The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended. Emergency diesel generator failed to start due to the remote fuel shut off valve being closed. Engineer opened the valve and the generator was tested and verified to be in good working condition.

Resolution: Vessel's chief engineer opened the remote fuel shut off valve and started emergency generator multiple time to verify functionality.

Fire-extinguishing appliances shall be kept in good order and be available for immediate use at all times. When testing emergency fire pump, PSCO observed the port side bridge wing hydrant was inoperable due to the connection being incompatible with the fire hose.

Paint lockers and flammable liquid lockers shall be produced by an appropriate fire-extinguishing arrangement approved by the administration. PSCO observed paint and flammable liquids being stored in the stevedore storage and water closet without fire-extinguishing arrangements.
