOCUMENT DOES NOT HAVE ITS CURRENT STICKER. MAKE NECESSARY ARRANGEMENTS TO PROVIDE THE PROPER STICKER.

PLACE SKIN VALVES ON THE THROUGH HULL FITTINGS FOR THE VESSEL'S KEEL COOLERS AFTER SUBMITTING PLANS FOR APPROVAL. YOUR VESSEL IS RESTRICTED IN IT'S OPERATION BETWEEN MILES 459 AND 486 ON THE OHIO RIVER. COMPLETION REQUIRED AT NEXT VESSEL DRYDOCKING, SCHEDULED OR REPAIR WHICHEVER IS FIRST.

PROVIDE ADEQUATE UNDERDECK STIFFENING IN WAY OF MAIN RUDDER HYDRAULIC DECK FITTINGS AND RENEW DECK PLATE AS NECESSARY.
