At each initial and subsequent inspection for certification, re-inspection and at every other vessel inspection the marine inspector shall require that all observed unsafe practices and hazardous situations be corrected. Vessel pilothouse shutdown for starboard engine was unable to shutdown starboard engine. Make repairs and demonstrate to attending marine inspector the pilothouse shutdown for starboard engine is able to shutdown starboard engine. 46 CFR 176.25-50(a) Old T

A device must be provided to automatically shut down power ventilation serving the protected space and engines that draw intake air from the protected space prior to release of the extinguishing agent into the space. Vessel starboard engine did not shut down during simulated CO2 activation via pressure switch. Make repairs and demonstrate to the attending marine inspector that both engines shut down during simulated CO2 activation. 46 CFR 181.410(b)(10)

On a vessel of at least 26 feet in length, a visual and audible alarm must be provided at the operating station to indicate a high water level. High water level alarm for aft compartment is not giving a visual alarm. Demonstrate to attending marine inspector visual and audible alarm for high bilge level in all required spaces. 46 CFR 182.530(a)

A vessel must comply with the structural design requirements of one of the standards listed. In the midship compartment under port side of fuel tank pitting at 5/64 inch. Provide hull thickness to verify wastage. Wastage beyond 25% requires repair proposal to be approved and complete prior to next credit drydock. 46 CFR 177.300

A vessel must comply with the structural design requirements of one of the standards listed. Amidship on both port and starboard sides, small individual holes approximately 1/16" in diameter are present on the deck. Effective temporary repairs in place now, make permanent prior to next credit drydock. 46 CFR 177.300

Rail courses or the equivalent must be installed between a top rail required by paragraph (a) of this section, and the deck so that no open space exists that is more than 305 millimeters (12 inches) high except on passenger decks of a ferry or of a vessel on an excursion trip the following must be installed: (i) Bulwarks;(ii) Chain link fencing or wire mesh that has openings of not more than 4 inches in diameter; or (iii) Bars, slats, rail courses, or an equivalent spaced at intervals of not more than 100 millimeters (4 inches) The vessel has bulwarks that decrease in height from the bow to the stern, thus increasing the distance between the top of the bulwarks and the required top rail. The decrease in height of the bulwark creates an opening that exceeds 4 inches on the port and starboard quarters and the stern of the vessel. Install courses to decrease the opening between the top rail and bulwarks to no more than 4 inches. If courses cannot be immediately installed submit a mitigation plan to be reviewed and approved by the attending USCG Marine Inspector until courses are installed. 46CFR177.900(g)

At each initial and subsequent inspection for certification, the owner or managing operator must be prepared to conduct tests and have the vessel ready for inspection of its fire protection equipment. Master and crew were not aware of testing or maintenance requirements for the heat detector. Create and adhere to a maintenance plan based on manufacturer's instructions. 46CFR176.810(a)

Where a flexible vent pipe section is necessary, suitable flexible tubing or hose having high resistance to salt water, petroleum oils, heat and vibration, may be used. Port fuel tank vent hose is rubbing on vertical support frames. Clean up and install chaffing gear. 46CFR182.450(g)

Fuel tanks must be adequately supported and braced to prevent movement. The supports and braces must be insulated from contact with the tank surfaces with a nonabrasive and nonabsorbent material. Metal band securing fuel tank is causing chaffing in areas discussed with operator. Clean up and install nonabsorbent chaffing gear. 46CFR182.440(b)(3)

A fixed gas fire extinguishing system may protect more than one space. The quantity of extinguishing agent must be at least sufficient for the space requiring the greatest quantity as determined by the requirements of this section. Air gaps exist where cables penetrate the bulkhead between the engine compartment and fuel tank compartment. Eradicate air gaps in bulkhead. 46CFR181.410(a)(4)

Gauges to indicate engine revolutions per minute, jacket water discharge temperature, and lubricating oil pressure must be provided for all propulsion engines installed in the vessel. The gauges must be readily visible at the operating station. The oil pressure gauge for the starboard engine is inoperable. Repair gauge. 46CFR184.402(c)

Except on a vessel limited to daylight operations, the compass must be illuminated. Master and crew were unaware of how to illuminate the compass. Provide proof of or install illuminated compass.

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards. Install back cover for switchboard at operating station and cover opening on front of switchboard as discussed. 46CFR183.200(b)

The vessel owner or operator must pay the annual vessel inspection fee each year on or before the vessel's user fee anniversary date, unless the fee has been prepaid under 2.10-105 of this subpart. 46CFR2.10-101(b)

A vessel shall be prepared to test the steering systems of the vessel and make them available for inspection to the extent necessary to determine that they are in suitable condition and fit for the service intended. Vessel was unable to demonstrate proper operation of its primary steering system. Repair and prove proper operation.

A qualified servicing facility shall conduct inspections and tests of a fixed gas fire extinguishing system. Vessel's fixed gas system had been serviced and tagged but did not have the fire servicing certificate on board. Provide fire servicing certificate.

Vessel should be compliant with the standards acceptable to the cognizant OCMI as good marine practice. Vessel's steel anchor is stored in direct contact with the aluminum hull and causing initial corrosion. Provide a protective material between the anchor and the hull to prevent future corrosion.

An exposed hazard such as gears or rotating machinery, must be properly protected bya cover, guard, or rail. One of the vessel's machinery guards is attached with plastic zip-ties. Provide proper metal supports for the engine's machinery guard.

Connections must be made to battery terminals with permanent type connectors. Connections are made with wing nuts. Remove wing nuts and install hex nuts with lock washers.

Employers must ensure that they and their crew members meet the requirements of part 16. Provide proof of enrollment of all crew members in random chemical testing program.

On a vessel of at least 26 feet in length, a visual and audible indicator must be provided at the operating station indicate high water level in each normally unmanned space with a through-hull fitting, machinery space, shaft alley, space with non-watertight closure and other spaces subject to flooding. Vessel has one visual indicator for all required spaces. Install and demonstrate operation of audible and visual indicators for each space required.

Each battery used in lifesaving equipment must be replaced during the annual inspection if the expiration of the battery has passed or the battery is without an expiration date. Replace the battery in the life-ring floating light.

Nonmetallic flexible hose from starboard engine to keel cooler showing signs of wear. Renew hose with approved type.

Remove or terminate all dead end electrical wiring throughout vesel.

Provide repair proposal for hole and pitting in anchor storage area on starboard side. Complete repair following USCG approval of proposal.

Provide repair proposal for pitting/wastage along keep, notably found in midsection storage. Crop and renew once proposal is USCG accepted.

Make available for inspection keel plating underneath fuel tank.

1/4" diameter hole found inside of the anchor storage locker. The hole penetrates the hull on the starboard side, 3 feet above the waterline and 2 feet aft of the bow. Additional pitting corrosion found within 5 inche radius of the hole inside of the compartment. Submit repair proposal to OCMI for review by the next credit dry-dock.

Replace and reseat the watertight hatch gasket for the void space located forward of the main cabin. (number 1 void space)

Make proper connections to the grounding wire on the fuel tank portside fuel fill hose. Replace corroded grounding connections on the fuel fill grounding conductors.

Replace corroded fuel hose connections penetrating the number 2 void space and engine room bulkhead.

Replace blocked flame screen for the fuel tank located on the portside.

Replace deteriorated grab line and life line on the ring buoy. Apply retro reflective tape to the ring buoy or replace in kind.

Replace broken glass for the CO2 engine room fire extinguishing system remote activation located at the operating station.

Replace inoperable starboard running light.

Passenger count reduced to 18 adults based on the amount of lifejackets onboard in a servicable condition.

Adult lifejackets are required to be stowed separate from children's lifejackets. Separate adult lifejackets from children's lifejackets and label the children's stowage area with minimum 2 inch lettering for total number and "children's lifejackets."

Clean paint off of grounding conductors attached to the main engine. Conductors at the connection should be metal to metal.

Provide adequate support for the exhaust cooling line on the starboard main engine. The flexible hose connected to the main engine attaches to a steel elbow that is not supported.

Clean corroded terminal connections for postive battery conductors located at the battery switch bus bar.
