Each tank vessel must undergo dry dock, internal structural, and cargo tank internal examinations at intervals prescribed in 46 CFR Table 31.10-21(a). Dry dock, internal structural, and cargo tank internals for NRC VALOR expire after 30 March 2024, as stated on your COI. Vessel will be in transit from St. Croix to Rhode Island in order to conduct credit dry dock, but dates will expire in route. During transit, vessel not be authorized to carry cargo or freight. On or before your arrival, schedule your credit dry dock. Local OCMI will decide manner in which this deficiency will be cleared.

Vessel has at least one tank overfill sensor switch per cargo tank that is designed to activate an alarm when its normally closed contacts are open. Repair alarm to working condition.

The contents must be marked on the containers, and the containers must be labeled in accordance with DOT requirements for flammable liquids.

The shipper and/or owner of the barge shall be responsible for the installation of the required placards or signs and maintenance of them. Relabel both sun faded DANGER KEEP OUT sign.

A CO2 system must be installed in all paint lockers. Replace/ repair extinguisher hose is cracked.

Emergency instructions must be conspicuously displayed at each muster station for life raft. Replace dilapidated sticker at muster station.

Warning signs shall be placed that it will be visible on all sides. Replace dilapidated sign on the Star board side of barge.

Life jackets must be readily accessible. Repair hinge to the life jacket locker.

High level alarm be identified in black letters at least 2 inches high on white back ground. Post signage or paint signage for high and over fill alarms.

Any vessel of at least 5 net tons owned by citizens of the U.S. is eligible for documentation. Provide updated docs. 1. Barge Security Officer 2. Updated VGP 3. Security Annual Exercise 4. Vessel Response Plan 5. SOPEP 6. P/V testing cert. 7. Cargo Piping Hydro cert.

All values and fittings shall be of material, design and manufacture for the intended service. Repair broken bolt on valve 1P. Repair broken weld on hinge stop of hatch to tank 1S.

Red warning signal shall be so placed that it will be visible on all sides. Grey flag needs to be painted red.

A vessel must comply with the applicable requirements for a station operators license and safety installation certificates to be issued by the FCC as set forth in 47 CFR part 80. Vessel is required to have a active safety station license and operators license.

Each life jacket carried on a vessel on a oceans route must have a life jacket light. Replace broken lights on life jackets.

At least one ring life buoy must be fitted with a lifeline.

All exposed and dangerous places such as gears and machinery shall be properly protected with covers and guards. Re-install guard around slop tank pump.

A vessel or object being towed, shall exhibit sidelights and a stern light. Install batteries in Navigation lights.

Each vessel must have a stability letter issued by the Coast Guard before the vessel is placed into service. This letter sets forth conditions of operation. Document not found on board or able to be located electronically by vessel representative. 46 CFR 31.10-30

Each nonmetallic transfer hose must not burst, bulge, leak, or abnormally distort under static liquid pressure at least 1.5 times the maximum allowable working pressure. MI observed previous annual test of cargo piping was tested pneumatically. 33 CFR 156.170 (c)(1)(iii)

Each pressure vessel listed on the COI must be thouroughly examined externally every 5 years. In addition, each pressure vessel listed on the COI that is fitted with a manhole or other inspection opening so it can be satisfactorily examined internally, must be opened for internal examination every 5 years. The two pressure vessels listed on the NRC Valor COI were due for an internal and external inspection in January 2020. The pressure vessels shall be opened for an internal examination by a USCG marine inspector prior to operational service. 46 CFR 31.30-1 and 46 CFR 61.10-5(b)

Each tank vessel that operates in salt water must undergo drydock, internal structural, and cargo tank internal examinations, with two examinations within any five year period. No more than three years may elapse between any two examinations. Vessel's drydock, internal structural, and cargo tank internal examinations have lapsed. Prior to operations complete required examinations. - 46CFR31.10–21

On a tank barge, each cargo tank must be equipped with an overfill device. The overfill device for the #4 Stbd tank was alarming in normal condition. The crew had placed a metal device at the top of the overfill pull test switch to keep the alarm from going off. If this tank is to be used before the next dry docking period in April 2019, the crew must continually monitor the level of the tank while filling. 33CFR155.480(a)(2)
