All vessels shall be provided with bilge pumps in accordance with Table 182-10(a). The two required fixed power pumps shall be self priming and shall each be driven by different sources of power. Vessel was unable to provide evidence of a working bilge pump. Vessel's fire pump was unable to obtain suction when using the fire pump as a bilge pump. 46 CFR 182.25-10(a) OLD - T

The Bridge-to-Bridge Act and the regulations of this part apply to the following vessel in the navigable waters of the United States: (a) Every power-driven vessel of 20 meters or over in length while navigating. Vessel did not have a Bridge-to-Bridge Certificate on board. Technician will be coming next week to inspection radio installation, once complete supply proof of inspection and certificate. 47 CFR 80.1001

The lights prescribed in these Rules (Subpart C) shall have an intensity as specified in Annex 1 to these Rules (33 CFR part 84), so as to be visible at the following minimum ranges. Vessel stern light and lower red light for the "restricted in their ability to maneuver" were not operable. Have technician correct issue and provide report and proof of correction. 33 CFR 83.22

Exhaust piping shall be properly supported by non-combustible hangers or blocks. Port main exhaust had a broken hanger in the over head. Re-weld and provide photo evidence of correction. 46 CFR 182.20-15 (old T)

Each hydraulic release used in the installation of any liferaft, inflatable liferaft, lifefloat, or bouyant apparatus must meet Subpart 160.062 of this chapter. Inflatable liferaft painter was not connected to the weak-link. Corrected on the spot by crew. 46 CFR 180.20 (e)

Adequate emergency lighting automatically actuated upon failure of the main lighting system shall be fitted along the line of escape in vessels having lounge areas below the main deck as per subpart 177.30-7 of this subchapter. MI observed no emergency lighting in crew accommodation. 46 CFR 184.30-5

In no case shall the clearance between the bottom of the hand portable fire extinguisher and the floor be less than 4in. MI noted that the pilot house fire extinguisher was mounted with the base in contact with the deck plate underneath it. 46 CFR 175.600(k)(1) NFPA10-6.1.3.8.3

Equipment and material that is required by this subchapter to be approved or of an approved type, must have been manufactured and approved in accordance with the design and testing requirements in subchapter Q (Equipment, Construction, and Materials: Specifications and Approval) of this chapter or as otherwise specified by the Commandant. MI observed incorrect bracket installed for pilot house fire extinguisher. 46 CFR 175.800(a)

Each portable fire extinguisher on a vessel must be of an approved type. The minimum number of portable fire extinguishers required on a vessel must be acceptable to the cognizant Officer in Charge, Marine Inspection, but must not be fewer than the minimum number required by Table 181.500(b) and other provisions of this section. The galley does not have 40-B:C PFE and a 40-B:C is needed in the vicinity of the machinery space exit. 46 CFR 181.500(a)

Each vessel must have the following: (1) Marine charts of the area to be transited, published by the National Ocean Service, U.S. Army Corps of Engineers, or a river authority that— (ii) Are currently corrected. The marine charts onboard are not up to date. 33 CFR 164.33 (a)(1)

Except as otherwise provided in this section, rails or equivalent protection shall be installed near the periphery of all weather decks accessible to passengers and crews. MI noted missing chain courses on both port and starboard sides of the upper deck. 46 CFR 177.35-1(a)

Vessels operated solely within 20 nautical miles of land must be equipped with a VHF-DSC radiotelephone installation meeting the requirements of § 80.1101(c)(2), except that a VHF radiotelephone installation without DSC capability is permitted until one year after the Coast Guard notifies the Commission that shore-based sea area A1 (see GMDSS Areas and Search and Rescue Areas. Navigation Center (uscg.gov)) coverage is established (see Rescue 21 Distress System Coverage | Navigation Center (uscg.gov)). Vessels in this category must not operate more than 20 nautical miles from land. Sea Area 1 was established on 20JAN2015, and the vessel's GPS is not on the emergency power circuit which causes the VHF's Digital Selective Calling (DSC) capability to be disabled when the vessel experiences a loss of power. 47 CFR 80.905(a)(1)

Life preservers shall be distributed through the upper part of the vessel in protected places convenient to the persons on board. MI noted life jackets stored in non-easily accessible location.

A vessel must be provided with a main steering gear that is capable of moving the rudder from 35° on one side to 30° on the other side in not more than 28 seconds. Rudder stuck to port preventing vessel maneuverability. Repair to the satisfaction of attending Marine Inspector.

Remove caps on both main engines and replace with approved means. 46 CFRR 183.10-10

Cable entering bottom of engine room panel on port bulkhead needs to be repaired. 46 CFR 183.10-30

Junction box over work table in forward engine compartment needs to be repaired and secured. 46 CFR 183.10-30

Life buoy on main deck plug missing needs to be replaced. 46 CFR 180.70

Vessel missing list of employees enrolled in drug consotium. Place list of enrolled employees on vessel with DAPI documentation. 46 CFR 16.230(d)

Fire pump does not have gage. Install gage at fire pump outlet. 46 CFR 181.10-1(c)

Vessel's magnetic compass does not have deviation table. Obtain appropriate deviation table for compass. 46 CFR 184.402 705- To be completed at drydock.

During fire drill the fire pump failed to start. Cause as stated by vessel Master related to the breaker.

Watertight door leading from aft deck to galley found with bottom right dog (looking aft) no longer providing any watertight seal.

Four (4) chemlights fround either missing or expired.

Records of enrollment in a drug testing consortium and random drug test results for crewmembers not available for examination.

VRP available for review on-board was valid through June 23, 2014.

Stability letter not available for examination.

Vessel did not make pressure vessels available for internal examination and testing.

Remove dead end wire on bridge aft stbd corner

Install Public Address speaker in lower accomodation space

Provide light for magnetic compass.

Provide non-expired EPIRB battery.

Make bilge visual alarm function properly. Light constantly on.

Make permanent repairs at next drydock to hydraulic steering pipes located in ballast tank aft of engine room. All repairs to be cleared by attending USCG marine inspector. No commercial and/or other operations permitted until this 835 is cleared by USCG marine inspector.

Master was unable to test the fire detection system in the E/R or the accommodation space. Required to prove proper operation of the fire and smoke detectors in the vessel.

Provide a proposal to MSU Galveston, for repair of the vessel and location to where the repairs are to be completed.

Vessel is to remain moored at Barron's Marine, Freeport, TX.

Remove unapproved wiring from port and starboard battery boxes in the engine room.

Repair leak on the ballast piping valve controlling aft bulkhead E/R.

Repair leaking hydraulic fittings on machinery in centerline port area of engine room.

Remove oily water from bilge.

Replace PVC and plastic parts on hydraulic tank sight glass fittings in aux. space with approved materials.

Provide electrical mats in front of all switchboards.

Repair and prove operation of the ballast pump gage.

Provide stamped approved plans for crane and associsted piping installation.

Provide adequate emergency lighting for engine room emergency egresses.

Provide two fire ext. in the vicinity of the engine room.

MAKE PERMANENT REPAIRS TO THE FOREPEAK AREA TO THE SATISFACTION OF THE ATTENDING COAST GUARD MARINE INSPECTOR.

MAKE LOWER FOREPEAK HOLD AVAILABLE FOR INSPECTION BY ATTENDING COAST GUARD INSPECTION.

VESSEL TO PROCEED TO CONRAD INDUSTRIES, SHIPYARD FOR REPAIRS.

MAKE REPAIRS AND TEST FOR CG INSPECTOR S/S GEN OVERSPEED TRIPS.

OBTAIN TIDAL CURRENT TABLES.

CROP OUT AND RENEW WASTED ANGLES IN OVERHEAD AND RECONNECT OVERHEAD TO ANGLES OUTBOARD OF LOWER ACCOMODATION AIR CONDITIONER IN CREW SPACE.

CROP OUT TWO (2) FRAME MEMBERS THAT ARE FRACTURED IN THE ENGINE ROOM, PORT SIDE. AT LEAST THREE (3) FOOT SECTION SHOULD BE CROPPED OUT. REWELD NEW INSERTS LIKE ORIGINAL IN CROPPED OUT AREA.

REPAIR TACHOMETER ON PORT S/S GENERATOR; RESTORE TO ORIGINAL CONDITION.
