The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended. Emergency diesel generator failed to start due to the remote fuel shut off valve being closed. Engineer opened the valve and the generator was tested and verified to be in good working condition.

Fire-extinguishing appliances shall be kept in good order and be available for immediate use at all times. When testing emergency fire pump, PSCO observed the port side bridge wing hydrant was inoperable due to the connection being incompatible with the fire hose.

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship and persons onboard. PSCO found on deck containments with missing or wasted mechanical scuppers which would allow fuel oil and hydraulic fluid to spill on deck.
