Each inlet or discharge pipe that penetrates the hull below a line drawn parallel to and at least 150 millimeters (6 inches) above the deepest load waterline must have means to prevent water from entering the vessel. MI observed throughhull fitting for starboard aft engine room bilge did not have valve or metallic fitting. Install valve and replace with metallic fitting to the satisfaction of the MI. 46 CFR 179.350(c)

All other vessels must undergo an inspection for certification as specified in § 176.404 and annual inspection as specified in paragraph (b)(1) of this section. Annual inspection. Your vessel must undergo an annual inspection within the 3 months before or after each anniversary date. If the annual inspection reveals deficiencies in your vessel's maintenance, you must make any or all repairs or improvements within the time period specified by the OCMI. Vessel has not made the required improvements of their fire drill logs and has surpassed the required time frames set out. Vessel is restricted from carrying passengers until requirements have been met. 46 CFR 176.500(b)(1)(ii)

Lakes, bays, and sounds, and rivers routes. A vessel on a lakes, bays, and sounds, or rivers route must carry: (1) Three hand red flare distress signals approved in accordance with § 160.021 in subchapter Q of this chapter, or other standard specified by the Commandant; and(2) Three hand orange smoke distress signals approved in accordance with § 160.037 in subchapter Q of this chapter, or other standard specified by the Commandant. Vessel had expired flares on board. Replace expired flares to the satisfaction of the Marine Inspector. 46CFR180.68(b)

All hardware shall be brass, bronze, or stainless steel, and of the approximate size indicated by the drawings. Steel hardware, protected against corrosion by plating, is not acceptable. Snap hook springs shall be phosphor bronze or other suitable corrosion-resistant material. One lifejacket was missing its corrosive resistant clip. Repair the lifejacket to the satisfaction of the Marine Inspector. 46CFR180.71(c) 46CFR160.055-3(i)

A vessel not required to have a power driven fire pump by § 181.300 must have at least three 9.5 liter (21/2 gallon) buckets, with an attached lanyard satisfactory to the cognizant OCMI. Vessel was missing the required lanyards. Install lanyards to the satisfaction of the Marine Inspector. 46CFR181.610

The words “FIRE BUCKET” must be stenciled in a contrasting color on each bucket. Not all fire buckets were properly labeled. Label all fire buckets to the satisfaction of the Marine Inspector. 46CFR181.610

Firefighting drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include the following information: (1) Date of the drill and training; and (2) General description of the drill scenario and training topics. Vessel did not have any logs for drills. Vessel conducted fire drills during inspection, log drills to the satisfaction of the Marine Inspector. 46CFR185.524(d)

Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include the following information: (1) Date of the drill and training; and (2) General description of the drill scenario and training topics. Vessel did not have any logs for drills. Vessel conducted drills during inspection, log drills to the satisfaction of the Marine Inspector. 46CFR185.520(f)

A vessel must have two independent means of controlling each propulsion engine. Control must be provided for the engine speed, direction of the shat rotation, and engine shutdown. Vessel does not have second independent means of controlling propulsion engine. Install second independent means of controlling propulsion engine to the satisfaction of the Marine Inspector. 46CFR182.620(a)

Except as provided in paragraph (c) of this section, a vessel must be provided with an auxiliary means of steering that is: (1) Of adequate strength; (2) Capable of moving the rudder from 15 degrees on one side to 15 degrees on the other side in not more than 60 seconds with the vessel at one-half its maximum service speed ahead, or 7 knots, whichever is greater; and (3) Controlled from a location that permits safe maneuvering of the vessel and does not expose the person operating the auxiliary means of steering to personnel hazards during normal or heavy weather operation. Vessel has no means of a secondary means of steering on board. Install a seoncdary means of steering to the satisfaction of the Marine Inspector. 46CFR182.620(a)

An exposed hazard, such as gears or rotating machinery, must be properly protected by a cover, guard, or rail. Vessel has exposed rotating machinery when you first enter the engine room. Install a protective guard to the satisfaction of the Marine Inspector. 46CFR177.960

Each life jacket, immersion suit, and ring life buoy must be marked in clearly legible block capital letters with the vessel's name. Vessel had multiple lifejackets that did not have the vessel’s name. Operator satisfactorily marked those life jackets during the inspection. 46CFR185.604(b)

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. Vessel's aspirin was expired. 46 CFR 184.710

An exposed hazard, such as gears or rotating machinery, must be properly covered by a cover, guard, or rail. Shafting from Engine auxiliary to para-sail winch has no adequate guard. Belt Pullies on front of Engine are not adequately guarded. Provide Guards for winch shafting and Engine belt pullies prior to April 1st, 2020.46 CFR 177.960

Both of the vessel's high water alarms failed to indicate at the operating station. Vessel must correct issue with the high level alarms prior to carrying passengers.

Add all crew in safety sensitive position to drug and alcohol testing program. 46CFR16

Provide first aid kit meeting the requirements of 46CFR160.041
